Wenn es noch eines Beleges bedurfte, dass Gesundheit im Zeitalter der Globalisierung ein Politikfeld für
Global Governance ist, dann wurde er spätestens mit dem jüngsten Ausbruch des Ebola-Fiebers in Guinea,
Liberia und Sierra Leone geliefert. Die Krankheit bedrohte nicht nur die Menschen und Volkswirtschaften der
drei Länder und ganz Westafrikas, sie stellte auch ein potenzielles Risiko für die globale Sicherheit und
Stabilität dar. Dass Gesundheitsthemen auch auf globaler Ebene behandelt werden müssen, wurde schon
durch die Ausbreitung von HIV/AIDS in den 1980er Jahren deutlich. Der UN-Sicherheitsrat befasste sich im
Jahr 2000 zum ersten Mal mit dem Thema.
Das Politikfeld Gesundheit wird von einer nahezu unüberschaubaren Zahl von Akteuren auf lokaler, regiona-
ler, nationaler und globaler Ebene bearbeitet, die finanziell nicht nur sehr unterschiedlich ausgestattet sind,
sondern auch unterschiedliche Ziele verfolgen und deren Politiken unterschiedliche Reichweiten haben. Da
gibt es zunächst die nationalen Gesundheitsministerien, bilateralen Agenturen der Entwicklungs-
zusammenarbeit und NGOs; globalen Anspruch haben das UN-System mit der Weltgesundheits-
organisation (WHO), UNICEF, dem Bevölkerungsfonds UNFPA und UNAIDS und schließlich die multi-
lateralen Entwicklungsbanken, allen voran die Weltbank. Daneben agieren hybride globale Gesundheits-
initiativen wie der Global Fund, die Impfallianz GAVI und UNITAID. Große philanthropische Stiftungen wie die
Bill & Melinda Gates Foundation, die Rockefeller Foundation und der Wellcome Trust spielen bei der
Herstellung einer Global Health Governance mittlerweile ebenfalls eine wichtige Rolle. Beachtenswerte
Akteure sind schließlich globale zivilgesellschaftliche und Nichtregierungsorganisationen wie Médecins sans
Frontières, Oxfam und CARE International sowie, last but not least, die multinationalen Unternehmen der
Pharmaindustrie, die Vereinigungen der medizinischen Berufe und akademische Institutionen, die sich um
die Qualifizierung von Medizinern kümmern.
Global Health Governance ist Politik in einem Mehrebenensystem, in dem sich Akteure mit unterschiedli-
chen Legitimationsgrundlagen und Rechenschaftspflichten sowie teilweise divergierenden Interessen
koordinieren oder eben nicht. Die Sozialwissenschaften befassen sich seit geraumer Zeit mit den sich
daraus ergebenden Fragen; seit 2007 gibt es eine Online-Zeitschrift namens Global Health Governance.
Diskutiert wird hier und in der Literatur u.a. welche Rollen einzelne Akteure und Akteursgruppen spielen,
welche Interessen sie haben, welche Interessen sich auf welcher Ebene durchsetzen, wie es zu einem Aus-
gleich kommen kann, welche institutionellen Arrangements und Normen entstehen und wie sich Global
Health Governance vor dem Hintergrund einer sich intensivierenden Globalisierung entwickelt.
Die vorliegende Bibliographie gibt ohne jeden Anspruch auf Vollständigkeit einen Überblick über die seit
2000 erschienene Literatur, die wesentliche Fragen von Global Health Governance berühren.

In Deutschland fehlt es zusehends an Gesundheitsfachkräften. Daher wächst das Interesse, solche
Kräfte auch in Entwicklungsländern zu rekrutieren. Um damit verbundene Probleme für die
Herkunftsländer zu vermeiden, orientiert sich die Bundesregierung wie andere Industrieländer auch an
einem Verhaltenskodex, den die Weltgesundheitsorganisation (WHO) 2010 verabschiedet hat. Dieser
Kodex ist allerdings widersprüchlich: Er empfiehlt, auf die Rekrutierung aus bestimmten Ländern zu
verzichten, will aber gleichzeitig das Recht der Fachkräfte auf internationale Mobilität nicht beschränken.
Viele Industrieländer suchen diesen Widerspruch aufzulösen, indem sie sich an eine ebenfalls von der
WHO 2006 erstellte Liste von 57 Staaten halten, die einen »kritischen Mangel« an
Gesundheitsfachkräften aufweisen obwohl sie auf unzureichenden Daten und zweifelhaften Kriterien
beruht und die WHO klargestellt hat, dass die Liste eine genauere Länderanalyse nicht ersetzen kann.
Wie können Deutschland und andere Industriestaaten ihren steigenden Bedarf decken, ohne den

Since the 1990s the growing number of actors involved has considerably changed the field of global health
governance (GHG). Partnerships between states, international governmental organizations (IGOs) such as
the WHO, the pharmaceutical industry, and also civil society organizations have helped overcome conflicts
between the profit-driven production of medicines and the health needs of poor countries. However, they
have also led to a vast entanglement of responsibilities, with the WHO losing its profile as the central
authority on global health. In recent years, however, the impacts of the Paris Declaration on GHG and a
number of other processes have again strengthened the position of the WHO. (GIGA)

Eine wachsende Zahl neuer Akteure hat das Politikfeld der Global Health Governance (GHG) seit den
1990er Jahren erheblich verändert und zu einer Konzentration auf die Bekämpfung spezifischer Krankheiten
geführt. Partnerschaften zwischen Staaten, internationalen Regierungsorganisationen wie der WHO, der
pharmazeutischen Industrie, aber auch zivilgesellschaftlichen Organisationen haben geholfen, Konflikte
zwischen einer profitorientierten Herstellung von Medikamenten und Gesundheitsbedürfnissen in armen
Ländern zu überwinden. Allerdings führten sie zu einem unüberschaubaren Gewirr von Verantwortlichkeiten,
während die WHO an Profil verlor. Die im Jahr 2005 verabschiedete "Erklärung von Paris über die
Wirksamkeit der Entwicklungszusammenarbeit" hat auch GHG stark beeinflusst. in den letzten Jahren hat
die WHO ihre Rolle wieder stärken können. Sie übernahm zunehmend eine aktivere Rolle in der globalen
Gesundheitsdiplomatie. Dies geschah besonders durch die erfolgreichen Verhandlungen zweier

This article takes the state of health in the world today as the starting point for a backward look at the
trajectory that has led to our current position and speculation about prospects for improved global health
in the future. Our model of social development and its dominant value system, which has promoted
scientific progress but has also brought about great social, economic and health instability, is
interrogated. This leads to questions such as what it means to be healthy and what the practice of
medicine is about. Three potential scenarios for global health in the future are outlined. It is suggested
that deep introspection about our current value system is required to achieve a paradigm shift that could
reverse current trends and lead both to improvements in health globally and to less human insecurity.
The authors conclude that while we have the material resources to achieve ambitious goals we may lack
the moral and political will to do so. An expanded discourse on ethics and human rights as well as on
the limits of what is politically possible may provide the impetus to drive change towards an improved

This paper analyses the role of actors from developing countries in global processes of policy making and
governance. To systematically examine the channels of influence of Southern actors and the interactions in
global governance it develops the concept of interfaces. It differentiates between organisational, discoursive,
legal and resource-transfer interfaces in global governance. This approach is exemplified in the analysis of a
specific field of global governance, the global fight against HIV/AIDS. The paper examines the role of
Southern governments and non-state actors in the central organisations of global health, their influence in
debates and discourses on strategies to fight HIV/AIDS, and the financing mechanisms that were
introduced to fight HIV/AIDS in the developing world. It shows that albeit actors from Northern countries
dominate global governance in general, in particular areas the current institutional setting of global
governance provides significant opportunities for rather weak actors such as civil society organisations and
governments from the South to influence strategies and policies. (GIGA)
