Social Security including healthcare is historically a bismarckian system (occupation-
based)
For healthcare, historical occupational-based funds (agriculture, salaried and
independent workers) still exist but healthcare coverage is now universal and unified
In 1996, institution of the Yearly Social Security Law presented by the Government to
Parliament in October for the following year
- The law forecasts the financial trajectories for Social Security domains (health, family,
pensions, work injuries) and the policies that enable them
- For healthcare, a particular instrument called “Objectif National des Dépenses
d’Assurance Maladie” (National Objective of Healthcare Expenditures) is voted (200
billion euros for 2019, + 2,5%)

The Social Security Law and the ONDAM are presented by expenditure category,
not by objectives/programme
The « Quality and Efficiency Programs », implemented by the law (« loi
organique du 2 août 2005 relative aux lois de financement de la Sécurité sociale »),
in existence since 2008, aim to give a more programmatic view of the Social
Security/healthcare budgets
- They are directly inspired by the similar mecanisms set up, since 2006, for the
State budget (« loi organique relative aux lois de finances »), objectives and
indicators issued in the « Programmes annuels de performance and rapports
annuels de performance »
Aims of the « Quality and Efficiency Programs »:
- Provide transparent reporting on the quality and outcomes of Social Security
organizations
- Improve clarity of public policies by informing all decision makers and
stakeholders : members of Parliament, workers’ unions, Social Security
funds…
- Contribute to policy evaluations by measuring the performance of social and
healthcare policies
