This document outlines the coverage and procedures for a school accident agreement between a clinic and a school for the 2010-2011 year. It covers trauma injuries from external violent events that result in contusions, wounds, fractures or internal injuries. It details where students can get emergency treatment, how costs are covered depending on expenses, follow up care, reimbursement for treatment outside Santiago or abroad, hospitalization benefits, use of the emergency room for non-trauma issues, and registration requirements for parents and grandparents. It also describes a CPR and defibrillator training project for students and staff.
2. What does this cover? Any emergency TRAUMA, caused by external events in a violent way, resulting in contusions, wounds, fractures or internal injuries. 2 Trauma Emergency

4. How does it work? If the costs are under $350.000 pesos Clínica Las Condes covers the cost. If expenses are over $350.000 then the Zero Copayment goes into effect and CLC pays the difference not covered by your own health insurance: Fonasa, Isapre or International Insurance. 5. Follow-up Appointments All follow up visits take place at the Emergency Department, there is no charge for this. If you are referred to a Specialist Consultant, and it does not take place within the Emergency Department but at the specialty Outpatient Clinics area, this consultation is covered by the Zero-Copayment with e-vouchers (Imed). 6. If I’m outside Santiago or Abroad… The expenses can be reimbursed by providing the original bills or invoices and the physician’s diagnosis. You must notify CLC at 02 610 7777 within the following 48 hours. Reimbursement claims must be submitted within 30 days. See page 11. 6 Trauma Emergency

9. What other benefits do we get from CLC? Students can use the ER for a Non Trauma emergency. Parents and Grandparents can use the Emergency Service. 9 NonTrauma Emergency
10. How does it work? Only in Clínica Las CondesER room. Zero-copayment or a discount equivalent to 100% of the difference not covered by your health insurance: Fonasa, Isapre or International Insurance. If you don’t have health insurance or if the ER assistance is not included by your health insurance, this benefit will not apply. 10 NonTrauma Emergency
11. What should I do if the e-vouchers don’t work at the ER or I need to get reimbursed? Pay at the ER cashier. With the receipt go to your Isapre/Fonasa office at Level -1 in front of Café Moka, or get in touch with your complementary health insurance or international health insurance in order to get reimbursed. The difference not reimbursed by your health insurance, will be covered by CLC. For that, you have to go to COPAGO CERO Office at Level -1 nearby Café Moka. 11 NonTrauma Emergency
12. Hospitalization / Ambulance Hospitalization is NOT covered with the Zero-copayment benefit. However, you’ll get a 10% discount after personal health insurance coverage. Students receive a 50% discount in Ambulance and Helicopter in non-trauma emergency. Parents and Grandparents are not entitled to this discount. 12 NonTrauma Emergency
13. Where to register? Parents and Grandparents have to register at Convenio de Accidentes Office in Clínica Las Condes in order to be entitled to this benefit. This office is located at Level -1 Building A Contact Person: M. Teresa Zuñiga 13 NonTrauma Emergency
14. CPR and Defibrillator Project School received from CLC 2 Defibrillators (AED). These are located: One outside the Nurses’ office in MS building and the other at the back entrance of the Gym facing the cafeteria, by the swimming pool. All students enrolled in 9th grade are receiving training in basic CPR and AED use, also our staff and faculty members are being trained. 14
