Beginning August 1, 2012, the Homeless Services Assessment Center at Carpenter's Shelter will become the central assessment facility for homeless and at-risk single indiviudals.

- 1. Carpenter’s Shelter HSAC HOMELESS SERVICES ASSESSMENT CENTER
- 2. Homelessness in the City of Alexandria Currently looks like: 352 literally 49 families 213 single homeless adults • 51 adults • 330 sheltered • 88 minor • 157 single men • 22 unsheltered children • 56 single women
- 3. Current Diversion and Shelter Services • Contact Department of Community Families and Human Services Shelter Liaison • Are assessed for diversion and shelter services Single • Self refer to shelters - call daily to check on available bed space Adults • Case workers are also able to refer single adults to shelters
- 4. HSAC Purpose and Services 1. Establish an assessment center for single adults in order to assess for diversion and shelter services 2. Coordinate with DCHS for diversion services and emergency shelters for shelter placement 3. Maintain bed space availability list and refer clients to shelters 4. Connect to necessary services as soon as possible
- 5. Service Flow HSAC Shelter Services Diversion Services (Carpenter’s (OCS) Shelter, ACS, DV Shelter) Participate in Provide financial Subsidies will be If unable to program including Ultimate goal is Diversion from assistance and/or available to rapidly divert, will seek case management most appropriate homelessness case management re-house eligible shelter services and support housing for client to maintain housing clients services

HSAC Process Client expresses need and schedules an appointment with Program Coordinator During appointment, Program Coordinator and client complete Eligibility Assessment and Services Needs Assessment Based on outcome of Eligibility Assessment, client will Based on outcome of Services Needs Assessment, Program either be referred to DCHS for diversion services or to an Coordinator will make appropriate referrals emergency shelter for placement Once client has been referred to DCHS or emergency shelter, Eligibility Assessment will follow client and receiving agency/organization will conduct an in-depth intake assessment and begin case management
- 7. HSAC Key Objectives 1. Develop HSAC and all components 2. Cultivate and promote coordinated services among City of Alexandria homeless services providers 3. Reduce average shelter stay for single adults and families by 25% 4. Increase successful diversion of single adults and families by 25% 5. Effectively and efficiently utilize rapid re-housing and diversion funds as subsidies 6. Track all rapid re-housing and diversion funds in the City of Alexandria 7. Generate statistics to inform the community, grant makers, constituents, and policy makers
