Vocational expertise and partnership
THESE FIGURES WILL BEGIN TO MAKE MORE SENSE IF WE COMPARE THESE WITH OTHER KNOWN ACTIVITIES. FOR EXAMPLE, THE RISK OF DEATH IN CLIMBING UP TO THE BASE CAMP OF MOUNT EVERST IS 1 IN 1750, SCUBA DIVING IS 1 IN 34000, SKY FALL IS 1 IN 100,000, EVEN MOTOR CYCLE RACING IS 1 IN 1000 â SAFER THAN CHILD BIRTH IN POOR TRANSPORT SITUATIONS OF 1 IN 700. MOTHR HAS NO CHOICE..
THESE FIGURES AVAILABLE FROM THE WORLD BANK DATABASE INCLUDE NO OF PREGNANCIES â FERTILITY RATE.
I AM IN FAVOUR OF INCLUDING THESE, INSTEAD OF MATERNAL MORTALITY RATIO, AS FAMILY PLANNING BECOMES A MEASURE FOR PREVENTING MATERNAL DEATHS INSTEAD OF BEING A CULTURAL OR RELIGIOUS ISSUE.
The Tower burnt out in 35 minutes â mothers and babies will die of emergencies in 30 minutes â
There is no time to be lost â we need a fast track transfer from home to expert care . Concept of 3 delays.
âDelay kills â kill delaysâ
No time to be lost in fast track transfer to the best possible care â MERN concept â Maternity Emergency Resource Network
More and better trained professionals (firefighters) capable of giving basic resuscitation with basic equipment
THIS IS ABOUT A SILENT WAR WITH HEAVY CASUALTIES. 300,000 ARE JUST THE TIP OF THE ICE BERG. 6 MILLION DISABILITIES AND 7 MILLION CHILD DEATHS ARE RELATED, AND THEY CAN ALL BE PREVENTED THROUGH STRATEGIC LOW COST INITIATIVES. â FELLOW ROTARIANS YOU CAN BE A PART OF THAT ACTION
Equipment purchase is good , but is not a substitute for poor skills and non-availability of health workers. We need Calmed type VTT Global Grant as a priority ,in many areas.
We can â without diverting too much money from the GPEI initiative through vocational training and partnership
WE NEED A TEMPLATE OF ACTION. I PRESENT ONE CALMED- WHICH STANDS FOR COLLABORATIVE ACTION IN LOWERING OF MATERNITY ENCOUNTERED DEATHS, - MATERNAL AND CHILD DEATHS RELATED TO CHILDBIRTH, WAS BORN. IT HAS THRERE STRATEGIES - RELIES ON MULTIDISCIPLINARY NEEDS ASESSMENT .THE PROGRAMME IS THEN TAILORED TO THE NEEDS AND PRIORITIES. THIRDLY OUTPUT AND OUTCOME ARE MEASURED TO ASSESS IMPACT AND ENSURE SUSTAINABILITY

COMMUNITY MOBILISATION - -ASHAS AND ASHA TRAINERS PLAY MAJOR ROLES. FLIP CHARTS,FILMS,COMMUNITY WOMENS GROUPS
This slide shows summary of actions underatken by two global grants supported six VTT visits in two states of India with high maternal mortality. The programme covered 5 years, cost 100,000 USD, exceeded its target through training 39 master trainers, 218 doctors midwives and nurses, as well as over 500 ASHAs. These groups continue to train utilising the training materials left behind as legacies. Green arrow is for Meghalaya â GG 1988385
What are the resulting outcome ?
The output and outcome from these two global garnts are summarised in this slide. We calculated the number of mothers and babies lives saved, on a modeled estimate. We are aware that some of the improvements might have been contributed by initiatives already in place. Nevertheless, the improvements noted were verified .
We conclude that the Calmed programme and associated initiatives was a low cost, effective programme- only $ 400 of rotary foundation grant was associated with saving one mother, 5 babies and resulted in effective training of one extra doctor or nurse in the care of pregnant women in the target community.
There was more
Each pilot programme at its completion left three important legacies for future benefit. These included training resources like manuals, films, simulators etc, a strong partnership for future programmes on the lines I have described and a workforce of trained motivated doctors, nurses and midwives. What better way to ensure a sustainable highly effective programme than these legacies?
Now some photos
This slide shows the extent of external partnership as shown in these logos. World federation of obstetrics and gynaecology FIGO , global library of womens medicine Glowm, Laerdal Foundation , medical aid films MAF, Rotarian action group on population development RFPD and Rotarian doctors fellowship IFRD all contributed. WHO and MEDBOX are future partners as is the RCOG (logo not shown)
Proposed Vocational Alliance â Rotarians, Rotaractors, RAGs,Fellowships,Government and NGOs joined up action
For training resources, expertise and funding .
â FINALLY A SUCCESSFUL GLOBAL RESOURCE AIMED AT ZERO PREVENTABLE MATERNAL DEATH, SHOULD HAVE A COMMITTED TEAM, WITH RESOURCES AND WILL TO SUCCEED.- A GLOBAL PLATFORM OF ACTION
A ROAD MAP TO THIS GLOBAL PLATFORM SHOULD START WITH DETERMINED ROTARIANS, COLLABORATION, USE OF TECHNOLOGY AND EMPOWERMENT OF COMMUNITIESâ ABILITY TO RESPOND AT COUNTRT LEVEL
Alliance is both Global (for programme support) and Country level (implementation)
Rotary Vocational Alliance is a network of dedicated Rotarians aiming for design, implementation and scaling up of holistic maternal mortality reduction programmes based on helicopter views of community needs. Itâs assets include technical experts, NGOs, Academics and professional volunteers - doctors and nurses - , equipped with know how and resources for implementation
Question slide option #1
Our Vision â- Rotarians are people of action- this is so true
