17. Environmental Assessment CSR Tracer Coordinator: Rhonda Gentry, RN, BSN Location: JCMC 5200/5300 Unit Who Conducts a CSR? Patient Safety Coordinator. What is a CSR? The CSR (Continuous Service Readiness) provides hospitals with education and an internal audit determining if the staff are meeting healthcare regulations and following policies for safety of the patient.
