Consent and Release
On Tuesday, Oct. 19, 2010, the library will
be participating in Wyoming Snapshot: A
Day in the Life of Wyoming Libraries by
collecting information such as comments,
photographs and video. In connection with
this event, the library may issue
newsletters, update its website and produce
promotional materials. Photographs, videos
and testimonials of library patrons produced
for these purposes help the library
demonstrate the many benefits of library
use.
I give permission to the library to use my
image and voice in the library’s newsletter
and/or website and/or promotional materials. I understand that, given technology which allows
sharing of information, my image and voice ultimately may appear on the Internet.
I release and hold harmless the library and its agents and employees from and against any
claims or liability arising from or related to the use, publications or distribution of the
photographs/video/sound recordings.
