Project title: Protocol for hypertensionmanagement in the community. Problem statement:
Hypertension is a chronic medical condition afflicting the population. Many people admitted
in hospitalwith hypertension are unaware of the warning signs and symptoms, risk factors,
and precautionary measures related with hypertension. This lack ofknowledge can lead to
poor management outcomes which is hypertension crisis. Therefore, patients with
hypertension need to be educated beforedischarge to prevent hypertension crisis. Plan:
Develop a protocol for hypertension management in the hospital to educate hypertensive
patient andfamily about hypertension crisis. Using oral presentation associated PowerPoint,
television presentation and pamphlet about hypertension management.Also teach
strategies to reduce other health risk factors associate with hypertension. During the
hospitalization or out patient, involve healthcareproviders such as doctors, nurses to
demonstrate the benefit of being compliant with hypertension control. Develop a
community based wellness programwhere patient will receive more teaching on ways to
improve dietary habits, wellness, and medication compliant to help them maintain healthier
life stylechoices. Goal: Develop a protocol for patients and family members of patients
diagnosed with hypertension to help monitoring blood pressure. Create asimple brochure
information packet for patients and family members of patients at discharge. Promote a
healthy lifestyle by educate hypertensivepatients that taking an active role in their health
status will help them prevent hypertension crisis which is can be a life threatening
situations.
