- Stress the importance of having the Incident Reports in within 24 hours of the incident.
- Start by typing in the youth’s name into the left hand text box. The name of the child will come appear in the search results. Click on the child’s name.
- The Child’s record will appear. To write an incident report, click on “Group Notes” located in the lower left menu of the screen.

- You begin my choosing the participants of the incident. You may choose more than one youth. Signatures are important and cannot be left off. A consultant must always be added as a signature. Often it is wise to add a therapist, caseworker, supervisor etc.
- The Date of Incident, day of week and time of Incident will be automatically put in. You will need to change this for the correct time of the incident. REMIND THE TRAINEE THAT IT IS IMPERATIVE THAT THE INCIDENT REPORT BE COMPLETED WITHIN 24 HOURS OF THE INCIDENT.

- This is where you will fill out a general description of the incident.
- Remember that an incident is to be written behaviorally specific and should not include judgment, slang or derogatory terms.
