1. Case Narrative
On November 4, 2010 Qantas Airlines was operating Flight 32 from Singapore to Sydney, Australia (Administration). Qantas
Flight 32 was an Airbus A380 aircraft equipped with four Rolls Royce Trent 900 high bypass ratio turbofan engines
(Administration). Five flight crew members, 24 cabin crew members and 440 passengers were onboard Qantas Flight 32
(Administration). Four minutes after takeoff, as the aircraft was reaching an altitude of 7000 feet, there were two loud "bangs."
(Administration). The loud bangs were the result of an uncontained engine rotor failure of the number 2 engine, which is on the
left–hand side, inboard of the aircraft (Administration). The debris from the engine rotor failure impacted the aircraft causing
structural and systems damage (Administration). The debris mainly consisted of sections of the intermediate pressure or IP turbine
disk inside the engine. There were four large sections of the turbine disk that were projected. Two of the sections of the turbine
disk projected through the left wing leading edge and front spar, penetrating the left fuel tank, causing fuel to leak during flight.
One of the sections of the turbine disk went through the belly fairing, and the last turbine disk was found on the ground. The debris
also affected several systems because it severed various electrical wiring and hydraulic lines.
Right after the engine rotor failure, the flight crew of Qantas Flight 32 received a turbine overheat warning on the electronic
centralized aircraft monitor, or ECAM, for the number 2 engine (Administration). The flight crew then received clearance from air
traffic control, or ATC, to enter a holding pattern (Administration). After going through the ECAM procedures and aircraft
controllability checks, the flight crew was able to successfully land at Changi Airport (Administration). Following the landing and
coming to a complete stop, the flight crew delayed on issuing an emergency evacuation because engine number one could not be

The loud bangs were the result of an uncontained engine rotor failure of the number 2 engine, which is on the left–hand side, inboard of the aircraft (Administration). The debris from the engine rotor failure impacted the aircraft causing structural and systems damage (Administration). The debris mainly consisted of sections of the intermediate pressure or IP turbine disk inside the engine. There were four large sections of the turbine disk that were projected.
