Lurigio Loyola University Chicago, Illinois John Fallon Thresholds This paper presents a case study that illuminates the clinical and practical challenges that accompany the treatment of people with serious mental illness (SMI) and criminal involvement. We discuss the historical conditions that led to the influx of a large number of people with SMI into the criminal justice system. We discuss the case history of Richard P., which illustrates the use of Assertive Community Treatment (ACT) to care for criminally involved people with SMI. We focus on the ACT model that was employed by Thresholds to treat Richard P. It was known as the Thresholds Jail Program. We track his progress in the program and explicate the case management considerations that are most salient in treating offenders with SMI. Lurigio Loyola University Chicago, Illinois John Fallon Thresholds This paper presents a case study that illuminates the clinical and practical challenges that accompany the treatment of people with serious mental illness (SMI) and criminal involvement. We discuss the historical conditions that led to the influx of a large number of people with SMI into the criminal justice system. We discuss the case history of Richard P., which illustrates the use of Assertive Community Treatment (ACT) to care for criminally involved people with SMI. We focus on the ACT model that was employed by Thresholds to treat Richard P. It was known as the Thresholds Jail Program. We track his progress in the program and explicate the case management considerations that are most salient in treating offenders with SMI. Provision of mental health services is a necessary and complex part of any correctional operation.
Attention to planning and implementation of services to meet the mental health needs of population greatly contribute to a smooth running facility; inattention can lead to problems, negative publicity and litigation.
Correctional administrators must know the standards for care of mentally ill offenders.
Process for care of the mentally ill has changed significantly over the past fifty years. With the advent of psychopharmacology and focused therapies most people suffering from mental illness may be managed in outpatient settings.
Introduction
The management of most mentally ill patients as outpatients has resulted in the deinstitutionalization of people needing care.
However, the lack of community resources and existing support systems has led to the inadequate treatment of some mentally ill persons and has resulted in their placement in the criminal justice system.
Current data reflect more mentally ill persons in jails and prisons than in community mental institutions.
About half of the inmate population has been diagnosed with a mental health problem with approximately 16% diagnosed with serious mental illness.
The Diagnostic and Statistical Manual of Mental Disorders
Most commonly used classification system of mental illness and defects.
A mental disorder is a: “clinically significant behavioral or psychological syndrome or pattern that occurs in an individual and that is associated with present distress or disability.”
Major mental disorders include diagnosis of: schizophrenia, major depression, or bipolar disorder.
Many inmates demonstrate personality dysfunction and meet criteria for Antisocial Personality Disorder and other personality disorders, which remain difficult to treat.
Guidelines and Standards
The responsibility for provision of care to those who are denied the ability to choose their own care because of confinement has been defined in the Courts and standards of several organizations published to review.
The current standard of care requires basic and clinically relevant care. Problems arise when care falls below accepted standards and may result in deliberate indifference.
Deliberate indifference can be evidenced by: lack of access, failure to follow through with care, insufficient provision of staff resources, and poor outcomes due to negligent care.

Several organizations have established minimum guidelines for the treatment of mentally ill in correctional environments:

This is a workshop about communication and collaboration. We will experience how we can analyze the reasons for resistance to change (exercise 1) and practice how to improve our conversation style and be more in control and effective in the way we communicate (exercise 2).
This session will use Dave Gray’s Empathy Mapping, Argyris’ Ladder of Inference and The Four Rs from Agile Conversations (Squirrel and Fredrick).
Abstract:
Let’s talk about powerful conversations! We all know how to lead a constructive conversation, right? Then why is it so difficult to have those conversations with people at work, especially those in powerful positions that show resistance to change?
Learning to control and direct conversations takes understanding and practice.
We can combine our innate empathy with our analytical skills to gain a deeper understanding of complex situations at work. Join this session to learn how to prepare for difficult conversations and how to improve our agile conversations in order to be more influential without power. We will use Dave Gray’s Empathy Mapping, Argyris’ Ladder of Inference and The Four Rs from Agile Conversations (Squirrel and Fredrick).
In the session you will experience how preparing and reflecting on your conversation can help you be more influential at work. You will learn how to communicate more effectively with the people needed to achieve positive change. You will leave with a self-revised version of a difficult conversation and a practical model to use when you get back to work.

- 2. FICM Forensic Intensive Case Management Greg McCutcheon, MA Executive Director
- 3. Forensic Intensive Case Management (FICM) The CMU in collaboration with Dauphin County MH/MR and the Dauphin County Jail Diversion Program provide targeted case management services to non- violent offenders linked with the criminal justice system that are in need of mental health services.
- 4. The Case for FICM •Research indicates that individuals with severe mental illness are overrepresented in jails and prisons in the U.S. •The prevalence of severe mental disorders in correctional facilities range between 6 and 9%, and are significantly higher than the rate of 2.8% in the general population. •Individuals who exhibit symptoms of a mental illness at the time of an encounter with the criminal justice system are more likely to be arrested. •Persons with serious mental illness are jailed more often than hospitalized.
- 5. The Case for FICM •People with serious mental illness spend longer in jail for similar convictions than those without serious mental illness. •Many offenses that lead to incarceration for individuals with SMI are minor. •Individuals with serious mental illness who experience things not directly related to their illness (i.e. homelessness) are more likely to return to jail. •Case management can lead to a reduction in arrests and incarceration.
- 6. Individuals with serious mental illness are more likely to become involved with the legal system, and once they do, are more likely to be incarcerated and spend more time detained than individuals who do not suffer from mental illness.
- 7. •Diversion = avoiding or radically reducing jail time by using community based treatment and an alternative •Focuses on Post-Booking but Pre-Sentencing intercepts or diversion models for non-violent offenders Individuals suffering from mental illness have been diverted from the criminal justice system, have had their charges reduced, or served a decreased sentence due to the coordination of services between the mental health and criminal justice systems. To date, more than 10,000 jail days have been saved due to these efforts. The Jail Diversion Program

What is FICM? •Forensic Intensive Case Management is a specialized unit within the mental health department of the CMU. •Designed to work with individuals who are diagnosed with a mental illness and are also involved with the criminal justice system. •Delivered by Intensive Case Managers who are specially trained and possess an expertise in working with the criminal justice system. •Multi-faceted, and provide support to the individuals being served through innovative strategies generally not associated with their traditional ICM counterparts.
- 9. •Forensic Intensive Case Managers are trained and have specialized expertise in working with the criminal justice system. •FICM caseloads are comprised of individuals enrolled in the MH Jail Diversion Program and in the future will include other identified individuals linked with the criminal justice system. •The Forensic Intensive Case Managers carry a smaller caseload than their traditional ICM counterparts. How is Forensic Intensive Case Management (FICM) different from ICM?
- 10. 1. Engaging the individual in mental health treatment 2. Coordinating and communicating services between the mental health and criminal justice system 3. Identifying community resources that eradicate the marginal situations generally associated with the forensic population 4. Identifying and rebuilding relationships with family and friends who serve as support for the individual 5. - 11. Forensic Intensive Case Management services allow individuals involved with mental health and criminal justice the opportunity to participate in their treatment planning while assisting them in confronting and overcoming the barriers and obstacles that confront them.
