Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

Spice & JWH-018 – Worte der Warnung
Nachdem nun kürzlich in einer ganzen Reihe von Produkten der Marke „Spice“ JWH-018 von der
Frankfurter Firma „THC Pharm“ als der eigentlich aktive Inhaltsstoff identifiziert wurde, beginnt nun das
Rätselraten um diese Substanz. Gesundheitliche Risiken, vor allem über längere Zeiträume hinweg,
scheinen nicht absehbar. Die Kiffer-Community reagiert zu großen Teilen arglos und unbekümmert. Da
Spice ja eine ausgesprochen angenehme Wirkung entfaltet, werden Bedenken hinsichtlich des
Gefährdungspotentials von JWH-018 in den Hintergrund gedrängt. Doch leider gibt es in der Tat genug
Anlass zur Sorge. Wer nicht gedankenlos alles raucht was high macht, möge sich bitte die folgenden Zeilen
durchlesen und selbst entscheiden.
Was bereits jetzt feststehen dürfte: JWH-018 und verwandte Derivate sind mitnichten harmlos!
JWH-018 und die AAI-Familie
Um sich ein ungefähres Bild zu verschaffen, womit man es hier eigentlich zu tun hat, möchte ich in aller
Kürze einen Überblick liefern, um was für Substanzen es sich bei JWH-018 und eng verwandten
Verbindungen handelt.
Seit der Entdeckung des aktiven Prinzips in Hanf (Cannabis sativa) im Jahre 1964 durch Gaoni und
Mechoulam1 wurde eine Lawine an neuen Entdeckungen losgetreten. Der Wirkstoff war das allseits
bekannte ∆9-Tetrahydrocannabinol (im weiteren Text einfach THC).

Ein weiterer Meilenstein in der Cannabinoid-Forschung war die Entdeckung von spezifischen Rezeptoren
für THC.2,3 Momentan gelten 2 Subtypen dieser Rezeptoren als nachgewiesen, kurz als CB1 und CB2
bezeichnet. Vor allem CB1 ist für die psychoaktive Wirkunge von THC und anderen Cannabinoiden
verantwortlich. Es soll aber nicht unerwähnt bleiben, dass Hinweise auf mindestens einen weiteren, bisher
nicht identifizierten Subtyp existieren.
Abgerundet wurde das Bild, als dann auch körpereigene Stoffe identifiziert werden konnten, die an diesen
Rezeptoren wirkten.4 Der bekannteste Vertreter ist hierbei das Anandamid.
Wie man bereits an den Strukturen von THC und Anandamid erkennt, haben Substanzen, die an den
gleichen Rezeptoren angreifen und dieselbe Wirkung verursachen, nicht zwangsläufig einen ähnlichen
Aufbau. Dies gilt auch für die Familie der Alkylaminoindole (AAI). Ausgehend vom bekannten und
etablierten Schmerzmittel Indometacin entwickelte die Firma Winthrop Sterling (heute Teil von Sanofi) in
den 1980er Jahren die Substanz Pravadolin.5 Diese zeigte im Tierversuch überraschenderweise eine starke
2. schmerzlindernde Wirkung, und zwar auch noch bei einer Dosis, bei der die Forscher eigentlich keine
Wirkung mehr erwartet hätten. Angestrebt wurde eine Hemmung der Cyclooxygenase, ein Enzym,
welches an der Bereitstellung von Botenstoffen beteiligt ist, welche Schmerzen verursachen. Am selben
Enzym greift übrigens auch Aspirin an. Nach einiger Untersuchung wurde klar, dass es sich bei Pravadolin
um einen Agonisten an den Cannabinoid-Rezeptoren handelte. Damit war die Substanzklasse der
Alkylaminoindole entdeckt. Die strukturelle Ähnlichkeit zu dem, von der Arbeitsgruppe um John W.
Huffmann entdeckten, JWH-018 fällt auch dem Laien sofort ins Auge.

Die für die Wirkung an CB1 wichtigen Strukturmerkmale, das sogenannte „Pharmakophor“, ist relativ
simpel: Ein Indol-Ringsystem (blau), an welches ein aromatisches Ringsystem in Position 3 geknüpft
wurde (rot; wichtig hierbei die Verknüpfung über eine C=O-Gruppe) sowie ein Alkylrest am
Stickstoffatom des Indols (grün).
Wie man an Pravadolin sieht, gibt es einigen Spielraum für Veränderungen, ohne jedoch die Wirkung auf
CB1 und CB2 zu verlieren. Dies ist auch der Grund dafür, dass verschiedene Forschungsgruppen aus
Industrie und Universitäten mehrere hundert Variationen der AAIs entdeckt haben (und es werden nach wie
vor weitere Abwandlungen veröffentlicht). Ein gefundenes Fressen für den quasi-legalen Drogenmarkt!
Ähnliche Entwicklungen konnten bereits bei Amphetaminen und Piperazinen beobachtet werden, besser
bekannt unter der Trivialbezeichnung „Designerdrogen“. Auch die AAI-Familie ist streng genommen
nichts anderes als eine Gruppe von Designerdrogen. Und auch die AAIs werden das Schicksal der
Prohibition teilen, nun, da sie in Spice entdeckt wurden.
Was zeichnet JWH-018 aus?
Was aber zeichnet JWH-018 nun dermaßen aus, dass es in den letzten Monaten so eine schlagartige
Popularität erlangt hat?6 Es ist der simple Aufbau, der JWH-018 so populär bei Untergrundchemikern
macht. Bereits vor dem Hype um die AAI-Familie waren zahlreiche andere synthetische Cannabinoide
bekannt; einige von THC abgeleitet, andere mit völlig verschiedenen Strukturen. Standards in der
Cannabinoid-Forschung sind zum Beispiel WIN 55,212-2 (ebenfalls ein AAI) oder das strukturell nicht
verwandte CP 55490 (wer genau hinsieht, erkennt hier leicht, dass es sich um eine stark reduzierte Version
von THC handelt).

Selbst klassisches THC ist synthetisch sehr wohl zugänglich und trägt den Namen Dronabinol. Dies ist
übrigens das Spezialgebiet von THC Pharm. Aber allen gemeinsam ist, dass die Synthesen in der Regel zu
aufwendig, zu teuer oder mit einer niedrigen Ausbeute versehen sind. So kosten z.B. 50 mg reines
WIN 55,212-2 bei „Tocris Bioscience“ (einem seriösen Anbieter!) fast € 300 und das kann man noch als
billig betrachten. Unüblichere Cannabinoide sprengen schnell die 1000-€-Grenze für wenige Milligramm.
Auch sind nicht alle synthetischen Cannabinoide bei 25 °C und Luftkontakt stabil (z.B. muss CP 55940 bei
-20 °C gelagert werden), was einer einfachen Lagerung und damit einer Kommerzialisierung bisher im
Wege stand. Dies alles machte die sonst sehr attraktiv erscheinenden synthetischen Verbindungen nicht
marktfähig, im Sinne der Headshop-Besitzer.
JWH-018 scheint DIE Lösung zu sein. Die Synthese geschieht in der Tat aus den 3 Teilen des
Pharmakophors (siehe oben), welche allesamt billig im Kilogramm-Maßstab zu kaufen sind und auch nicht
überwacht werden.7 Die Synthese-Ausbeuten sind hoch genug, der apparative Aufwand vergleichsweise
einfach und die Aufreinigung vermeintlich simpel. Aus diesem Grund hatten sich bereits mehrere Labore
an der Analyse der Spice-Produkte versucht, scheiterten aber bisher. Auch THC Pharm konnte nicht in
allen Proben JWH-018 nachweisen. Es bleibt also die berechtigte Vermutung, dass einige der Spice-
Produkte, aber auch die der Nachahmer, noch andere synthetische Cannabinoide enthalten.
Das Gesundheitsrisiko
Zunächst die guten Nachrichten: Cannabinoide, sowohl die natürlichen als auch die synthetischen, greifen
nicht akut in lebenserhaltende Prozesse unseres Körpers ein. Dies ist der Grund, weshalb bis heute kein
Cannabis-Toter berichtet wurde, zumindest nicht bezogen auf den unmittelbaren Konsum. Dies steht im
krassen Gegensatz zu anderen Substanzen, wie z.B. Heroin, Kokain und Amphetamin (um nur mal 3 der
bekanntesten Drogen zu nennen), an denen man sich allesamt tödlich vergiften kann. Doch sollte dies
nicht als Signal zur Entwarnung verstanden werden.
In seiner Email an Pierre Markuse hat John W. Huffmann völlig korrekt gesagt, dass zum momentanen
Zeitpunkt keinerlei toxikologischen Daten zu JWH-018 vorliegen. Generell gibt es sehr wenig belegte
Information in dieser Hinsicht über die AAI-Familie. Trotzdem ist es möglich einige Aussagen zu treffen.
Die leider bisher einzige Veröffentlichung stammt von einer Arbeitsgruppe der Universität New Orleans
aus dem Jahre 2006.8 Diese konnte für das eng verwandte Derivat JWH-015 zeigen, dass der natürliche
Metabolismus mit großer Wahrscheinlichkeit reaktive Zwischenstufen beinhaltet, die bekannt für ihr
krebserregendes Potential sind.

JWH-015
Die Ähnlichkeit zu JWH-018 ist augenscheinlich: Lediglich der Alkylrest am Stickstoffatom des Indols ist
um 2 Kohlenstoffatome kürzer; außerdem findet sich hier eine Methylgruppe in Position 2 am Indol-
System. Die krebserregende Wirkung geht aber von der Naphthoyl-Untereinheit aus (im Pharmakophor
zuvor in rot dargestellt). Daher ist ein Vergleich mit JWH-015 sehr wohl zulässig!
Ist JWH-018 also krebserzeugend? Die ehrliche Antwort muss lauten: Man weiß es nicht.
Aber dies kann man über unzählige andere Stoffe auch sagen, für die es keine ausreichenden
toxikologischen Daten gibt. Da aber bei einem sehr eng verwandten Stoff die Frage mit „Ja“ beantwortet
wurde, ist größte Vorsicht beim Konsum von JWH-018 angebracht.
Ein Kommentar zu den Tierversuchen: Die Experimente wurden an Fraktionen aus Rattenlebern
durchgeführt. Es handelte sich also nicht einmal um „ganze“, lebende Tiere, sondern nur einen kleinen
Teil davon. Auch ist für Nagetiere bekannt, dass diese einen wesentlich aktiveren Stoffwechsel haben als
Menschen. Man spricht z.B. bei Mäusen daher auch manchmal vom „Leben in Zeitraffer“. Außerdem
muss natürlich gesagt werden, dass Menschen keine Mäuse sind, und allein dadurch der Vergleich bereits
etwas hinkt. Dies alles klingt beruhigend, ist es aber aus fachlicher Sicht nicht. Es handelt sich hier um
eine sehr wohl übliche Vorgehensweise zur Bestimmung des krebserregenden Potentials einer Substanz.
Eine realistische Einschätzung wäre es, wenn man sagen würde, dass ein Abbau von JWH-018, so
wie in Rattenleber nachgewiesen, in Menschen zumindest wahrscheinlich ist, aber auf keinen
Fall ausgeschlossen!!!
Leider haben Falschinformationen und gezielt manipulierte Versuchsergebnisse das Vertrauen in derartige
Aussagen beschädigt. So wurde z.B. auch von THC einst behauptet, dass es die Chromosomen in
menschlichen Zellen beschädige, also unser Erbgut manipuliere. Dies gilt heutzutage als widerlegt. Daher
bin ich vorsichtig und sage bewusst: Man weiß es nicht. Doch die Zeichen stehen eher zu Ungunsten der
Konsumenten. Dies zu verneinen, ist meiner Meinung nach grob fahrlässig.
Zu dem erörterten Problem gesellen sich noch ein paar weitere, was das Bild verkompliziert. Diese sollen
kurz zusammengefasst werden:
1. JWH-018 ist simpel im Aufbau und daher billig herzustellen. Dies ist aber auch einer der größten
Schwachpunkte dieser Verbindung, wenn es darum geht, sie im Körper abzubauen. An JWH-018
sind einfach zu wenige Ansatzpunkte für unsere Leberenzyme, um es in eine leicht ausscheidbare
Form zu überführen. Das mit Abstand wichtigste Organ zur Ausscheidung körperfremder
Substanzen ist die Niere, welche als eine Art Filter fungiert. Damit Stoffe über die Niere
ausgeschieden werden können, müssen sie zuvor aber in eine wasserlösliche Form überführt
werden. Leider ist JWH-018 praktisch unlöslich in Wasser und die Abwesenheit von weiteren
funktionellen Gruppen, wie z.B. Amine, Alkohole, Ester, Amide usw. , macht es sehr schwierig
JWH-018 wasserlöslich zu gestalten. Daher ist die Verweildauer im Körper, auch wenn diese
bisher nicht bei Menschen untersucht wurde, mit guter Wahrscheinlichkeit sehr hoch.
Die Pharmakologie praktisch aller Cannabinoide bringt es mit sich, dass diese nunmal generell
fettlöslich sind. Mit anderen Worten: Die CB-Rezeptoren sind so aufgebaut, dass einfach nur
fettlösliche Substanzen gut passen. Es ist allgemein bekannt, dass auch THC sehr lange im
5. menschlichen Körper verweilt, da es sich ins Fettgewebe einlagert. Bei chronischen Konsumenten
kann man noch wochenlang THC-Abbauprodukte im Urin nachweisen. Mit JWH-018/015 ist es
praktisch gleich, wenn nicht sogar noch drastischer, da THC noch vergleichsweise effektiv
abgebaut werden kann. Selbst wenn also der oben beschriebene Metabolismus von JWH-018,
welcher zu potentiell krebserregenden Substanzen führt, sehr unwahrscheinlich ist, so muss man
trotzdem konstatieren: Die Verbindung hat am Ende jede Menge Zeit um selbst die
unwahrscheinlichsten Metabolismus-Wege zu beschreiten!
2. Abgesehen von der eigentlichen Toxizität von JWH-018 muss man unbedingt auch die der
Verbrennungsprodukte berücksichtigen. JWH-018 lässt sich, wenn man den zahlreichen
Erfahrungsberichten aus diversen Foren folgt, effektiv verdampfen. Dazu wird der Stoff in ein
Gefäß gebracht, welches i.d.R aus Glas ist, dieses dann von außen (!) erhitzt und die entstehenden
Dämpfe eingeatmet. Die Prozedur ist vom Crack-„Rauchen“ bekannt. Entsprechende Glaspfeifen
sind in Headshops nicht unüblich.
Spice hingegen wird geraucht. Der Unterschied ist, dass bei dieser Prozedur die Substanz in direkten
Kontakt mit der Flamme bzw. Glut kommt. Dies bringt deutlich höhere Temperaturen mit sich
und dadurch eben auch deutlich mehr temperaturbedingte Zersetzungsprodukte. Man darf getrost
davon ausgehen, dass diese nicht unbedingt gesünder als die Stammverbindung sind. Wenn schon
JWH-018 konsumiert wird, dann plädiere ich unbedingt für ein Verdampfen und gegen das
Rauchen. Ganz im Sinne einer Schadensminimierung. Da Spice ausschließlich geraucht werden kann
(die zahlreichen Kräuter verhindern ein effektives Verdampfen), muss ich meine Meinung hierzu
wohl nicht näher ausformulieren.
3. Ein Detail, welches nur selten Einzug in die einschlägigen Diskussionen findet, ist die Reinheit des
konsumierten Produktes. Es liegt in der Natur des Menschen, psychoaktive Substanzen aller Art
zu konsumieren. Dafür sind mehrere Jahrtausende Kulturgeschichte Beweis genug. Und es liegt
auf der Hand, dass die Menschen auch entgegen guter Argumente damit fortfahren werden. Daher
muss es unbedingt oberstes Anliegen sein, den Drogenkonsum, wenn er sich schon nicht
vermeiden lässt, doch wenigstens so sicher wie möglich zu gestalten. Die Produktreinheit steht
hierbei an oberster Stelle.9 Leider zeigt die Realität, dass es bei kommerziellem JWH-018 bei
Weitem nicht die Regel ist, sauberes Produkt zu erhalten. Ich beziehe mich hier explizit auf den
„freien“ Markt und nicht die seriösen Händler, welche ausschließlich Forschungseinrichtungen
beliefern (z.B. die zuvor erwähnte Firma Tocris).
Um es in aller Deutlichkeit klarzustellen: Reines JWH-018 ist weiß bzw. farblos! Fast alle
Chargen, inklusive der, die ich selbst in der Hand gehalten habe, sind hellgelb, da leicht verunreinigt.
Die per HPLC nachgewiesene Reinheit betrug in diesem Fall ca. 98,5 %. Dies wäre für ein offiziell
zugelassenes Arzneimittel übrigens deutlich zu stark verunreinigt; das Reinheitskriterium wären
hier 99.95 %! Außerdem müssen bei Arzneimitteln alle Verunreinigungen über 100 ppm deklariert
werden und deren toxikologische Unbedenklichkeit bei den relevanten Konzentrationen
bescheinigt werden. Wie zuvor erörtert wurde ist bereits die Toxizität von JWH-018 selbst sehr
bedenklich, die Art der Verunreinigungen hingegen ist praktisch nicht bekannt.
Wieso aber sollte denn kommerzielles JWH-018 überhaupt verunreinigt sein? Wurde nicht zuvor
gesagt, dass die Aufreinigung simpel ist? Nein! „Vermeintlich simpel“ war der absichtlich gewählte
Wortlaut. Im Blog von Steffen Geyer fiel der Name eines Onlinehändlers, der sich im Moment
eine goldene Nase am (noch legalen) Geschäft mit JWH-018 und verwandten Derivaten verdient.
Ich werde die Internetadresse nicht kundgeben, da ich diesen Handel absolut missbillige.
Allerdings fand ich Verweise auf besagten Händler in mittlerweile 3 Drogen-orientierten Foren, in
einem machte ein Vertreter des Händlers sogar ganz unverblümt Werbung für den neuen
Kassenschlager. Was dieser allerdings anbietet sieht so aus:

Man müsste farbenblind sein, um zu übersehen, dass diese Substanz dunkelbraun ist. Die Flocken im
Vordergrund weisen einen helleren Farbton auf, was aber einzig und allein daran liegt, dass sie von dem
massiven Stück abgekratzt wurden. Ich wiederhole: Ein heller Gelbton wäre im Höchstfall noch tolerierbar.
Die abgebildete Substanz soll angeblich „>98 % pure“ sein. Der Händler besitzt sogar die Dreistigkeit,
folgendes Statement abzugeben:
„JWH-018 is a brown, lumpy, slightly-sticky dry compound.”
Nun ist Spice kein reines JWH-018, sondern enthält nur einen sehr geringen Anteil der Verbindung. Was
soll es also Spice-Konsumenten kümmern? Die Firmen, welche Spice bzw. die Plagiate vertreiben,
beziehen ihr JWH-018 alle von derselben Quelle: Auftragslabore in China. Außerdem spricht die
Anwesenheit vergleichsweise großer Mengen (ungesunder) Lösungsmittel nicht gerade für eine akkurate
Verarbeitung der Spice-Produkte. Die Wahrscheinlichkeit, dass Spice mit derartig verunreinigtem
Ausgangsstoff wie auf dem Bild zu sehen ist hergestellt wurde, ist nicht nur groß, sondern meines
Erachtens nach quasi gesichert. Für den Händler zählt nur der Profit. Jeder Aufreinigungsschritt ist mit
zusätzlichen Kosten sowie Produktverlusten verbunden. Das ist nun mal die Natur der Dinge und kann
von jedem Chemiker bestätigt werden. Der Rest ist ein simples Zusammenzählen der Fakten…
Abschließend sei darauf verwiesen, dass, auch wenn Cannabinoide praktisch nicht akut tödlich sind, sie
dennoch ein hohes akutes Gefährdungspotential besitzen. Der Grund liegt auf der Hand: Eine reguläre Dosis
reines JWH-018 beträgt ca. 1-3 mg, sofern man keine ausgeprägte Toleranz besitzt. Kaum jemand hat eine
Waage mit der nötigen Präzision zu Hause bzw. kann per Augenmaß abschätzen, wie viel 3 mg sind.
Daher sind Überdosierungen vorprogrammiert. Reguläre Kiffer werden nun entgegenhalten, dass es für
die meisten Menschen ein Limit gibt, über das hinaus mein einfach nicht noch stärker „high“ werden
kann. Dies ist korrekt, sofern die Diskussion bei THC bleibt.
THC ist ein partieller Agonist an CB1, das heißt, es ruft nur einen Bruchteil der biochemischen „Antwort“
hervor, welche eigentlich theoretisch möglich wäre. Sind alle Rezeptoren besetzt und dadurch
(teil)aktiviert, weil man extrem große Mengen Marihuana geraucht hat, dann geht es einfach nicht weiter.10
JWH-018 hingegen ist ein voller Agonist an CB1, es ist also problemlos möglich eine stärkere psychoaktive
Wirkung hervorzurufen, als es mit THC je möglich ist. Der passionierte Kiffer wird sich darüber eventuell
freuen, aber eine Überdosierung mit derart hochpotenten Cannabinoiden ist extrem unangenehm. Gut,
man stirbt nicht. Aber es gibt genug Grade davor, die einem Angst machen sollten. In diversen Foren
(siehe unter anderem Quelle 6) wurde darüber im Detail berichtet. Konsumenten erzählten speziell von
Panikattacken, die jeglichen Witz vermissen lassen, wie er sonst gerne in Berichten von THC-Konsum
mitschwingt. Paranoide Zustände, wenn auch nur vorübergehend, Angst und Panik sind die häufigsten
Nebenwirkungen, wenn man sich selbst oder die Dosis überschätzt. Dies gilt selbstverständlich auch für
Spice. Durch Panik verursachte Selbstverletzungen liegen sehr wohl im Bereich dessen, was möglich und
wahrscheinlich ist.
Fazit
Ich möchte mit diesem Artikel niemanden belehren. Zuviel wurde in der Vergangenheit in dieser Hinsicht
falsch angegangen und das Vertrauen in Drogenaufklärung ist bei vielen Endverbrauchern dahin.
Hingegen ist es mein ehrliches Anliegen, über die Gefahren aufzuklären. Bitte liebe Spice-Konsumenten,
7. nehmt die Warnung ernst! Ich gehe nicht davon aus, dass die meisten ihren Konsum einstellen werden,
aber Aufklärung tut auf jeden Fall Not. Es darf am Ende niemand sagen, er hätte von nichts gewusst!
