El riesgo cardiovascular se origina a partir del aumento de la masa corporal, lo cual condiciona mayor resistencia a la insulina e hiperinsulinemia, que desemboca en cualquiera de las enfermedades que constituyen el síndrome metabólico, teniendo como consecuencia el aumento de riesgo cardiovascular. Los datos epidemiológicos sugieren que las enfermedades metabólicas en México, incluyendo a la HAS, debido a la alta prevalencia de sobrepeso y obesidad que actualmente existe y que va en aumento, aunado a la transición demográfica con la cual incrementará la población en mayor riesgo, acelerarán su crecimiento en los próximos años, incrementando considerablemente la incidencia de eventos cardiovasculares en México.

There is a close association between diabetes mellitus and arterial hypertension, and hyperglycemia is thought to be a contributory cause. In vitro studies suggest that high glucose concentrations reduce acetylcholine-induced vasodilation, thus promoting hypertension. Acute hyperglycemia causes alterations in the coagulation system that increase the risk of thrombosis. The production of intercellular adhesion molecule 1 (ICAM-1), a pro-adhesion protein and marker of activation of atherogenesis, is stimulated by acute hyperglycemia in diabetic patients.
La dieta DASH puede recomendarse sin la necesidad obligada de acudir a un nutriólogo. El número de raciones de los diferentes grupos de alimentos y el tamaño de la ración, expresada en términos caseros, ayuda a que el médico le explique con claridad al paciente que puede y que no puede comer.
Todos los humanos deberíamos hacer mpas ejercicio físico en nuestra vida diaria, prefiriendo caminar a la locomoción en vehículos de motor, cuando esto es posible. Pero aparte de ese ejercicio cotidiano, se debe programar una sesión diaria (o por lo menos varias veces a la semana) de actividades dinamicas, de 30 minutos, precedidas de una etapa de calentamiento y otra de enfriamento. Esta actividad preventiva y recreativa puede complementarse con algunas sesiones de actividades puramente recreativas, pero que demandan gasto enrgético: como el golf, la jardinerpia, las labores de reparación doméstica, los pasatiempos de mecánica o carpintería. El menos tiemnpo posible se deberá gastar en actividades meramente sedentarias, com ver TV, o juegos de salón.
La diabetes del Nuevo-ataque confiere un riesgo similar de eventos de CV a la diabetes establecida en pacientes del hypertensive tratados
Simvastatin 40 mg per day significantly decreased the risk of first major vascular events by 24% in all patients (including patients with CHD, CVD, peripheral vascular disease, and/or diabetes) and by 22% in patients with diabetes (both p<0.0001). Among the 10,269 patients treated with simvastatin, 2033 (19.8%) experienced at least one major vascular event (nonfatal MI, coronary death, revascularization, or stroke). Of the 10,267 patients in the placebo group, 2585 (25.2%) had a major vascular event. Of 2978 patients with diabetes who received simvastatin, 20.2% experienced major vascular events over five years of follow-up. Major vascular events occurred in 25.1% of 2985 patients with diabetes who received placebo. 24,26
The above slide shows that CV risk remains even after antihypertensive therapy. The bar graph depicts the results of a number of studies evaluating the efficacy of varying antihypertensive agents and their effects on CV risk. Like the lipid-lowering studies, solely treating a patient’s hypertension does not sufficiently protect patients from the risk of CVD. Even in active controlled trials, risk for CV events remains.
The previous slide demonstrated CV risk remains despite antihypertensive therapy in patients at risk for CVD. The data presented in this graph are the results of a series of studies that evaluated the efficacy of statins on lipid profiles and clinical end points, as well as their potential limitations in reducing the risk of coronary disease. More specifically, this slide shows the effectiveness of statins on reducing the risk of the combined primary end point of CHD death, nonfatal MI, or any cerebrovascular event. This slide is of particular relevance to our discussion because it demonstrates that it is insufficient to simply treat an individual risk factor for CVD, because although this may diminish patients’ risk somewhat, patient risk for further events remains very high.
Emberson J et al, used estimates of the relative risk reductions from meta-analyses of randomised trials in combination with data from a prospective observational study of CVD (the British Regional Heart Study) to analyse the impact of different risk reduction strategies in primary prevention. The study examined the effects of prevention strategies based on single risk factor assessment or total risk assessment. They concluded that assessment of overall risk leads to more effective intervention than assessment based on single risk factors. Furthermore, multiple interventions have considerably greater benefits than interventions based on targeting single risk factors. Ten percent reductions in long-term mean blood cholesterol and BP could have reduced major CVD by 45%. Jackson R et al, conducted a review of the randomised trials of BP or blood cholesterol lowering treatments and outlined the rationale for targeting BP and blood cholesterol lowering therapy to patients at high absolute CV risk. They concluded that separate management guidelines for raised BP and blood cholesterol need to be replaced by integrated CV risk management guidelines. They also posited that because CV risk factors interact with each other, moderate reductions in several risk factors can be more effective than major reductions in one.
