Community Health Nursing – Everything – Week 2 Great work on module one, everyone!
This week, you will have a primary discussion question due on Thursday by midnight and it
will need to be at least 400 words and you are permitted to use “I” since the discussion is
asking for your input and opinions. You will have two responses to fellow students due by
midnight on Sunday with at least 200 words each. Remember that in all modules and all
discussions or written assignments, you may use “I” in in this course, due to they are your
opinion. You are to discuss life review and use at least one scholarly source other than the
textbook. Professional development assignment clarification- due by midnight on Monday.
You are to identify an older adult (OA) and follow the instructions in the assignment tab of
your classroom, don’t forget to submit the log hours into Project Concert too. Do not copy
and paste or rewrite the assignment questions in your paper. One paper is to be submitted
into the drop box and is to have all of the assignment parts- do not submit the assessments
separately and do not submit the assessment in your paper- summarize the outcomes in
paragraph format. Week 2 Life Review DiscussionYour paper is to have at least 1500 words,
not counting the references page. It is to have at least two scholarly references other than
the learning materials that were provided in the classroom. (No Wikipedia or Wiki sites
since they are not considered peer reviewed or scholarly). Don’t forget that all written
assignments are to have a title page, introductory paragraph, your assignment(s), a
concluding paragraph, and a references page. Do not copy and paste your assessment
information into the assignment- the assignment is to be in APA 7th edition formatted
paragraphs of at least three or more sentences, and APA headings. DO NOT submit the
assessment forms for SPICES or other assessments, just summarize the outcomes in your
paper. **Remember that you cannot cite authors with same last names without adding
either an initial, or if their initials are the same, add their full first names -the same goes for
references. Example: (Hall, Ryan, Hall, Richard, & Chapman, 2003). Go to Project Concert
link in module two assignment area.There are to be 2-4 hours denoted on your log under
geriatric assessment, clarifying briefly what you spent your time on, and do not forget to
submit information into Project Concert. Hours to count on your log for required modules
include your hours researching, writing, speaking to the community, collaborating with
medical professionals but NOT time that is spent towards the discussion assignment.
Project Concert is a system the School of Nursing has implemented on September 3rd. This
system will help with: • electronically track your clinical hours in your clinical courses
N492: Community Health Nursing – Everything – Week 2 • schedule and report on clinical
evaluations • create student portfolios • track documents and expiration dates What you
need to know: • Login Information. The website is:
https://secure.projectconcert.com/aspen/login.aspx. You cannot google this site, so please
be sure to save as a bookmarked page. • If you have trouble with the site and/or log in, be
sure to email projectconcerthelp@aspen.edu • Username. Your username is our Learner ID.
It will be an entirely numeral username. Call any Aspen adviser and they can help you find it
on the student portal. • Password. Your password is in all CAPS and will need to be reset
upon your first login. The password is: GO What will you be using immediately: • Entering
Your Clinical and Practicum in the Following Courses. If you in a clinical or practicum
course, you will now be entering time logs in Project Concert. •Click here to see the video:
Here is a great video that will show you the highlights. • Here are some quick instructions
on how to get started: • o Click on the Experience Tab. o It will have a “NEW ENTRY” which
you can see the course you are in. You will include the day you are completing hours, put
the start and end time (8:00am to 12:00pm) and select the faculty member who is teaching
your course. Be sure to include what module number the log is for. o Select your Preceptor
(instructor in this course) and clinical site if you are completing hours at a site. o Select your
Mentor (instructor name) if you enter a log on the time you are working on your Project. o
Select the Module you are working on o Select the type of activities you participated in that
day. You can select multiple activities if applicable, such as reading, researching,
interviewing people, windshield surveys, actual presentation times, etc. o Finally hit Submit
which will route your entry to your faculty member to approve. As of today, Students should
be: N492: Community Health Nursing – Everything – Week 2 • • • • • • • • • • 1. Entering
times no more than weekly-one log with the required hours, or more, on the week(s) that
log is due is preferable. Students should not be entering multiple logs in a day or week. Be
sure to designate the correct module that you are adding log hours for. 2. Times should be
entered as a block of time. If you do not have the option to choose a block of time, then
choose start and stop times for the block of time. For example, choose start of 12pm and end
time of 3 pm, if it is 3 hours of activity (the exact times is irrelevant, it’s the total hours we
need). Again, it is best to submit one log for the entire week, with activities listed and at
minimum the required amount of hours. (Mod 2 in this course requires at least 2 hours,
mod 7 requires at least 10 hours and mod 8 requires at least 8 hours. You will ONLY submit
logs in mod 2, 7, and 8 for this course. 3. Select All That Apply for the activities for that block
of time for the day (again, no more than one entry per day, and preferably one log for the
entire week). If the activity isn’t listed, choose ‘other’ and use the textbox, but ALL activities
under that day and block of time should be in one entry. 4.Choose the Module # – required
5.Logs are REQUIRED for the class, no matter how th project is completed, and must be
submitted into project concert to pass the course. In this course a log is due: Mod 2, for at
least 2 hours, mod 7 for at least 10 hours, and mod 8 for at least 8 hours. **In order to see
previous logs, please click on EXPERIENCE, EXISTING, put in your start date of the class,
click on SEARCH. Then you will see your previous logs you have submitted. If they say
Pending, the instructor has not seen them yet. If they say DRAFT, the instructor cannot see
them and you have to revise and click SUBMIT (do not click SAVE). If they say Rejected,
view/review them to see what the error was, then fix it and click SUBMIT for the instructor

Review and Ego Integrity Goals After completing this module, you will be able to do the

Conduct geriatric life review assessment and create an individualized plan of care for this
older adult in the community Overview Healthy aging is aging actively. A person who
chooses to identify and participate in seeking the best possible adaptations and health
affirming activities will not be stymied by age as a destination. Theories give us context for
ideas not yet proven. Many theories evolve and are validated by research, while others are
disproved. Theories about aging are being challenged by the longevity and quality of life
that individuals over age 65 can look forward to in the present when they make healthy

Erikson. If you had to rate them on a scale of 1-10, with 1 representing a full state of despair
and 10 representing full ego integrity, what rating would you give your older adult?
Describe at least two nursing diagnoses for this client. Create a plan of care for the client to
include at least three nursing goals with two nursing interventions each. The assignment
should be written in an APA-formatted essay. The essay should be at least 1500 words in
length and include at least two scholarly sources other than provided materials. Follow APA
7th edition Student Sample formatting in the APA 7th edition manual. If you refer to your
assessment tools such as the SPICES assessment, it is to be cited in text and referenced in

This week, you will have a primary discussion question due on Thursday by midnight and it will need to be at least 400 words and you are permitted to use “I” since the discussion is asking for your input and opinions. You will have two responses to fellow students due by midnight on Sunday with at least 200 words each. Remember that in all modules and all discussions or written assignments, you may use “I” in in this course, due to they are your opinion. You are to discuss life review and use at least one scholarly source other than the textbook. Professional development assignment clarification- due by midnight on Monday. You are to identify an older adult (OA) and follow the instructions in the assignment tab of your classroom, don’t forget to submit the log hours into Project Concert too. Do not copy and paste or rewrite the assignment questions in your paper. One paper is to be submitted into the drop box and is to have all of the assignment parts- do not submit the assessments separately and do not submit the assessment in your paper- summarize the outcomes in paragraph format. Week 2 Life Review DiscussionYour paper is to have at least 1500 words, not counting the references page. It is to have at least two scholarly references other than the learning materials that were provided in the classroom. (No Wikipedia or Wiki sites since they are not considered peer reviewed or scholarly). Don’t forget that all written assignments are to have a title page, introductory paragraph, your assignment(s), a concluding paragraph, and a references page. Do not copy and paste your assessment information into the assignment- the assignment is to be in APA 7th edition formatted paragraphs of at least three or more sentences, and APA headings. DO NOT submit the assessment forms for SPICES or other assessments, just summarize the outcomes in your paper. **Remember that you cannot cite authors with same last names without adding either an initial, or if their initials are the same, add their full first names -the same goes for references. Example: (Hall, Ryan, Hall, Richard, & Chapman, 2003). Go to Project Concert link in module two assignment area.There are to be 2-4 hours denoted on your log under geriatric assessment, clarifying briefly what you spent your time on, and do not forget to submit information into Project Concert. You will include the day you are completing hours, put the start and end time (8:00am to 12:00pm) and select the faculty member who is teaching your course. Be sure to include what module number the log is for. o Select your Preceptor (instructor in this course) and clinical site if you are completing hours at a site. o Select your Mentor (instructor name) if you enter a log on the time you are working on your Project. o Select the Module you are working on o Select the type of activities you participated in that day. You can select multiple activities if applicable, such as reading, researching, interviewing people, windshield surveys, actual presentation times, etc. o Finally hit Submit which will route your entry to your faculty member to approve. As of today, Students should be: N492: Community Health Nursing – Everything – Week 2 • • • • • • • • • • 1. Entering times no more than weekly-one log with the required hours, or more, on the week(s) that log is due is preferable. Students should not be entering multiple logs in a day or week. Be sure to designate the correct module that you are adding log hours for. 2. Times should be entered as a block of time. If you do not have the option to choose a block of time, then choose start and stop times for the block of time. For example, choose start of 12pm and end time of 3 pm, if it is 3 hours of activity (the exact times is irrelevant, it’s the total hours we need). Again, it is best to submit one log for the entire week, with activities listed and at minimum the required amount of hours. (Mod 2 in this course requires at least 2 hours, mod 7 requires at least 10 hours and mod 8 requires at least 8 hours. You will ONLY submit logs in mod 2, 7, and 8 for this course. 3. Select All That Apply for the activities for that block of time for the day (again, no more than one entry per day, and preferably one log for the entire week). If the activity isn’t listed, choose ‘other’ and use the textbox, but ALL activities under that day and block of time should be in one entry. 4.Choose the Module # – required 5.Logs are REQUIRED for the class, no matter how th project is completed, and must be submitted into project concert to pass the course. In this course a log is due: Mod 2, for at least 2 hours, mod 7 for at least 10 hours, and mod 8 for at least 8 hours. **In order to see
