- 13. I’ve spent the majority of my professional career working with highly impacted individuals and their families.

- 18. My family is a member of a Patient -Centered Medical Home Model. “My daughter has two chronic medical conditions. Her doctors and every member of the team have given us the best medical care, dignity and respect possible.” *Daughter gave permission for disclosure.
