The "LINKS" project aimed to maximize relationships between general practitioner surgeries and community resources in local areas. The project found that approximately 20% of GP or nurse consultations required signposting patients to community resources, with 70-80% of those recommendations being accepted. All participating practices became more aware of support resources and interested in using them. However, sustaining the directories of resources requires ongoing time and effort. Strengthening these links in the future could involve providing education to practice staff or link workers across larger areas.
