DESIGN TO REDUCE
FEVER RATE
IN
A GIVEN COMMUNITY
ABSTRACT
Myself Dr. Gogate is working as M.O M.M.&H.S. Cl.A. Officer working since
20 th June 1986. During my tenure of service,I have been exposed to different
types of Environments. I have worked in Rural, Tribal, Semi Urban & Industrial
populations, so I have experience in knowing the problems & remedial measures.
Difficulties involved given me the necessary impetus to work more.
By 2nd jan 1998 I had been transferred to a remote rural P.H.C. I was
surprised to see plenty of mosquitoes that made to use mosquito repellent, mat,
agarabathies. Never ever I could sleep without mosquito repellent measures. A
thought came to my mind that If there are finite number of mosquito breeding
places then the problem can be remedied.
After series of observations. I could locate mosquito breeding places, so in turn
eliminated them. Then after about sixteen days mosquito menace was
controlled & I could sleep without mosquito repellant & windows wide open.
This led me to think, then comes the mathematical statement.
If there are "n" number of mosquito breeding places then there are "n" number
of ways (say actions )that we could control/eliminate mosquito breeding places.
RAGE OF TROPICS
India is a tropical country, because of
its high temp & humidity it
Provides immense opportunity for
mosquito generation.
OUR EXPERIENCE
As per our experience, as number
of mosquitoes increases, fever
rate also increases. I.e. to say
fever rate is proportional to
mosquito Population. This is
evident by increase of fever rate
in rainy season& Decrease in
fever rate in winter season.
FEVER
 Fever is a Symptom not disease.
Fever is a result of natural Immune
mechanism. Malaria is an important
disease,whichpresentsFever
Mosquito is an importantvector, which
transmits malaria.now This mosquito
is an important vector, which
transmitsmalaria,filaria,dengue,
japnese encephalitis.
 Now chichengunia added
WE KNOW
 mosquito breeds on Salient
&clean waters, either Artificial or
natural.
 Whenever insect predator
balance is perturbed there is an
exponential growth of
mosquitoes
*Insect predator balance
Under normal conditions Population of Predator and Pray SPIRAL
around a Stable Mean Value. However it is possible to purturb this
system by removal of one species. Suppose for example the Pray is
Insect and is removed by insecticide application then it is possible for
the population of the predator to collapse. Then after some time lag
when the insecticide activity is lost the mosquitoes shows exponential
growth. Predator having less productivity shows exponential decay.

With the Advent of Newer
Human Settlements, Utilities,
Water Storage, Waste Water
Mismanagement,
Constructions, There Are
Newer and Newer Mosquito
Breeding Places
MOSQUITOES
 There Are Three Major Types of
Mosquitoes 1) Anopheles2)
Culex 3) Edes Egiptii
 They All Need Salient Waters.
Anopheles Prefers Natural
Waters ,Culex Prefers Septic
Tank Systems and Edes Egiptii
Prefers Man Made Water
Sources.
NOW Therefore in a
given community
 If There are “n” Number of
Mosquito Breeding Places Then
There are “n” Number of Ways
(Say Actions) That We Could
Control /Eliminate Mosquito
Breeding Places.
Now this “n” is very
large
Therefore a New Communication
Design Is Created by Which We
Could Reach Every Household
and Study the Results
FOR THIS STUDY
We Need a Locality / Hamlet in Which We Will Train One
HEALTH GUARD Per Fifty Households.Train the Health
Guards About Techniques of Control / Elimination of
Mosquito Breeding Places .
These Health Guards Will Fill up Data Sheets, Arrange
Mass Media Meetings and Initiate Actions Therefore by Using
Defined Communication Design We Will Be Able to Control
Mosquito Population to Near Zero. This Communication
Design Is a New Tool in Public Health Science (Under
Development)
THIS COMMUNICATION
DESIGN CONSISTS OF
(I) Surveillance and Awareness generation phase
(II Mass media &Action phase
(III) Nuisance & Continuation phase
(IV) No mosquito phase and maintainance phase
[I] SURVEILLANCE AND AWARENESS
GENERATION PHASE
As the name suggests, this first phase included
#Surveillance Baseline studies
# Resource mobilization Resource person meeting
. # Preparation of publicity material
Poster Wallpapers Booklets Pamphlets
# Preparation of training material
Training of Health Guard Training of public
# Selection of health Guards
#Planning #Mapping #Planning for second phase
Scheduling of mass-media meeting
In the surveillance phase Data sheets are filled in by
health guards by Home to Home visits
[II] MASS MEDIA &ACTION PHASE
 This phase as the name suggests is the
process of conducting mass media meeting
& action is immediately followed.
 This phase is simply search &
control type. The people will be trained to
identify, locate, search& control /eliminate
mosquito-breeding places
 In this phase used oil musline
cloth & engineering methods & materials
also gappi fish are used
 As soon as people are aware of mosquito
breeding places, they will in turn control
/eliminate these places
 The mass- media meeting consist
how it is important to control mosquito-
The female mosquito enters after full blood meal (because it becomes heavy) enters
from outlet of the septic where she knows salient water is available that she could
safely lay her eggs without any natural enemies.
After about seven Days time the eggs hatches and transform in to larva’s
and then again in to adults
Again these adult mosquitoes are ready, they come out form vent/ cervices of the
septic tank and same cycle repeats by itself
[III] NUISANCE &
CONTINUATION OF CONTROL
PHASE
 When we will complete first two phases, that is to
say that, when people will take action and control
mosquito breeding places, then the people will
expect mosquito free nights, but on the contrary,
the people will complain of triggering of
mosquitoes.
 This happens partly due to Awareness
generation and also because when you take
measures to control mosquito-breeding places,
the mosquitoes becomes homeless & so become
more virulent
This phase lasts for 16 to 60 days depending on the longevity
of the mosquitoes.
In this phase people will be motivated to use anti mosquito
measures, training of which has already been taken in phase [II]
In this phase you will find four fold increase in the
consumption of anti mosquito coils
Triggering of mosquito menace is nothing to panic but it is
an indication that our anti mosquito mission is successful.
[IV] NO MOSQUITO PHASE
AND MAINTAINANCE PHASE
 If we are successful in stage (I)&(II), stage
(III)&(IV) are results or outcome.
If we are not successful in searching &
controlling mosquito breeding places ,
then we are not reaching stage(III)&(IV).
This stage requires continuation of
awareness generation, periodical checks,
insect surveys

THREATS
1 People living in less number of mosquitoes goes to high number of
mosquitoes area are more prone to have sleepless nights & high
risk of disease transmission
I have an innovative scheme of mosquito control thereby
reduce fever rate & improve HEALTH.
The scheme do not use insecticides as
programme intervention. it is an environment friendly
scheme. It is not health system dependent.
I am a medical officer working with Govt of
MaharashtraThe scheme has been approved by the
University as a subject topic for Ph.D.
As I am a medical officer, I am trying for resources
from our Director general. I’m not receiving matching
resources to enable me to do necessary Experiments so I am
searching for overseas support.
Whenever a concept is approved for as
Ph.D. thesis, it is bound to that resources should flow to the
idea to prove it or otherwise. As the scheme has potential to
save the world from chickengunia dengue malaria filaria like
vector bourn diseases you may be able to make the
authorities to respond THE SCHEME USES MATHEMATICAL
FINANCIAL APPLICATIONS
Adequate Mosquito Control is subtle for control of Vector borne
Diseases. The DISEASES, MALARIA, FILARIA, J.E DENGUE
CHICKENGUNIA are the examples that the Mosquito could
transmit. This list known to this Date & not exclusive. Therefore we
have made a generalization that Diseases that presents with Fever are
probably been transmitted by Mosquito, therefore Fever Rate is
proportional to mosquito population
ON AN AVERAGE
A family of five persons needs to spend a sum of Rs. 30to 50 /per
month for mosquito repellents mats / Agarbatthies etc.
If a person got fever the treatment cost ranges between Rs 100to Rs
500 The person could not attend his duties on an average of 2to 5 Days
therefore average loss of wage works out Rs500
Therefore if we consider on an average if we could save Rs
200/per person we could save Rs. 20 thousand crores for India.

.1 I have understood the problem. You have used nothing new but all that is covered in the
NVBDCP programme. What innovation is involved?
ans:We know mosquito breeds on salient waters. We know if we control mosquito-breeding
places we could be able to control. Mosquito population. In practice that is not
happening.
We are of the opinion that the problem is not remediable

over the last couple of hundred years have
been through Public Health not through
Medicine but Public Health is notoriously
unprofitable & people don’t make profit

These Health Guards Will Fill up Data Sheets, Arrange Mass Media Meetings and Initiate Actions Therefore by Using Defined Communication Design We Will Be Able to Control Mosquito Population to Near Zero. This Communication Design Is a New Tool in Public Health Science (Under Development)

  • 18. [II] MASS MEDIA &ACTION PHASE  This phase as the name suggests is the process of conducting mass media meeting & action is immediately followed.  This phase is simply search & control type. The people will be trained to identify, locate, search& control /eliminate mosquito-breeding places  In this phase used oil musline cloth & engineering methods & materials also gappi fish are used  As soon as people are aware of mosquito breeding places, they will in turn control /eliminate these places  The mass- media meeting consist how it is important to control mosquito-

The female mosquito enters after full blood meal (because it becomes heavy) enters from outlet of the septic where she knows salient water is available that she could safely lay her eggs without any natural enemies. After about seven Days time the eggs hatches and transform in to larva’s and then again in to adults Again these adult mosquitoes are ready, they come out form vent/ cervices of the septic tank and same cycle repeats by itself

[III] NUISANCE & CONTINUATION OF CONTROL PHASE  When we will complete first two phases, that is to say that, when people will take action and control mosquito breeding places, then the people will expect mosquito free nights, but on the contrary, the people will complain of triggering of mosquitoes.  This happens partly due to Awareness generation and also because when you take measures to control mosquito-breeding places, the mosquitoes becomes homeless & so become more virulent
  • 41. This phase lasts for 16 to 60 days depending on the longevity of the mosquitoes. In this phase people will be motivated to use anti mosquito measures, training of which has already been taken in phase [II] In this phase you will find four fold increase in the consumption of anti mosquito coils Triggering of mosquito menace is nothing to panic but it is an indication that our anti mosquito mission is successful.

If we are not successful in searching & controlling mosquito breeding places , then we are not reaching stage(III)&(IV). I am a medical officer working with Govt of MaharashtraThe scheme has been approved by the University as a subject topic for Ph.D. As I am a medical officer, I am trying for resources from our Director general. I’m not receiving matching resources to enable me to do necessary Experiments so I am searching for overseas support. Therefore we have made a generalization that Diseases that presents with Fever are probably been transmitted by Mosquito, therefore Fever Rate is proportional to mosquito population ON AN AVERAGE A family of five persons needs to spend a sum of Rs. 30to 50 /per month for mosquito repellents mats / Agarbatthies etc. If a person got fever the treatment cost ranges between Rs 100to Rs 500 The person could not attend his duties on an average of 2to 5 Days therefore average loss of wage works out Rs500 Therefore if we consider on an average if we could save Rs 200/per person we could save Rs. 20 thousand crores for India.
