The document summarizes the results of a hand hygiene project conducted by 8 hospitals participating in the Joint Commission Center for Transforming Healthcare. The project found that actual hand hygiene compliance rates were much lower than hospitals estimated, often less than 50%. Through analyzing data and improving processes using Lean Six Sigma methodology, hospitals identified 15 root causes of low compliance. Solutions targeted each root cause. As a result, by August 2010 hand hygiene compliance rates across the hospitals increased to an average of 82%. The Targeted Solutions Tool provides a framework and solutions for hospitals to address barriers to hand hygiene.
