(POA)
A Durable Power of Attorney is a legal
document that enables the you to
designate another person called the
agent, to act on behalf of you, even in
the event you become disabled or
incapacitated.

3. DECLARATION OF GUARDIAN OF THE ESTATE
A Declaration of Guardian is a legal document where
you tell the court who you want to serve as your
guardian if there is ever a guardianship proceeding
for you. There are two types of guardians: Guardian of
your estate, and guardian of your person.
Guardian of Your Estate: The Guardian of your
estate is entitled to possess and manage your
property including all of the assets in your estate.
They will also enforce any obligation in favor of you
and bring and defend lawsuits by or against you.

POWER OF
ATTORNEY
(MPOA)
A Medical Power of Attorney (MPOA) is a
legal document that allows an individual
to designate another person to make
medical decisions for him or her when he
or she cannot make decisions for himself
or herself.

DIRECTIVES
TO
PHYSICIANS
An advance directive is a document by
which a person makes provision for
health care decisions in the event that, in
the future, he/she becomes unable to
make those decisions.

6. HIPAA RELEASE
Your medical privacy is a right that’s protected
by law.
This is an incredibly useful law that protects you
from breaches of your privacy — but it can also
become a bit of a pain if you're very ill and a
loved one is trying to get information so they
can help you manage your care.
You can provide an opportunity for your loved
one by signing a HIPAA release form that
authorizes him or her to receive information
about your health.

7. DECLARATION OF GUARDIAN OF THE PERSON
Guardian of Your Person: The Guardian of your
person has the duty to provide care, supervision,
and protection for you. This includes providing
clothing, food, medical care (i.e., decisions
regarding operations, medications, and medical
procedures and treatments) and shelter.

AGENT TO
DISPOSE OF
REMAINS
This declares a person’s preference for
burial, or other options and allows the
writer to name the individual he or she
would like to be in charge of ceremonial
arrangements. The form also allows the
writer to leave special, written instructions
on their arrangements.
