We need to move Healthcare
from Information to Wisdom
"Where is the Life we have lost in living?
Where is the wisdom we have lost in knowledge?
Where is the knowledge we have lost in information?"
T.S. Elliott
Distributed economies, society or communities are today’s structure
to deliver productivity, sustainability and growth …
100 year old centralised structures are socio-economically and environmentally unsustainable
Distributed economies (DE) are “a fresh strategy to guide industrial development towards becoming
more sustainable. The concept calls for a transformation in the industrial system towards DE departing
from the socio-economically and environmentally unsustainable dynamics associated with large-scale,
centralised production units that are favoured by neoclassical economic drivers.”1
100 year old structures from the industrial Revolution
Hierarchies use point to point communication and put
2 information elements in the internet
Today’s structure
Wisdom Networks enable peer to peer
collaboration and put all 6 information
elements in the internet
1. Distributed economies – A new engine for innovation, Allan Johansson, The International Institute for Industrial
Environmental Economics, 2005 ( http://www.sciencedirect.com/science/article/pii/S0959652604002719 )
People are evolving toward digital "books" shared with everyone, everywhere, anytime
across all communities ... and crowd creates the next an era of distributed prosperity!
Facebook brought "books" to social in 10 years
Facebook reduced social interaction to books of “things” that
could shared with anyone, everywhere and restructured global
social time and attention and achieved transparency and
meritocracy in less than 10 years.
Wisdom Networks bring "books" to everything else in 2-5
years
Wisdom Networks will reduce organisations, industries and
society to books that can be shared with anyone, everywhere,
anytime and transform organisations, industry and the remainder
of society within 2-5 years.
Only 30 networks required to achieve distributed prosperity?
As few as 30 wisdom networks can transform society and achieve
distributed prosperity. Each can be launched within 90-180 days
and achieve critical mass within 2-5 years.

Shell News Lens Scenarios (is.gd/newlens)
Mountains (Shell scenario)
"The first scenario, labelled “mountains”, sees a strong role
for government and the introduction of firm and far-reaching
policy measures. "1
"This is the world with status quo power locked in and held
tightly by the currently influential. Stability is the highest
prize: those at the top align their interests to unlock
resources steadily and cautiously, not solely dictated by
immediate market forces. The resulting rigidity within the
system dampens economic dynamism and stifles social
mobility." 1
Oceans (Shell Scenario)
"The second scenario, which we call “oceans”, describes a more
prosperous and volatile world. Energy demand surges, due to
strong economic growth. Power is more widely distributed and
governments take longer to agree major decisions. "1
"Influence stretches far and wide in the world of Oceans. Power
is devolved, competing interests are accommodated and
compromise is king. Economic productivity surges on a huge
wave of reforms, yet social cohesion is sometimes eroded and
politics destabilised. This causes much secondary policy
development to stagnate, giving immediate market forces
greater prominence."1

Punctuated reform
(productivity, growth)
"When stresses rise, and a crisis emerges, some
actors exhibit relevant forms of resilience that
enable them to adapt and reform. "1
"Financial, social, political or technological capital

change/reform."1
Trapped transition
(drift, decline, collapse)
"Others, however, struggle until the crisis escalates
to a level that enforces either dramatic and painful

responses delay change, causing conditions to
worsen until ultimately a reset is forced or a collapse
occurs."1

Global Health Care faces enormous challenges
A Wisdom Network overcomes the underlying problem
with information structures and productivity
Problem
Global Health Care faces enormous challenges.
● Demographics – unprecedented population growth and ageing
● Information Age technologies that are isolated and opaque, require expensive integration
and embed the status quo
● Demands on global health care are likely to increase 5 fold within 30 years
● Diminishing returns and declining productivity in the status quo means continuing
permanent crisis
The consequences for patients, providers and the industry are dire:
● Delivery costs escalating much faster than CPI
● Health professionals which don't have the knowledge or wisdom
● Unsatisfactory patient care and outcomes.
● 1 in 6 patients are poisoned, maimed or killed by the Western health care system
The solution for these problems is at hand but requires a substantial cultural shift. The patient
needs to be the focal point of health care. Community wisdom networks need to replace
enterprise information technology.
Solution
● The most productive economic asset is human time and attention. Wisdom Networks
restructure human time and attention and recover time and attention lost with opaque
Information Age structures and provide a rapid jump in productivity and capability.
● Wisdom Networks are an Over the Top (OTT) strategy that integrates fragmented
information technologies and links to "Things" on the internet
● Wisdom Networks organise a 6 stage process to create wisdom
● Everyone in the health community can see every "thing", every "where" which delivers
transparency. Transparency and rapid sharing of wisdom Books (knowledge and
outcomes) means that a decision based upon meritocracy can be made.
Demands on global health care likely to
increase 5 fold within 30 years
● Population growing from 2 billion in 1960 to 9 billion in 2050.
● Average life expectancy in developed countries went from
approximately 50 years in 1900 to almost 90 years now.
● The world’s middle class was approximately 1 billion people
at the turn of this century, consuming 80% of all goods and
services. This middle class will grow to 3 billion people by
2040.
● Some societies are moving from problems of
malnourishment to problems of obesity inside one
generation.
Diminishing returns and declining productivity in
the status quo means continuing permanent
crisis
Maintaining the status quo likely to deliver crisis:
● Diminishing returns from investment in the status quo means
that productivity is declining or negative.
● Governments already under budget pressure and struggling
to deliver services today.
Insanity of investing in the status quo
Due to diminishing returns, incremental investment in information or
management technology will not increase productivity.
Incremental investment in the status quo suffers diminishing returns and will
not increase productivity, growth, patient outcomes or service future needs
"Insanity: doing the same thing over and over again
and expecting different results". - Albert Einstein
General purpose technology drives most economic growth. First
generation uses it within the status quo.
The second generation reshapes the status quo.
We can't afford to wait 30 years for the next generation
to use the internet to reshape health
Erik Brynjolfsson: The key to growth? Race with the
machines, TED, February 2013
"Let's start this story when 120 years ago American factories
started to electrify their operations igniting the second
industrial revolution. The amazing thing is that productivity
had not increased in those factories for 30 years. 30 years!
That's long enough for a generation of managers to retire."
"You see the first wave of managers simply replaced their
steam engines with electric motors, but they didn't redesign
the factories to take advantage of electricity's flexibility. It fell
to the next generation to invent new work processes and
then productivity soared often doubling or even tripling in
those factories. Electricity is an example of a general
purpose technology like the steam engine before it."
"General purpose technologies drive most economic growth
because they unleash cascades of complementary
innovations like light bulbs and factory redesign." - Erik
Brynjolfsson 1
The internet is a general purpose technology.
First generation sped up information structures
The first generation has used it to speed up the status quo ...
100 year old information hierarchies. They haven't used it to
take advantage of the internet's flexibility.
Second generation reshapes
The second generation will reshape the hierarchy to take
advantage of the internet. Wisdom Networks crowd create the
Network Society and deliver a rapid jump in productivity and
growth.
Repeating the innovations of the last 50 years
deliver 1.8%
If we match the pace of Information Age innovation over the
last 50 years, we can not achieve a GDP growth rate greater
than 1.8%. We are unlikely to achieve 1.8% GDP growth.
WISDOM
Only restructuring human time and attention can
deliver the radical jump in productivity and growth
Only restructuring human time and attention
Only human time and attention is all pervasive. Only the restructure of
human time can deliver a jump in productivity and growth
Wisdom Networks restructure human time and attention
Wisdom Networks:
● provide a more efficient means of person to person
communication ... and exponentially more efficient means
to share knowledge and focus wisdom and effort in real-
time
● deliver transparency, meritocracy, liquidity, and
accessibility to any community
● you only need one across the world to run multiple
communities ... Facebook organises 500m+
Technological Singularity
"Singularity is the point at which “all the change in the last million
years will be superseded by the change in the next five minutes.”
Kevin Kelly,Wired Magazine
“… a future period during which the pace of technological
change will be so rapid, its impact so deep, that human life will
be irreversibly transformed. Although neither utopian nor
dystopian, this epoch will transform the concepts that we rely on
to give meaning to our lives, from our business models to the
cycle of human life, including death itself.” – Ray Kurzweil
"The technological singularity is the theoretical emergence of
superintelligence through technological means." – Wikipedia

TeleHealth thrives in the "Internet of Everything" ... Do we get further exponential
benefits if we upgrade "Information Telehealth" to "Wisdom Telehealth"?
What is telehealth?
“Telehealth is the delivery of health-related services using telecommunications and related
technologies in the support of patient care, health education, and administrative activities.”
Although there has been a lot of talk and excitement about the potential of Telehealth for more than
twenty years, it’s time has truly come. This is made possible by technological developments but also
by the urgent need to contain costs spiralling out of control. The cost factor is what creates the political
will to embrace this concept.
Telehealth results in dramatic improvements in health outcomes
The Whole Systems Demonstrator (WSD) programme was established by the Department of Health
(DH) [in the UK] to evaluate how the use of telecare and telehealth technologies can support people
with long-term health and care needs to live independently.” The results of telehealth for patients with
long term health and care needs, as found by the WSD trial, in the UK were:
● 45% reduction in mortality rates
● 15% reduction in A&E visits
● 20% reduction in emergency admissions
● 14% reduction in elective admissions
● 14% reduction in bed days
● 8% reduction in tariff costs
● These results will also translate in savings of £1.9 Billion over the next 5 years
The inclusion of remote biosensing through the wireless network will improve health outcomes
by orders of magnitude and will do away with the need to develop old world infrastructures.
Telehealth - Information or Wisdom?
Current views of Telehealth are based on expensive information integration ... what would the
cost/benefit be if individuals at the point of health care service delivery were empowered with

Wisdom Networks make use of Internet of Everything (IoE)
What we connect? Everyone can see every "Thing" so
everybody can participate and contribute everywhere
How we connect? Every community is reduced to 6 types
of "Things" which are tagged, linked ,and shared to create
"Books" which consumers push through a process to achieve
outcomes
What is the Internet of Everything (IoE)?
According to Cisco, "the Internet of
Everything (IOE) brings together people
(humans), processes (manages the way
people, data, and things work together), data
(rich information), and things (inanimate
objects and devices) to make networked
connections more relevant and valuable than
ever before - turning information into actions
that create new capabilities, richer
experiences, and unprecedented economic
opportunity for businesses, individuals, and
countries.”
“$14.4 trillion of value (25% of global
corporate profits) will be created or
redistributed by the “Internet of Everything”
(IoE) between 2013 and 2022.”
What is The NWN?
Wisdom Networks organise the IoE using
"Things" collected into "Books" so that
everybody can see, participate or contribute
to each of their communities in real-time from
their mobile device or browser.

Wisdom Networks are an Over the Top disruptive platform
integrating technology, people and information
"The invasion of over-the-top (OTT) services has begun. It is time for telecommunication
operators to prepare their positions and choose their weapons to win the battles for mobile
voice, messaging, media, and cloud services.
We believe telecom operators will be best prepared to meet those challenges if they take
the following steps:
1. Choose your battlefields.
2. Redefine value propositions and business models.
3. Create new platforms and approaches.
4. Rethink organizational approaches.
5. Remain flexible and ready to adjust.
There cannot be one OTT strategy. Indeed, each battleground—voice, messaging,
media, and cloud—will demand a different strategy and a different combination of
offensive and defensive tactics. Some battles will be won, some lost. But overall, we
believe that telecom operators, with their consumer insights and inherent ability to deliver
breakthrough customer experiences, are uniquely positioned to win this war."
Winning the OTT War: Strategies for sustainable growth,
AT Kearney, September 2012
Is this the OTT Platform strategy to rule them all?:
1. Chooses a "Wisdom" battlefield, not information or community
2. Offers an integrated wisdom value proposition to consumers,
industry and society and aggregates OTT point solutions.
Consumers can choose to use other point solutions (ie GDocs for
collaboration or DropBox for file storage). The NWN's Single
Sign-on or Open-ID may be important.
3. Pioneers a single platform (or ONE OTT STRATEGY) that
leverages and integrates third party investments in community
and information point solutions.
4. provides a peer to peer structure to organisations and industry
in a Network Society
5. Is adaptable, flexible and leverages the collective innovation
of open source software community

Health Wisdom
What does health look like as a Wisdom Network?
Patient healthcare within a Network Society has different "things" and "books"
The screen below shows “Things” in the health community.
Complex reduced to 6 "Things"
The NWN reduces complex communities
to 6 types of "Things" and a few simple
interactions in a peer to peer network. The
few simple interactions are create, link,
share, tag and collaborate. This means
one cloud service can bring significant
change to all communities or industries at
the same time.
"Book" links "Things"
A “Book” shows how “Things” are linked.
Each community is just a combination of
“Things” and “Books”, but rely on the same
simple interactions.

Why?
Wisdom Networks deliver 80% of the outcome at <10% of the cost in 6 months
● Pareto principle - 80% of the outcomes for <20% of the effort or cost
○ an 80% outcome deployed quickly within 6 months
○ The pursuit of the additional 20% dramatically increases cost and probability of failure
● Global coverage encompassing all countries
○ Each country is simply a "tag" identifying where the "Thing" is
● A Wisdom Network is assembled from open source technology (not built from scratch)
○ Large global communities can deliver real-time merit based innovation and services
○ Open source has <25% of the Total Cost of Ownership of Proprietary software
○ Assembling open source faster than coding proprietary and a richer pool of innovation
● "Things" are simply links of data or files direct from their source (or where they were
produced)
○ By linking direct to source, some of the cost and delays of "Big Data" can be avoided. The Wisdom
Network just indexes, and does not need to produce the "Thing". Other SaaS can produce or modify
"Things".
○ Source include: A "personal data store" on a mobile phone, open data from enterprises and SME's
○ Avoid some "Big data" costs: Big data will drive US$28 billion of worldwide IT spending in 2012,
growing to US$34 billion in 2013, according to Gartner, Inc. Big Data to create 4.4 million IT jobs
globally by 2015
● Less hardware, software and communications technology
○ Wisdom Networks only require a handful of technologies
○ Web application: Linux Apache MySQL PHP
○ Cloud servers: Amazon
○ Devices: Tablets and phones (probably Android because of greater power and versatility)
○ Internet Browser:
○ Communications: Skype and Internet (or TCP/IP)
● 500 "Things" in one place mean less integration cost and complexity from integrating
fragmented, incompatible information technology
○ Desktop and most SaaS applications manage a handful of "things" (mainly "information" and "1-2
"collaboration")
General Benefits

Health Wisdom puts the IoE to work to deliver value and outcomes
"Internet of Everything (IoE) will enable better-connected devices and data-driven patient management,
resulting in improved healthcare effectiveness and efficiencies."
"Many of the inefficiencies in healthcare today are the result of siloed sources of knowledge and
information — it is difficult to access all of the relevant knowledge available at the point of care. In addition,
many measurements and tests are administered manually. The greater number of sensors and
connections in IoE will allow for shorter hospital stays due to smarter home monitoring systems and
improved care from standardized treatments that conform to all known best practices."
2013 Current state (without IoE) 2022 Potential with IoE
Long hospital stays to ensure patients can
thrive at home after discharge
Reduced costs and improved quality of life
from shorter hospital stays, with remote
monitoring systems that ensure health
Limited number of health conditions with
home / mobile monitoring capabilities
Wider number of health conditions with home
/ mobile monitoring capabilities
Uncoordinated and manual collection of
patient test records
Improved decision making from single
electronic collection of patient records
Ad-hoc interpretation of medical test results
and conditions
Improved patient care from standardized
treatments that conform to best practices
Multiple doctors offer care in an
uncoordinated manner
Improved patient care and health outcomes
from consolidated, patient-centric views of all
treatment aspects
● Value created: Continuous
monitoring of health conditions in a
less-expensive setting - home and
mobile devices; all care aspects
consolidated and coordinated
● Types of connection(s):
Machine-to-machine and machine-
to-people
● Technology enabler(s): Medical
devices, home IT connections,
mobile and wireless connections,
security
Patient Value Proposition
Before Wisdom Networks
● Patient doesn't "Own" and control his personal
records. They are scattered around all the
services providers
● Patients with chronic conditions must be
hospitalized for extended periods or make
regular visits to doctors and hospitals, often for
trivial, routine checks. This increases the risk of
contracting infections and other illnesses.
● Patient's data is collected only at these sporadic
visits
● Contribution to the patient's condition and
possible treatments are done by the doctor or
the hospital alone, in isolation. Health care pros
make accurate treatment decisions only 50% of
the time (Wellpoint)
● Compliance for elderly patients and/or patients
with chronic conditions is one of the industry's
major problems. There is no effective and
affordable solution
With Wisdom Networks
● Patient owns and controls ALL his medical
records. These are accessible to ALL the
providers as authorized by the patient with any
browser and without proprietary software
● Using remote biosensing devices hospital stays
are considerably reduced and routine visits for
trivial checks are no longer necessary.
● The quality of the data is infinitely superior
because it is harvested 24/7 and placed on the
record in real time. Alarm levels can be set for
immediate response.
● Crowdknowledge improves the accuracy and
quality of the treatment prescribed from 50% to
90%, at no extra cost
● The same device that collects the biosensing
data can provide audio visual messages to
improve compliance, reducing risk to patient and
cost to the system
"There are few areas more in need of supercharged decision-support than health care. Doctors and nurses are drowning in
information with new research, genetic data, treatments and procedures popping up daily. They often don’t know what to do,
and are guessing as well as they can. Health care pros make accurate treatment decisions only 50% of the time.
Crowdknowledge can increase the level of accuracy to 90% of the time.
The best part is the potential for distributing the intelligence anywhere via the cloud, right at the point of care. This could be
the most powerful tool we have seen to date for improving care and lowering everyone’s costs via standardization and
reduced error.” Samuel Nussbaum, Wellpoint Chief Medical Officer.
Crowd create patient
healthcare
Why not?
Why Not? Wisdom Networks will set fire to
your crops and asphyxiate your children!
Reluctance to change
“And typically, vested interests fight against change. People that have an interest in the old ways of
working, people that dominate face-to-face meetings and like that a lot, for example—they don’t want
to move toward some other kind of platform where it’s more of a meritocracy, rather than a personality
thing.”
– Don Tapscott
Lobbying by industry against wisdom
“Dear Mr. President: The canal system of this country is being threatened by a new form of
transportation known as ‘railroads’ … As you may well know, Mr. President, ‘railroad’ carriages are
pulled at the enormous speed of 15 miles per hour by ‘engines’ which, in addition to endangering life
and limb of passengers, roar and snort their way through the countryside, setting fire to crops, scaring
the livestock and frightening women and children. The Almighty certainly never intended that people
should travel at such breakneck speed."
– Martin Van Buren, Governor of New York, 1830.
Fear, uncertainty, doubt
“Rail travel at high speed is not possible, because passengers, unable to breathe, would die of
asphyxia"
– Dr Dionysys Larder (1793-1859), professor of Natural Philosophy and Astronomy,
University College London“
We are starting to reconsider existing information Age approaches to
information and security which will enable Wisdom networks
The patient owns personal information
"Personal information becomes legally recognized as yours While there is a push by the White House
and others to implement a consumer privacy Bill of Rights that increases consumer privacy, a larger shift is
already under way that will radically shift the security and privacy debate. In the future, you may own the
data that exists about you or that you create online, rather than it being owned by the companies that
collect it (like Amazon.com, Facebook, Google, and Twitter)."
Personal Wisdom Network shares with larger wisdom networks
"Personal data stores protect your information—In addition to enabling you to know about and control
your own information, data ownership will allow you to protect your information. Data stores will also help
us benefit from our own information."
Access to your personal data is a fundamental human right
"When you have legal rights over your information, you have more control over how it is collected and
used. This view that data about you is property, and that you own it, is gaining momentum, such as in the
United Kingdom, the World Economic Forum, and the European Union’s recognition of personal data
protection as a fundamental human right."
The Network asks the individual about access to their information in real-time
" ... the network itself will play an important role in improving IoE security. Currently, network intelligence is
mostly centralized and often resides far away from the “edge” of the Internet. As networks evolve along
with IoE, the ability to make intelligent decisions based on data as it enters and flows over the Internet will
be more distributed (located where ever it’s needed). With this ability, security threats, for example, could
not only be stopped, but also identified and located before any harm is done.
Answering the Two Most-Asked Questions About the Internet of Everything #IoE, Dave Evans , March 18, 2013
Crowd creating patient healthcare ....
is a feature of a
National Wisdom Network (NWN)
The National Wisdom Network (NWN)N unites people in the cloud and
reinvents how they live and work within all the communities within which they
participate.
Crowd create wisdom
Society has crowd created connectivity (internet), information (world wide web)
and software (open source software communities). Wisdom Networks builds
on the shoulders of these giants and crowd creates wisdom (knowledge and
action focused on outcomes). The NWN crowd creates a Network Society.
NWN brings wisdom to individuals in every community
NWN moves beyond the Information Age and organises the wisdom of crowds
to transform health, equity market, education, democracy and other fields of
human endeavour. Many current problems don’t need to be solved, because
they can be eliminated by solving the underlying problem.
Overcome the information problem
The underlying problem is that information is isolated, communities are opaque
and deriving profit from information discourages the inevitable and necessary
shift to transparency, access and a prevailing meritocracy which is the
environment in which wisdom can be achieved.
Wisdom Books achieve comprehensive, efficient person to person
communication
Wisdom Books reveal knowledge and focus wisdom about any "Thing",
anywhere, anytime to anyone and achieve comprehensive and efficient person
to person communication ... they upgrade fragmented, opaque and incomplete
person to person information technology at the heart of our information society.

of new information
● Community collaborates around each
patient
GOVERNMENT
● Health 3.0 assembles free open
source web technologies in one
year.
● Government can set national
standards for national electronic
health records
● Privacy/security are the
responsibility of the patient
PROVIDERS
● Providers participate in patient care
like they participate in Facebook
● They can download an open source
application to run their practice
ORGANISATIONS
● Organisations include hospitals,
insurance companies, pharmaceutical
companies and other service provides
● Organisations can integrate their
existing systems into the “Wisdom

Development 4.0
Other open source solutions can be integrated into the Wisdom
Network ... "things" in other systems are simply linked into Books
● Health 1.0 tries to integrate 100’s of proprietary and
incompatible technologies at a cost of billions over
many years
● Health 3.0 simply assembles and integrates
complementary, compatible, open source, web
based) and proven technology in a fraction of the time
and cost and a far higher probability of success
● Health 3.0 benefits
○ Total Cost of Ownership is 1 to 25% of
proprietary solutions
○ Launch within 90 days and then build toward
demand
○ Leverage the innovation of global open source
communities and contribute to a global community
○ Contribute to a global community
○ Developing countries could bypass Health 1.0
investments in a similar way that they bypassed
investment in telephone lines by going straight to
mobile phones – simpler, cheaper and better!
● Wisdom Network will index and aggregate "Things"
from existing systems. A comprehensive plan could
include free and open source software for offices and
enterprises within the community. This software is
more likely to be designed for the internet and able to
make data available by URL. http://en.wikipedia.org/wiki/List_of_freeware_health_software

Wisdom Networks: a cloud of things, books, communities, process and outcomes distributed among the people Wisdom Books: are a comprehensive and more efficient means of communication (compared to incompleted, opaque information technology) Health Wisdom: What does Health look like (as a wisdom network)? Crowdcreating Health is part of a National Wisdom Network and Network Society

  • 4. Distributed economies, society or communities are today’s structure to deliver productivity, sustainability and growth … 100 year old centralised structures are socio-economically and environmentally unsustainable Distributed economies (DE) are “a fresh strategy to guide industrial development towards becoming more sustainable. The concept calls for a transformation in the industrial system towards DE departing from the socio-economically and environmentally unsustainable dynamics associated with large-scale, centralised production units that are favoured by neoclassical economic drivers.”1 100 year old structures from the industrial Revolution Hierarchies use point to point communication and put 2 information elements in the internet Today’s structure Wisdom Networks enable peer to peer collaboration and put all 6 information elements in the internet 1. • 5. People are evolving toward digital "books" shared with everyone, everywhere, anytime across all communities ... and crowd creates the next an era of distributed prosperity! Facebook brought "books" to social in 10 years Facebook reduced social interaction to books of “things” that could shared with anyone, everywhere and restructured global social time and attention and achieved transparency and meritocracy in less than 10 years. Wisdom Networks bring "books" to everything else in 2-5 years Wisdom Networks will reduce organisations, industries and society to books that can be shared with anyone, everywhere, anytime and transform organisations, industry and the remainder of society within 2-5 years. Only 30 networks required to achieve distributed prosperity? As few as 30 wisdom networks can transform society and achieve distributed prosperity. Each can be launched within 90-180 days and achieve critical mass within 2-5 years. Hierarchy (individuals have influence) Shared Digital Books (everyone has influence)

  • 8. DISTRIBUTED DECENTRALISED WISDOM NETWORKS PROSPERITY Shell defines WHAT? two alternative scenarios are in our future ... Wisdom Networks are HOW? we crowd create a better world and prosperity @marcuscake WisdomNetworks.im CC BY-NC-SA 3.0 STATUS QUO CENTRALISED INFORMATION AGE STAGNATION Source: 1 Shell News Lens Scenarios (is.gd/newlens) Mountains (Shell scenario) "The first scenario, labelled “mountains”, sees a strong role for government and the introduction of firm and far-reaching policy measures. "1 "This is the world with status quo power locked in and held tightly by the currently influential. Stability is the highest prize: those at the top align their interests to unlock resources steadily and cautiously, not solely dictated by immediate market forces. The resulting rigidity within the system dampens economic dynamism and stifles social mobility." 1 Oceans (Shell Scenario) "The second scenario, which we call “oceans”, describes a more prosperous and volatile world. Energy demand surges, due to strong economic growth. Power is more widely distributed and governments take longer to agree major decisions. "1 "Influence stretches far and wide in the world of Oceans. Power is devolved, competing interests are accommodated and compromise is king. Economic productivity surges on a huge wave of reforms, yet social cohesion is sometimes eroded and politics destabilised. This causes much secondary policy development to stagnate, giving immediate market forces greater prominence."1 Who will choose (and create) the future? People, corporations or goverment We could implement this future in 2 - 5 years We could be in a "trapped transition" for another 16 years

"trapped transition" (by default) or "Punctuated reform" Punctuated reform (productivity, growth) "When stresses rise, and a crisis emerges, some actors exhibit relevant forms of resilience that enable them to adapt and reform. "1 "Financial, social, political or technological capital encourage early action and result in effective change/reform."1 Trapped transition (drift, decline, collapse) "Others, however, struggle until the crisis escalates to a level that enforces either dramatic and painful restructuring or collapse."1 "Financial, social, political or technological capacity prove inadequate to withstand stresses. Behavioural responses delay change, causing conditions to worsen until ultimately a reset is forced or a collapse occurs."1 Source: 1 Shell News Lens Scenarios (is.gd/newlens) @marcuscake WisdomNetworks.im CC BY-NC-SA 3.0
  • 11. Global Health Care faces enormous challenges A Wisdom Network overcomes the underlying problem with information structures and productivity Problem Global Health Care faces enormous challenges. ● Demographics – unprecedented population growth and ageing ● Information Age technologies that are isolated and opaque, require expensive integration and embed the status quo ● Demands on global health care are likely to increase 5 fold within 30 years ● Diminishing returns and declining productivity in the status quo means continuing permanent crisis The consequences for patients, providers and the industry are dire: ● Delivery costs escalating much faster than CPI ● Health professionals which don't have the knowledge or wisdom ● Unsatisfactory patient care and outcomes. ● 1 in 6 patients are poisoned, maimed or killed by the Western health care system The solution for these problems is at hand but requires a substantial cultural shift. The patient needs to be the focal point of health care. Community wisdom networks need to replace enterprise information technology. Solution ● The most productive economic asset is human time and attention. Wisdom Networks restructure human time and attention and recover time and attention lost with opaque Information Age structures and provide a rapid jump in productivity and capability. ● Wisdom Networks are an Over the Top (OTT) strategy that integrates fragmented information technologies and links to "Things" on the internet ● Wisdom Networks organise a 6 stage process to create wisdom ● Everyone in the health community can see every "thing", every "where" which delivers transparency. Transparency and rapid sharing of wisdom Books (knowledge and outcomes) means that a decision based upon meritocracy can be made.
  • 12. Demands on global health care likely to increase 5 fold within 30 years ● Population growing from 2 billion in 1960 to 9 billion in 2050. ● Average life expectancy in developed countries went from approximately 50 years in 1900 to almost 90 years now. ● The world’s middle class was approximately 1 billion people at the turn of this century, consuming 80% of all goods and services. This middle class will grow to 3 billion people by 2040. ● Some societies are moving from problems of malnourishment to problems of obesity inside one generation. Diminishing returns and declining productivity in the status quo means continuing permanent crisis Maintaining the status quo likely to deliver crisis: ● Diminishing returns from investment in the status quo means that productivity is declining or negative. ● Governments already under budget pressure and struggling to deliver services today. Insanity of investing in the status quo Due to diminishing returns, incremental investment in information or management technology will not increase productivity. Incremental investment in the status quo suffers diminishing returns and will not increase productivity, growth, patient outcomes or service future needs "Insanity: doing the same thing over and over again and expecting different results". - Albert Einstein
  • 13. General purpose technology drives most economic growth. First generation uses it within the status quo. The second generation reshapes the status quo. We can't afford to wait 30 years for the next generation to use the internet to reshape health Erik Brynjolfsson: The key to growth? Race with the machines, TED, February 2013 "Let's start this story when 120 years ago American factories started to electrify their operations igniting the second industrial revolution. The amazing thing is that productivity had not increased in those factories for 30 years. 30 years! That's long enough for a generation of managers to retire." "You see the first wave of managers simply replaced their steam engines with electric motors, but they didn't redesign the factories to take advantage of electricity's flexibility. It fell to the next generation to invent new work processes and then productivity soared often doubling or even tripling in those factories. Electricity is an example of a general purpose technology like the steam engine before it." "General purpose technologies drive most economic growth because they unleash cascades of complementary innovations like light bulbs and factory redesign." - Erik Brynjolfsson 1 The internet is a general purpose technology. First generation sped up information structures The first generation has used it to speed up the status quo ... 100 year old information hierarchies. They haven't used it to take advantage of the internet's flexibility. Second generation reshapes The second generation will reshape the hierarchy to take advantage of the internet. Wisdom Networks crowd create the Network Society and deliver a rapid jump in productivity and growth. Repeating the innovations of the last 50 years deliver 1.8% If we match the pace of Information Age innovation over the last 50 years, we can not achieve a GDP growth rate greater than 1.8%. We are unlikely to achieve 1.8% GDP growth.
  • 14. WISDOM Only restructuring human time and attention can deliver the radical jump in productivity and growth Only restructuring human time and attention Only human time and attention is all pervasive. Only the restructure of human time can deliver a jump in productivity and growth Wisdom Networks restructure human time and attention Wisdom Networks: ● provide a more efficient means of person to person communication ... and exponentially more efficient means to share knowledge and focus wisdom and effort in real- time ● deliver transparency, meritocracy, liquidity, and accessibility to any community ● you only need one across the world to run multiple communities ... Facebook organises 500m+ Technological Singularity "Singularity is the point at which “all the change in the last million years will be superseded by the change in the next five minutes.” Kevin Kelly,Wired Magazine “… a future period during which the pace of technological change will be so rapid, its impact so deep, that human life will be irreversibly transformed. Although neither utopian nor dystopian, this epoch will transform the concepts that we rely on to give meaning to our lives, from our business models to the cycle of human life, including death itself.” – Ray Kurzweil "The technological singularity is the theoretical emergence of superintelligence through technological means." – Wikipedia Information technology CountriesHierarchy INFORMATION AGE Wisdom Books Wisdom Networks Network Society

  • 16. TeleHealth thrives in the "Internet of Everything" ... Do we get further exponential benefits if we upgrade "Information Telehealth" to "Wisdom Telehealth"? What is telehealth? “Telehealth is the delivery of health-related services using telecommunications and related technologies in the support of patient care, health education, and administrative activities.” Although there has been a lot of talk and excitement about the potential of Telehealth for more than twenty years, it’s time has truly come. This is made possible by technological developments but also by the urgent need to contain costs spiralling out of control. The cost factor is what creates the political will to embrace this concept. Telehealth results in dramatic improvements in health outcomes The Whole Systems Demonstrator (WSD) programme was established by the Department of Health (DH) [in the UK] to evaluate how the use of telecare and telehealth technologies can support people with long-term health and care needs to live independently.” The results of telehealth for patients with long term health and care needs, as found by the WSD trial, in the UK were: ● 45% reduction in mortality rates ● 15% reduction in A&E visits ● 20% reduction in emergency admissions ● 14% reduction in elective admissions ● 14% reduction in bed days ● 8% reduction in tariff costs ● These results will also translate in savings of £1.9 Billion over the next 5 years The inclusion of remote biosensing through the wireless network will improve health outcomes by orders of magnitude and will do away with the need to develop old world infrastructures. Telehealth - Information or Wisdom? Current views of Telehealth are based on expensive information integration ... what would the cost/benefit be if individuals at the point of health care service delivery were empowered with Wisdom Networks?

According to Cisco, "the Internet of Everything (IOE) brings together people (humans), processes (manages the way people, data, and things work together), data (rich information), and things (inanimate objects and devices) to make networked connections more relevant and valuable than ever before - turning information into actions that create new capabilities, richer experiences, and unprecedented economic opportunity for businesses, individuals, and countries.” “$14.4 trillion of value (25% of global corporate profits) will be created or redistributed by the “Internet of Everything” (IoE) between 2013 and 2022.” What is The NWN? Wisdom Networks organise the IoE using "Things" collected into "Books" so that everybody can see, participate or contribute to each of their communities in real-time from their mobile device or browser.

  • 21. Wisdom Networks are an Over the Top disruptive platform integrating technology, people and information "The invasion of over-the-top (OTT) services has begun. It is time for telecommunication operators to prepare their positions and choose their weapons to win the battles for mobile voice, messaging, media, and cloud services. We believe telecom operators will be best prepared to meet those challenges if they take the following steps: 1. Choose your battlefields. 2. Redefine value propositions and business models. 3. Create new platforms and approaches. 4. Rethink organizational approaches. 5. Remain flexible and ready to adjust. There cannot be one OTT strategy. Indeed, each battleground—voice, messaging, media, and cloud—will demand a different strategy and a different combination of offensive and defensive tactics. Some battles will be won, some lost. But overall, we believe that telecom operators, with their consumer insights and inherent ability to deliver breakthrough customer experiences, are uniquely positioned to win this war." Winning the OTT War: Strategies for sustainable growth, AT Kearney, September 2012 Is this the OTT Platform strategy to rule them all?: 1. Chooses a "Wisdom" battlefield, not information or community 2. Offers an integrated wisdom value proposition to consumers, industry and society and aggregates OTT point solutions. Complex reduced to 6 "Things" The NWN reduces complex communities to 6 types of "Things" and a few simple interactions in a peer to peer network. The few simple interactions are create, link, share, tag and collaborate. This means one cloud service can bring significant change to all communities or industries at the same time. "Book" links "Things" A “Book” shows how “Things” are linked. Each community is just a combination of “Things” and “Books”, but rely on the same simple interactions. One platform, all community It is this innovation that mean all industries can be served by one wisdom network. The screenshot shows the health "Things" within a National Wisdom Network.

The Wisdom Network just indexes, and does not need to produce the "Thing". Other SaaS can produce or modify "Things". ○ Source include: A "personal data store" on a mobile phone, open data from enterprises and SME's ○ Avoid some "Big data" costs: Big data will drive US$28 billion of worldwide IT spending in 2012, growing to US$34 billion in 2013, according to Gartner, Inc. Big Data to create 4.4 million IT jobs globally by 2015 ● Less hardware, software and communications technology ○ Wisdom Networks only require a handful of technologies ○ Web application: Linux Apache MySQL PHP ○ Cloud servers: Amazon ○ Devices: Tablets and phones (probably Android because of greater power and versatility) ○ Internet Browser: ○ Communications: Skype and Internet (or TCP/IP) ● 500 "Things" in one place mean less integration cost and complexity from integrating fragmented, incompatible information technology ○ Desktop and most SaaS applications manage a handful of "things" (mainly "information" and "1-2 "collaboration") General Benefits ● Meritocracy ● Transparency ● Accessibility: ● Productivity ● Growth ● Accountability ● Wellbeing ● Utilisation increases ● global labour market ● Global capital market ● Global responsibility Developing countries can go straight to Wisdom Networks ● Developing countries saved billions by installing mobile phone networks and bypassed expensive copper fixed lines. ● Developing countries can save billions again by deploying comprehensive Wisdom Networks, rather than conventional, enterprise information technology and hierarchical management structures
  • 36. Health Wisdom puts the IoE to work to deliver value and outcomes "Internet of Everything (IoE) will enable better-connected devices and data-driven patient management, resulting in improved healthcare effectiveness and efficiencies." "Many of the inefficiencies in healthcare today are the result of siloed sources of knowledge and information — it is difficult to access all of the relevant knowledge available at the point of care. In addition, many measurements and tests are administered manually. The greater number of sensors and connections in IoE will allow for shorter hospital stays due to smarter home monitoring systems and improved care from standardized treatments that conform to all known best practices." 2013 Current state (without IoE) 2022 Potential with IoE Long hospital stays to ensure patients can thrive at home after discharge Reduced costs and improved quality of life from shorter hospital stays, with remote monitoring systems that ensure health Limited number of health conditions with home / mobile monitoring capabilities Wider number of health conditions with home / mobile monitoring capabilities Uncoordinated and manual collection of patient test records Improved decision making from single electronic collection of patient records Ad-hoc interpretation of medical test results and conditions Improved patient care from standardized treatments that conform to best practices Multiple doctors offer care in an uncoordinated manner Improved patient care and health outcomes from consolidated, patient-centric views of all treatment aspects ● Value created: Continuous monitoring of health conditions in a less-expensive setting - home and mobile devices; all care aspects consolidated and coordinated ● Types of connection(s): Machine-to-machine and machine- to-people ● Technology enabler(s): Medical devices, home IT connections, mobile and wireless connections, security
  • 37. Patient Value Proposition Before Wisdom Networks ● Patient doesn't "Own" and control his personal records. They are scattered around all the services providers ● Patients with chronic conditions must be hospitalized for extended periods or make regular visits to doctors and hospitals, often for trivial, routine checks. This increases the risk of contracting infections and other illnesses. ● Patient's data is collected only at these sporadic visits ● Contribution to the patient's condition and possible treatments are done by the doctor or the hospital alone, in isolation. Health care pros make accurate treatment decisions only 50% of the time (Wellpoint) ● Compliance for elderly patients and/or patients with chronic conditions is one of the industry's major problems. There is no effective and affordable solution With Wisdom Networks ● Patient owns and controls ALL his medical records. These are accessible to ALL the providers as authorized by the patient with any browser and without proprietary software ● Using remote biosensing devices hospital stays are considerably reduced and routine visits for trivial checks are no longer necessary. ● The quality of the data is infinitely superior because it is harvested 24/7 and placed on the record in real time. Alarm levels can be set for immediate response. ● Crowdknowledge improves the accuracy and quality of the treatment prescribed from 50% to 90%, at no extra cost ● The same device that collects the biosensing data can provide audio visual messages to improve compliance, reducing risk to patient and cost to the system "There are few areas more in need of supercharged decision-support than health care. Doctors and nurses are drowning in information with new research, genetic data, treatments and procedures popping up daily. They often don’t know what to do, and are guessing as well as they can. Health care pros make accurate treatment decisions only 50% of the time. Crowdknowledge can increase the level of accuracy to 90% of the time. The best part is the potential for distributing the intelligence anywhere via the cloud, right at the point of care. This could be the most powerful tool we have seen to date for improving care and lowering everyone’s costs via standardization and reduced error.” Samuel Nussbaum, Wellpoint Chief Medical Officer.
  • 39. Why Not? Wisdom Networks will set fire to your crops and asphyxiate your children! Reluctance to change “And typically, vested interests fight against change. People that have an interest in the old ways of working, people that dominate face-to-face meetings and like that a lot, for example—they don’t want to move toward some other kind of platform where it’s more of a meritocracy, rather than a personality thing.” – Don Tapscott Lobbying by industry against wisdom “Dear Mr. President: The canal system of this country is being threatened by a new form of transportation known as ‘railroads’ … As you may well know, Mr. President, ‘railroad’ carriages are pulled at the enormous speed of 15 miles per hour by ‘engines’ which, in addition to endangering life and limb of passengers, roar and snort their way through the countryside, setting fire to crops, scaring the livestock and frightening women and children. The Almighty certainly never intended that people should travel at such breakneck speed." – Martin Van Buren, Governor of New York, 1830. Fear, uncertainty, doubt “Rail travel at high speed is not possible, because passengers, unable to breathe, would die of asphyxia" – Dr Dionysys Larder (1793-1859), professor of Natural Philosophy and Astronomy, University College London“
  • 40. We are starting to reconsider existing information Age approaches to information and security which will enable Wisdom networks The patient owns personal information "Personal information becomes legally recognized as yours While there is a push by the White House and others to implement a consumer privacy Bill of Rights that increases consumer privacy, a larger shift is already under way that will radically shift the security and privacy debate. In the future, you may own the data that exists about you or that you create online, rather than it being owned by the companies that collect it (like Amazon.com, Facebook, Google, and Twitter)." Personal Wisdom Network shares with larger wisdom networks "Personal data stores protect your information—In addition to enabling you to know about and control your own information, data ownership will allow you to protect your information. Data stores will also help us benefit from our own information." Access to your personal data is a fundamental human right "When you have legal rights over your information, you have more control over how it is collected and used. This view that data about you is property, and that you own it, is gaining momentum, such as in the United Kingdom, the World Economic Forum, and the European Union’s recognition of personal data protection as a fundamental human right." The Network asks the individual about access to their information in real-time " ... the network itself will play an important role in improving IoE security. Currently, network intelligence is mostly centralized and often resides far away from the “edge” of the Internet. As networks evolve along with IoE, the ability to make intelligent decisions based on data as it enters and flows over the Internet will be more distributed (located where ever it’s needed). With this ability, security threats, for example, could not only be stopped, but also identified and located before any harm is done. Answering the Two Most-Asked Questions About the Internet of Everything #IoE, Dave Evans , March 18, 2013

  • 42. The National Wisdom Network (NWN)N unites people in the cloud and reinvents how they live and work within all the communities within which they participate. Crowd create wisdom Society has crowd created connectivity (internet), information (world wide web) and software (open source software communities). Wisdom Networks builds on the shoulders of these giants and crowd creates wisdom (knowledge and action focused on outcomes). The NWN crowd creates a Network Society. NWN brings wisdom to individuals in every community NWN moves beyond the Information Age and organises the wisdom of crowds to transform health, equity market, education, democracy and other fields of human endeavour. Many current problems don’t need to be solved, because they can be eliminated by solving the underlying problem. Overcome the information problem The underlying problem is that information is isolated, communities are opaque and deriving profit from information discourages the inevitable and necessary shift to transparency, access and a prevailing meritocracy which is the environment in which wisdom can be achieved. Wisdom Books achieve comprehensive, efficient person to person communication Wisdom Books reveal knowledge and focus wisdom about any "Thing", anywhere, anytime to anyone and achieve comprehensive and efficient person to person communication ... they upgrade fragmented, opaque and incomplete person to person information technology at the heart of our information society.
