2. Perform guest intake process to include orientation into the program, needs assessment, and
development of an individual case plan (to include steps and timetable). Timetable should reflect program
goal of secure employment and housing within 60-90 days.
Perform exit-process for guests to include exit interview, moving plan, and coordination of follow-up plan

Conduct and document weekly case meetings with each guest family to review case plan, track progress,
Set goals, deal with any issues, and provide guidance, counseling and needed resource referrals. Some
case meetings will occur at the congregations in the evening hours.
