        The undersigned would like to request for __________________________________________ _______________

Act (GAA).
          For approval of this request, the school hereby submits the following supporting documents:
          1.   Endorsement of the Schools Division Superintendent
          2.   Standard Letter-request for GAA Inclusion (QAAD Form No.8)
          3.   Copy of the Permit to Separate from Mother School
          4.   Copy of the Permit to Open and Establish the School
          5.   School Status Report
          6.   School Profile
          7.   Pertinent Documents that support the application for Permit to Open and Establish the School
                Inspection Report of the Division Inspection Team
                Feasibility Study
                Pertinent Documents of the School Site
                       Deed of Donation of the school site
                       Memorandum of Agreement between the school heads on the sharing of school site (applicable only if a school shares a
                       school site with the existing elementary/high school)
                Resolution of the Sangguniang Bayan/ Panlunsod to establish/open the school
                Certification of adequate Local Government Unit (LGU) support/funds for the operation of the school until such time
                 that the national government can take over
                Certification that there is no other high school, public or private, within 2-kilometers radius
                Certification that the following are available and adequate:
                       classroom teacher/s to handle the class/es
                       classroom/s (shoproom/s for Tech-Voc schools) to accommodate the grade/year level/s to be opened
                       chairs/desks, tables, blackboards
                       textbooks and other reference materials
                       equipment for Science, Math, TVE, MAPEH and other learning areas
                       funds for the initial operation of the school
                Plantilla of Personnel
                Credential of the Teacher/s who will handle the classes
                       Appointment/Designation
                       Transcript of Records
                       PRC License/Rating
