Purpose

 The purpose of this analysis is to help me
  understand my customers wants and needs.
  In my survey its ask questions about my
  potential customers interest in sports, how
  many injuries they incur in one year, and how
  much a potential customer would be willing
  to pay for these services, etc. With my
  analysis I plan understand what my potential
  customers want from my practice.

 How many sports injuries do you or they acquire in one
    year? i.e.. Siblings, friends
   What kind of injuries are they?
   What kind of sports do you or they play? i.e.. Siblings,
    friends.
   How do you treat your injuries?
   How often do you play sports weekly?
   Do you prefer to see a doctor for treatment of injuries?
   Are you willing to travel far to see a doctor for treatment?
    i.e.. Cast, braces, splints, etc.
   How much are you willing to pay for treatment? i.e.. Cast,
    braces, splints, etc.

willing to do something to self-treat

activities which may aggravate the
injured area more.

my potential patients will try to self-
treat themselves until they can see a
doctor.

Conclusion

 In conclusion, my potential want a doctor
  that is knowledgeable in their
  profession, who is flexible with office hours
  and can provide quality care at reasonable
  prices.
