Clinical experience and research over the last 30 years has shown that use of smallgauge spinal needles, particularly of the pencil-point design, is associated with a lower risk of PDPH than traditional cutting point needle tips (Quincke-point needles)

Como dato anecdótico, pero no menos importante, diseña e introduce el casco metálico usado por el ejército alemán en la 1ª Guerra Mundial. En los últimos años de su vida se aparta un poco de la medicina tradicional y escribe un artículo a favor de la homeopatia de Samuel Hahnemann. Gran amante de la naturaleza, en 1913 planta en su propiedad de Sauen un bosque siguiendo &quot;reglas filosóficas y biológicas naturales&quot;. Bier muere en 1949, a los 88 años, dejando un magnífico legado a la cirugía y a la anestesia.
Incidence Post-dural puncture headache is an iatrogenic complication of neuraxial blocks for labor analgesia. There is considerable variability in the incidence of PDPH, which is affected by several factors including age, gender, pregnancy, and needle type (design) and size (Table 1) [8, 12, 13]. In 1989, the incidence of PDPH attributable to the use of large gauge, cutting edge spinal needles was nearly 70%. Over time the use of fine gauge spinal has produced a great reduction in the incidence of this complication [8]. The clinical signs of PDPH may be observed following intentional dural puncture associated with the administration of spinal anesthesia or combined spinal-epidural anesthesia or unintentional dural puncture during epidural anesthesia [27]. Spinal anesthesia is a popular anesthetic technique for Cesarean delivery. The incidence of PDPH after spinal anesthesia varies greatly between studies. The incidence is 40% with a 20 GA needle; 25% with a 25 GA needle; 2–10% with a 26 GA needle, and less than 2% with a 29 GA needle [8]. However, technical difficulties are common when spinal block is attempted with needles of 29 GA or smaller. The principal factor responsible for the development of PDPH is the size of the dural perforation. Other factors such as the shape of the dural perforation and the orientation of the spinal needle have a less significant role. Therefore, a balance has to be struck between the risk of dural puncture and PDPH and technical failure [7]. Most experts agree that 25–27 GA needles probably represent the optimum needle size for spinal anesthesia. Clinical and laboratory studies confirmed that pencil-point needles produce fewer PDPHs than cutting edge spinal needles. As previously stated, the pregnant woman is at particular risk of dural puncture (and the subsequent PDPH) because of sex, young age, and the widespread application of neuraxial anesthesia [13, 18, 20–22, 28– 32]. Loss of resistance to air confers a higher risk of dural puncture than loss of resistance to fluid (normal saline) [18]. Unintentional dural puncture complicating 126

With spinal needles, the risk of PDPH diminishes with smaller diameter, atraumatic needles, but the risk is still appreciable (e.g., a 1 in 59 risk for Whitacre 27-G needles).
Onset and duration of PDPH Twenty studies reported temporal data on onset or duration of PDPH. Thirteen studies (five RCT,13,15,17,25,27 two cohort studies,31,32 and six case series46,48,49,52,55,59) reported the onset of PDPH, but six studies27,31,32,46,48,55 did not provide information on the number of patients for each day on which PDPH began (Table A). For epidural needles, the reported onset of PDPH ranged from less than one day to six days after ADP: 49 began within one day of ADP,49 20 began two days after ADP,49 15 began within two days,31 12 began after two days,59 30 began within three days,31 one began four days after ADP,25 55 began within five days,52 and 226 began within six days.52 For spinal needles, the reported onset of PDPH ranged from one to seven days after DP: two began one day after DP,49 74 began within two days of DP,15,32 67 began within three days,37,48,49 four began within four days,27 37 began within five days,49,55 and 38 began within seven days.46 The heterogeneous presentation of the data amongst studies prevented us from determining the median or mean time of onset for PDPH. Ten studies (three RCT,9,15,22 five cohort studies, 32,34,37–39 and two case series44,45) reported the duration of PDPH (Table A). No study reported data on the number of PDPH resolved by PDP day. Only one study45 reported the duration of PDPH after ADP from epidural needles. In the study by Holdcroft and colleagues, 13 of 1,000 patients experienced a PDPH, which lasted as long as six days.45 Treatment of PDPH was not reported. For spinal needles, the duration of PDPH ranged from one to seven days.15,22,32,34,37–39,44 Further analysis of the data on onset and duration was not attempted as the data were confounded by differences in prophylactic and therapeutic interventions between studies and by uncertainty on the length of patient follow-up. Median patient follow-up was six days in the eight studies9,17,27,31,33,37,39,49 that reported length of follow-up along with data on onset or duration of PDPH. Length of follow-up was unclear in the other 12 studies.

Bier is credited with the first description of the pathophysiology (leakage of CSF) of PDPH [6], and today there is no doubt that loss of CSF initiates the syndrome [7]. The classical description of the spinal dura mater (a dense, connective tissue layer surrounding the spinal cord) is of elastic and collagen fibers running it the longitudinal direction. Clinical studies based on this theory confirmed that PDPH was more likely when the cutting spinal needle was oriented perpendicular to the direction of the spinal dura fibers [7]. However, recent electron microscopic studies (which describe the dura mater as consisting of collages fibers arranged in several layers parallel to the surface) have challenged this classical description of the anatomy of the spinal dura mater [8]. These new studies revealed that each layer of the dura consists of both collagen and elastic fibers that do not demonstrate any specific orientation. In 1764, Cotugno first described the presence of a collection of fluid around the brain and spinal cord. In 1825, Magendie discovered that this fluid circulated around the brain and spinal cord [14]. The total volume of CSF in the adult subject is approximately 150 ml, 50% of which is within the cranium. About 500 ml of CSF is produced per day [7]. Production of CSF occurs primarily in the choroid plexus, but there is growing evidence of extrachoroidal CSF production [13, 15]. The CSF pressure in the lumbar region in the supine position ranges between 5 and 15 cm H2O, however, on assuming the vertical position, this pressure increases to over 40 cm H2O. Although the loss of CSF and subsequent decrease of the CSF pressure is not disputed, the actual mechanism producing the PDPH remains unclear [7, 8, 16–26]. The widely accepted theory explaining the pathophysiology of PDPH is based on the assumption of persistent leakage of CSF through the hole made by the spinal or epidural needle and decrease in CSF volume or pressure, or both, which leads to shifts of intracranial contents

Postdural puncture headache (PDPH) is a debilitating side effect of spinal anesthesia, the result of dural puncture and cerebrospinal fluid (CSF) leakage with an incidence of 3% to 75% in patient populations. Despite numerous in vitro and in viva studies that have identified predictors associated with PDPH, debate continues on the best technique to reduce CSF leak after dural puncture. The purpose of this in vitro study was to evaluate the relationship between spinal needle type (pencil tipped or cutting), needle size (22 or 25 gauge), and dura penetration angle from perpendicular (90 or 30 degrees), with the resulting CSF leak measured after dural puncture. Spinal needle designs continue to be studied and modified to reduce the incidence of PDPH in identified high-risk groups. For the study, 103 cadaver dura samples were punctured with randomly assigned needles at predetermined angles. The relationship between variables was analyzed. Our results found between needle tip designs a 5-fold increase in mean leak (Quincke > Whitacre) and between needle diameters (25 > 22 gauge), a 6-fold greater mean leakage. Puncture angle demonstrated no significant effect.
We have studied how subsequent placement of a subarachnoid catheter immediately after confirmation of a wet tap, and leaving the catheter in place for 24 hours affects the incidence of PDPH.

It has been reported that 70% pain relief has been achieved within 4 hours after caffeine (300-500 mg) was administered orally. Jarvis et al. reported that PDPH was relieved in 80% of the patients treated with intravenous 500 mg caffeine sodium benzoate (5,6). Al actuar mediante la liberación de esteroides adrenocorticales es menos previsible y menos específica en sus efectos que la hidrocortisona, y puede desencadenar mayor número de efectos secundarios2 Por último, mientras que se tiene gran experiencia con el uso de hidrocortisona en neonatología y obstetricia, no está aprobado el uso de la ACTH durante la lactancia24. De momento, sólo podemos especular con el mecanismo por el que los corticoides pueden ser eficaces en el tratamiento del síndrome CPPD, por tanto elegimos la hidrocortisona como corticoide tipo, suficientemente experimentado en obstetricia, con propiedades antiinflamatorias y a la vez acción leve sobre la bomba Na/K, que podría intervenir, de algún modo, en la producción de LCR22,2 La hidrocortisona, a las dosis empleadas en estos casos, puede considerarse segura siempre y cuando no se administre a mujeres con infecciones, diabetes, hipertensión o historia de ulcus gastroduodenal23. Creemos, que dadas las posibilidades que podría ofrecer esta nueva terapia, sería conveniente llevar a cabo ensayos clínicos que nos permitan establecer la efectividad de este tratamiento y precisar si puede ser una alternativa válida al parche hemático.
La primera teoría sugiere que el autólogo inyectados de sangre en el espacio epidural forma un coágulo, que se adhiere a la duramadre y parches directamente el agujero.
