Increasingly, online patient consultations have been replac-
ing face-to-face visits with real-time communication, including
the exchange of medical information. In aesthetic medicine,
online consults can help physicians give prospective patients a
preliminary idea of a proposed treatment plan prior to an in
person meeting. Historically, cloud-based, secure and reliable
online physician-patient “match-ups” have been difficult to come
by and implement successfully, but a recent start-up company is
ready to change all of that.

The recently released Zwivel platform from Zwivel, LLC (Paramus, N.J.) is
transforming the traditionally inefficient cosmetic consultation process by provid-
ing an online interactive prescreening tool, saving both aesthetic consumers and
practitioners valuable time. This online program integrates seamlessly within a
practice’s website, allowing the consultation process to begin remotely.
Using Zwivel’s online portal, prospective cosmetic patients can easily reach
out to a physician from their computer tablet or smartphone, or search for and
select from multiple practitioners based on factors such as proximity, specialty
and board certification status. Patients can submit information detailing the de-
sired procedures and outcomes; upload photos and record videos of them-
selves to illustrate their concerns about a single body part or multiple areas;
and answer some basic questions about their medical history, budget and time
frame for treatment. Physicians can then review the information and respond
with treatment recommendations and fee estimates. If the patient is satisfied with
the information received, he or she can ask for a full, in-person consultation.
“Zwivel is like Match.com for the aesthetic industry,” expressed plastic sur-
geon Gary D. Breslow, M.D., F.A.C.S., who is the co-founder and CEO of
Zwivel. “It does not replace the full, in-office consultation. Just like Match.com,
you still have to go out on the date, but you can find out if the date is worth
going on.”
According to Dr. Breslow, “Over 60% of all cosmetic consultations end up
with no procedure being performed. As a result, the process can be very inef-
ficient for both the physicians and patients, causing busy people to waste valu-
able time on consultations that go nowhere.”
Zwivel tackles this inefficiency head on, said Neil J. Zemmel, M.D., F.A.C.S.,
a plastic surgeon in Midlothian, Va., “Zwivel’s abilities run pretty deep. It lets us
bring the practice to the patient, and it gives them a really good feel for what
we do. We’ve gotten patients that have come into the office because of Zwivel,
and we’ve operated on many of them so far. It is a phenomenal online automa-
tion system that has changed the way we do business.”
For Robert Zubowski, M.D., a plastic surgeon in Paramus, N.J., any physician
running a high volume practice will see many people come into the office with
unworkable goals or unrealistic expectations in terms of surgical results. “It is
to the point where I’m trying to maximize the time I have for consultations and
avoid any that may be unnecessary,” he shared.
For instance, a prospective patient that comes in and is significantly over-
weight may want a procedure that is contraindicated, Dr. Zubowski expressed.
“If that patient was a Zwivel consultation, then I could have told her she needs to
lose weight before any procedures, and to come in after she loses the weight so
we can discuss her options. That process alone saves the patient a consultation
fee and time. Zwivel has helped minimize the number of those kinds of patients

and allows us to leave consultation slots open for someone who might be a better
candidate for a treatment.”
Dr. Breslow agreed, adding, “Aesthetic practitioners are some of the busiest
people in medicine. Many have long waiting lists of patients to get in to see them,
and no time to spare on consultations that don’t result in a procedure.”
Conversely, for cosmetic patients, “Internet search engines like Google can tell
you where a practice is located, contact information, the types of procedures
performed and gives you an overview of the practice in general,” Dr. Breslow con-
tinued. “However, they can’t tell you specifically what treatment would be best for
you, how much it will cost and who is the right physician for you. Zwivel creates a
sophisticated filter to fine tune that discovery process for each and every patient.”
The current version of Zwivel is free for both cosmetic patients and physicians,
so any aesthetic practitioner can sign up to start the program immediately. This
free tool called ZwivelDirect™, is uploaded to a practice’s website as a “Start Your
Consultation Now” widget, utilizing a colorful, eye-catching “pop up” design that
encourages use. The platform provides filters to ensure that physicians will only
receive requests for procedures that they perform.
“We live in a world where everything is Internet based, so my website is a very
important component to growing my practice,” expressed Robert Brueck, M.D.,
a plastic surgeon in Fort Myers, Fla. “Zwivel provides me with another option to
offer my patients. Also, many patients today like convenience, thus a platform
enabling an online consult is appealing to them.”
In addition to starting the Zwivel process via ZwivelDirect™, consumers can
also start their online consultation with multiple physicians simultaneously using
the ZwivelSearch™ platform located on Zwivel.com. They can go through the
same online consultation process as when they visit a practice’s website directly,
but with ZwivelSearch they have the option of selecting as many as five prac-
titioners to start a consultation with based on zip code, specialty and board
certification status.
Any physician who signs up to use Zwivel can be found on ZwivelSearch.
“We chose to limit search selections to five physicians at a time, because we
didn’t want people sending requests out to 50 or 100 physicians,” Dr. Breslow
explained. “Each physician gets a separate request and none of the other practi-
tioners can see that correspondence. It’s all on a one-to-one basis between each
patient and each clinician. The benefit of this service is that patients can receive
multiple opinions at the same time.”
The next edition of Zwivel will be a subscription-based premium service called
ZwivelPremium™, which is due in late 2015 and will carry even more interac-
tive features, Dr. Breslow added. “With this upgraded service, the physician
can save all the replies that they’ve sent and can reuse their videos for future
replies. They can also customize their replies and invite staff to participate in
the correspondence.”
Once patient and physician find one another through either the ZwivelDirect™
or ZwivelSearch™ platform, it proves to be cost-effective for both parties,

Dr. Zemmel reiterated. “Zwivel has allowed me to actually render a preliminary
medical opinion. It’s not a substitute for an in-person exam, of course, but I can
at least get a patient somewhere in the ballpark. And then we can even quote
them how much it’s going to cost, so the Zwivel process also helps to keep
patient expectations — at least their financial expectations — in line with what
we offer.”
Most prospective patients using Zwivel are reaching out tentatively and trying
to find that ballpark, too, Dr. Zubowski pointed out. “It doesn’t really seem like
they’re looking for a full consultation on Zwivel. They are basically saying this is
what I’m thinking. Is it realistic? Is my budget realistic? Do you agree that this is
something that would help me? If the answers to those questions are yes then they
will more likely than not book an appointment.”
For Dr. Zemmel, Zwivel has also increased website traffic and boosted conver-
sion rates. “Say you have a patient whose financial expectations are not reason-
able and they don’t have the budget or the resources,” he expressed. “Rather than
bringing them in and spending an hour doing a full consultation we’re able to
go online, render a preliminary opinion and let them know what the fee is going
to be. The patient might come back and say, ‘I need to save up a little bit more
money and then come in for a consultation in six months.’ It is a great system for
helping us run our business with very little waste.”
According to physician users, Zwivel requires prospective patients to provide
as much comprehensive information as possible when sending a request to prac-
titioners, which also gives them an opportunity to become more educated about
medical aesthetics prior to a face-to-face consultation.
“For instance, a patient might post on Zwivel, ‘I’m thinking about liposuction of
my abdomen and hips,” stated Dr. Zemmel. “I will look at their picture and see
that their weight is good and their height is good, but their photograph shows
that they have significant skin laxity. Therefore, they are not a good candidate for
a liposuction procedure. Instead, I would tell them that they need a tummy tuck.

send a video or selfies as part of the Zwivel consultation. After going through that
process, they may be able to more readily understand that they are indeed not a
good candidate. Before I have even met them face to face, I have refocused them
into the mindset of needing a more invasive procedure, such as a tummy tuck.”
After the practitioner gives the patient a quote on Zwivel and a consultation is
scheduled, another time saver occurs. “Then my staff becomes involved,” stated
Dr. Zubowski. “Somebody takes over from me, either my patient coordinator or
our front desk, and they use Zwivel to continue communicating with the patient.”
After a Zwivel patient has been scheduled for an in person consultation, a high
touch kind of communication has to be employed in order to successfully com-
plete the new patient process, he noted.
“You have to take care of these patients quickly and efficiently, and that in-
volves training staff on Zwivel and encouraging them to use it,” Dr. Zubowski
said. “It helps that my staff has been with me for over 20 years. If I dropped dead
during the consultation then they could probably finish it. Your staff has to know
exactly what to look for, but if they don’t know the answer to a question they
need to come to the physician. In my practice, in the course of the day, staff will
go online, check the Zwivel account and then post answers, so I don’t necessarily
have to do it all myself.”
Any time you introduce a new product, service or technology into a practice,
the practitioner has to expect some initial pushback from the staff. “There are
always concerns when we change or add a process, especially in the area of
patient outreach which is so critical to the business,” Dr. Zubowski continued.
When Zwivel was introduced to his staff, Dr. Zubowski’s office manager came
to him with a concern: “Oftentimes, when prospective patients come in for a
consultation, they don’t know yet whether or not they are a good candidate for
surgery or a procedure, or if they want to proceed with surgery at all,“ she noted.
“However, once they come in, meet the staff, see the office and how it is run,
then they see the value of the providers and the professional environment, which
is something that one can’t see or experience as easily on the Internet. Therefore,
wouldn’t that person be less likely to schedule a consult since they are remote?“
Despite the understandable logic in his office manager’s concern, Dr. Zubowski
advised that many Zwivel patients still call to make an appointment after submit-
ting a Zwivel request. “It seems that people want to get information quickly, and
Zwivel allows for that back and forth communication and patient education. If a
prospective patient calls for an appointment, and it’s a busy office then they may
have to wait several weeks to get in for a consultation. Zwivel allows the physician
or someone on staff to give a new patient immediate feedback regarding what
kind of procedure they want and whether their goals are realistic,” he said.
Practitioners also view Zwivel as an excellent adjunct to phone, email, website
and other methods used to capture new business. “Zwivel seems to have a lot
of functionality and capacity for patient recruitment, but it’s not something that
everyone would use,” Dr. Zemmel stated. “A lot of people continue to just make a
phone call for an appointment, but for the people that do use Zwivel, they say it
is very accessible and very easy to use.”

Zwivel’s ease of use and accessibility benefits the physician in other ways, Dr.
Brueck pointed out. “I am not always in the office when new Zwivel patients contact
me, but I can take my smartphone wherever I am and respond to a request. I try to an-
swer them as quickly as possible. The online consult is easy and does not have a steep
learning curve, and that is saying a lot for someone like me who is a computer flop.”
Since privacy issues are critical in online consults and Zwivel is entirely private,
secure and HIPAA/HITECH compliant. According to Dr. Breslow, “In the future,
we are planning on utilizing Zwivel’s HIPAA/HITECH compliant platform to en-
able physicians to not only interact with new patients, but conduct the same kind
of interactive, multimedia rich communication with existing patients. One of the
new tools in the works will accommodate Zwivel communication with existing
patients. The physician would send the patient a link that can be used to sign into
the system and create a HIPAA compliant portal between them.”
Even as Zwivel grows in complexity and feature richness, the system remains
simple to navigate, stated Dr. Zemmel. “We have not had anyone who doesn’t get
it,” he said. “In addition, I have never before seen such a positive response to a
program from both physicians and patients. It introduces efficiency into a process
that is traditionally very inefficient.”
According to Dr. Breslow, patient feedback has been overwhelmingly positive.
“Patients tell me all the time how great and cool this is. With 90% of all patients
being women – most having jobs, children or both – they don’t have time to go
from practice to practice looking for the right cosmetic physician. Zwivel stream-
lines the process for both parties.”
As Dr. Breslow maintained, “Zwivel makes the consultation process seamless
and more efficient. In a strange way, I am just as happy when patients don’t come
into the office after they go through the Zwivel process as when they do. This is
because I know that I just saved a lot of time and effort for both the patient and
myself. That is where Zwivel succeeds.”

for its ground-breaking research in fat reduction through controlled cooling.
Since the development of its market leading CoolSculpting® body contouring
treatment, pioneered by renowned Harvard University scientists Dieter Manstein,
M.D., Ph.D. and R. Rox Anderson, M.D., ZELTIQ has further invested in over ten
years of research, including scientific support in more than 50 published clinical
articles reporting on successful treatments and outcomes for more than 4,000
clinical trial patients. The system itself has been commercially available for more
than six years, with millions of treatments performed worldwide.
ZELTIQ has brought innovation to the aesthetic market by introducing applica-

The procedure works on the principle of cold panniculitis. Application of con-
trolled cooling can damage fat cells without effecting skin or surrounding tissues.
Fat cells are crystalized through the skin’s surface. As they die off, the body pro-
cesses the fat and cellular debris through its natural healing and waste removal
processes, resulting in selective non-invasive treatment of subcutaneous adipose
tissue. With long lasting results visible as early as three weeks, CoolSculpting
treatments are highly sought after by patients.

Aesthetic practitioners are still looking for just the right energy-based tech-
nology or combination of treatments that will achieve better outcomes with
little to no downtime. Most practices have likely acquired more than one
device to address various aesthetic indications – the non-invasive elimination
of fat being among the most requested.
Popular fat reduction energies include electromagnetic waves, ultrasound
and lasers as well as two other unique technologies: cryolipolysis, which
reduces fat by freezing it, and Selective Radiofrequency™ (RF), which is
proven to eradicate fat and was recently FDA cleared for circumferential reduc-
tion. The BTL Vanquish ME platform from BTL Aesthetics (Boston Mass.)
represents an advanced implementation of Selective RF. During treatment,
the unit’s non-contact device (or applicator) is positioned over the area to
be treated and delivers RF-based energy, initiating induced apoptosis in
the fat layers.
By comparison, the cryolipolysis fat reduction technique freezes a
specified area of fat using a cooling applicator that when applied to the
target area cools the underlying fat tissue without damage to the skin or
surrounding tissues.
Many physicians that purchased the CoolSculpting® cryolipolysis device
from ZELTIQ Aesthetics (Pleasanton, Calif.) later bought BTL Vanquish ME
as a complementary treatment to augment the system’s feature set. In some
cases the Vanquish ME replaced it altogether. Notably, many of the same
key opinion leaders that have lauded CoolSculpting for several years have
also noted that the BTL Vanquish ME is a fast, safe and effective way to treat
unwanted fat without downtime or complications. As well, it complements
the BTL Exilis device, which is used for skin tightening.

(IAPAM), added CoolSculpting in his practice two years ago, followed by
the BTL Exilis and then BTL Vanquish. “We needed the ability to treat skin
laxity, which CoolSculpting doesn’t provide,” he said.
The BTL Exilis comes in at this point, noted Dr. Kessler. “CoolSculpting has
been very nice for my healthy patients, but my older patients were getting
laxity. I researched skin tightening technologies and chose the BTL Exilis. I’ve
been using it over three years.”
When he upgraded to the BTL Vanquish ME, Dr. Kessler also saw its value
as a kind of aesthetic “finishing tool,” he added. “We’ve been able to smooth
out any irregularities resulting from cryolipolysis with the BTL Exilis and BTL
Vanquish ME. For instance, if you do both flanks on a person, one flank is
going to respond better than the other. Sometimes it’s a noticeable difference.
It has nothing to do with the technology of CoolSculpting; it’s just that every-
body’s body is different.”
As well, patients increasingly desire treatments on different areas of
the body and BTL Vanquish ME is a good adjunct to cryolipolysis in these
situations. “For instance, you can treat the inner thigh with CoolSculpting
and the outer thigh with BTL Vanquish ME. You can also treat the arms with
Vanquish ME. You would have to invest in additional applicators to treat
these areas with cryolipolysis.
For Peter J. Jenkin, M.D., F.A.A.D., a medical and cosmetic dermatologist in
Seattle, Wash., “The cryolipolysis science and technology made sense. It was
quite effective and we obtained consistent results,” he said. “But some patients
were uncomfortable, especially with the large handpiece when it was used in
the central abdomen.”
After three years of performing CoolSculpting treatments and having also
implemented the BTL Vanquish during that same period, Dr. Jenkin found that
he was using the Selective RF device more often and decided to replace his
cryolipolysis device with the BTL Vanquish ME.
According to Dr. Kessler, “With CoolSculpting it is one hour per site per ap-
plication, so when you’re looking at six areas with one device, it is six hours
of work. With the BTL Vanquish ME, you could conceivably do six areas with
the abdominal handpiece in four, 45-minute sessions. Yes, patients might have

to come back in four times, but not many patients are willing to stay for one
six hour treatment either.”
Since it can do a much larger area in less time, for Dr. Kessler, the BTL
Vanquish ME is much more time effective. “Also, it allows for a larger patient
pool and we haven’t had any complications or side effects with BTL Vanquish.”
“There is never a pain issue with BTL Vanquish ME, and it is not messy.
You can turn a room over in five minutes. There are no oils, gels or marking
up involved with treatment,” Mr. Russell maintained.
Ultimately, for many the bottom line in choosing BTL Vanquish ME comes
down to cost and return on investment. Specifically, Dr. Jenkin felt that,
“With CoolSculpting, the cost of consumables was high, so as the BTL
systems came along, the cryolipolysis device rapidly became marginalized
in my practice.”
In terms of business models, there is a clear advantage to employing the
BTL Vanquish ME, Dr. Kessler agreed. “Rather than paying a fixed fee for
each consumable that we use, which requires us to charge the patient a
certain amount for each area we treat, I have control over the price point,”
he said.
“Both systems are around $100K, so your per-month cost is going to be
the same,” Mr. Russell pointed out. “The big difference really is with BTL
Vanquish there are no consumables. The only other cost to consider is the
expense of your operator.”
BTL Vanquish has become a workhorse in the office, Dr. Kessler stat-
ed. “It is user and time friendly. From the patient’s experience, it’s also
very comfortable.”
Recently, Mr. Russell considered purchasing a second CoolSculpting
unit. “Having two units working together is popular. It’s call ‘dual sculpt-
ing’,” he said. However, after comparing the benefits of the cryolipolysis
and Selective RF technologies, he decided to purchase the BTL Vanquish
ME. “We have cut our treatment time in half and can help patients that we
weren’t able to treat before.”

However, Dr. Kessler pointed out one of CoolSculpting’s most important
benefits, from the practice business perspective. “You or someone on your
staff can position the patient, initiate the treatment, and then step away
and leave the area,” he said. The system has a pager button that patients
can use to summon staff. “With the BTL Vanquish ME device, the tempera-
ture is building and adipose tissue is getting hot, so somebody has to be
stationed nearby in order to lower the power or change the position of
the handpiece if the temperature gets too hot. Somebody has to be within
earshot of the treatment room.”
Patient safety and correct patient choice are a priority, Dr. Jenkin stated.
“With any energy-based device, it comes down to proper education of the
patient, giving them realistic expectations and also making sure they are the
right candidates for the procedure.”
Mr. Russell concurred, noting that it’s better to under promise and over
deliver. “Managing patient expectations is critical,” he said. “The ideal can-
didate wants to lose a few inches and also has to have realistic expectations.
This patient cares about the measurable changes, and we see people that run
from very firm to overweight.”
Ultimately, patient satisfaction is key, but so is practitioner satisfaction. “I’m
very pleased with both the BTL Exilis and BTL Vanquish ME,” Dr. Jenkin re-
ported. “They both have their own niche, patient satisfaction is very high and
the virtual complete absence of side effects and consumables makes it very
easy to deal with in terms of price point.”
According to Dr. Kessler, the complementary nature of CoolSculpting
and BTL Vanquish ME points the way towards more practitioners embrac-
ing energy-based technologies that interoperate well with other systems
and modalities. “We will never have that singular magic bullet kind of
device that is going to do everything we need to do,” he said. “This also
goes for plastic surgery procedures. How many different types of face-lifts,
breast lifts and other lifts are there? There is so much variability out there.
If you work with different technologies and techniques, then you learn the
best ways to use them.”

A distinguished faculty of luminaries and global thought leaders pre-
sented instructive live treatment demonstrations, hands-on workshops
and a diverse scientific program that covered all things relevant and cur-
rent in aesthetic medicine, including practice management, cutting edge
technologies and the newest procedures in the field.
TAS’s program began on Thursday, July 9 with three pre-show edu-
cational sessions. New this year was the industry’s first ever Feminine
Rejuvenation Symposium, which brought together world renowned
experts in non-surgical vulvovaginal rejuvenation techniques. This
course included live demonstrations of the latest technologies used in
these procedures.
As in previous years, the pre-show program also featured two ad-
vanced hands-on injectable training courses that provided attendees with
the principles and practices necessary to achieve optimal results for facial
shaping and rejuvenation, as well as wrinkle reduction using neurotoxins
and dermal fillers. These sessions included lectures and demonstrations
from the top physician injectors, such as William Philip Werschler, M.D.,
F.A.A.D., F.A.A.C.S.; Michael H. Gold, M.D.; Christopher Khorsandi,
M.D.; and Joseph Niamtu, III, D.M.D.
Each year, one of the highlights of the TAS pre-show session is THE
Certified Aesthetic Consultant (CAC) educational program, which covers
all aspects of running an aesthetic practice. Overseen by Jay A. Shorr
and Mara Shorr, this advanced training course offers a highly structured,
interactive platform with modules that educate and test attendees on mar-
keting, management skills, advanced practice metrics and more. Those
that successfully complete the curriculum and pass the final exam receive
national recognition as a Certified Aesthetic Consultant, which is the only
degree of its kind in our industry – helping all staff members gain the core
competencies critical to practice success.
On Friday, July 10, the main scientific program got underway with
THE Aesthetic Visionaries Welcome Address, in which the industry’s
major opinion leaders shared inspiring, forward-looking presentations
related to aesthetic medicine. Leif Rogers, M.D., F.A.C.S., emphasized
the show’s theme by discussing how to successfully manage the male
aesthetic patient. Red M. Alinsod, M.D., F.A.C.O.G., A.C.G.E., went
the opposite direction, delving into the new world of energy-based
feminine rejuvenation. Other important topics covered in this keynote
session included transdermal delivery of drugs and topicals, rescuing
botched patients via revision treatments and the developing market of
lipodissolving procedures.
Running through Sunday, the main program covered the foremost
topics in aesthetic medicine, including trends in body shaping and
skin tightening technologies; advanced injectable techniques and ap-
plications; the latest advances in anti-aging skin treatments, ranging
from topicals to the newest energy-based procedures; and even the
art of optimizing the business of skincare. Other sessions focused on
emerging procedures and technologies, including a session entirely
dedicated to feminine rejuvenation.
In addition to offering the most progressive educational program, TAS
continued its tradition of being the most entertaining meeting in the in-
dustry, underscored by THE Aesthetic Awards™, where physicians and
individual practices were recognized for their outstanding contributions

was a surprise cameo appearance by Justin Jedlica, also known as the
“Human Ken Doll,” who has had over 200 cosmetic surgery procedures
to achieve his unique look.
Incorporating two new co-hosts – plastic surgeon Leif Rogers, M.D. and
his practice partner, Rand Rusher, R.N. – the ceremony also featured an
expanded series of awards categories comprising some of the most inter-
esting emerging treatment segments including Best Patient Enhancement -
Lipofilling, Best Botched Patient Revision, Best Feminine Rejuvenation and
Best Male Patient Enhancement.
Additionally, Andrew P. Ordon, M.D., F.A.C.S., TAS’s scientific program
chairman was on hand to present his namesake Humanitarian Award to
Steven B. Hopping, M.D., a Washington D.C.-based plastic surgeon that
has been a tireless advocate for the disadvantaged, volunteering his
time and surgical skills to help those in need around the world. As well,
Howard Murad, M.D., honored David Matlock, M.D., F.A.C.O.G., with
the prestigious Howard Murad Lifetime Achievement Award for his life’s
work in the field of cosmetic surgery.
Throughout the entirety of TAS, Aesthetic TV® (ATV) was keenly pres-
ent, filming webisodes in the heart of the exhibit hall on the expansive
center stage. The ATV crew also filmed the awards show, conducted
news interviews and covered demonstrations of new products and tech-
nologies on the show floor.
Along these lines, the accompanying exhibit hall boasted more ven-
dors than ever, presenting a multitude of new products and services.
Among the most innovative technologies discussed was the novel internet
product, Zwivel from Zwivel, LLC (Paramus, N.J.). This unique consultation
platform enables physicians and patients to be matched in a convenient,
efficient and interactive manner. Potential patients quickly search for aes-
thetic physicians in their area that specialize in a given procedure, and
begin a cosmetic consultation with one or more physicians online from
their computer, smartphone or tablet without ever leaving their home.
Patients can share their concerns, indicate their procedure(s) of interest,
budget and timeframe, and even record a profile video. The selected
cosmetic physician(s) can then reply with their opinions, estimated fees
and a custom recorded video response.
TAS was not without advances in topical products such as Silagen®
Scar Therapy from NewMedical Technology, Inc. (Northbrook, Ill.). This
comprehensive line of physician-exclusive silicone scar treatments em-
ploys 100% pure medical grade silicone gel. Since keloid and thick scars
are formed by excess amounts of collagen production, when applied to
the skin, Silagen forms a flexible and invisible barrier over the scar. This
helps to prevent excessive transepidermal evaporative water loss and
aids in normalizing the production of collagen, keeping the creation of
keratinocytes under control. Silagen gel therapy is ideal for the face and
other visible areas, and can adhere better than silicone sheeting espe-
cially on areas of the body that move.
Among injectables, newly FDA approved dermal fillers were presented
at TAS as well. Radiesse® For Hands from Merz Aesthetics (Raleigh,
N.C.) is the first FDA cleared filler approved to restore lost volume in
the dorsum and can also help reduce the prominence of tendons and
veins. Delivering smooth, natural-looking results that can last up to one
year in many patients, this product not only provides immediate volume
and correction, but also continues to work by stimulating the body to

produce its own natural collagen. Made from calcium hydroxylapatite, it
is injected through a fine needle sometimes in multiple injections points,
and is followed by gentle massage.
Bellafill® from Suneva Medical, Inc. (San Diego, Calif.) also stood out
for having recently become the only dermal filler FDA cleared for the treat-
ment of acne scars. It is safe and effective for the correction of moderate to
severe, atrophic, distensible facial acne scars on the cheek in patients over
the age of 21. Bellafill adds volume to the skin to lift and smooth out pitted
acne scars to the level of the surrounding skin. This long lasting treatment is
a simple, in-office procedure with minimal to no downtime.
Staying on the topic of low or no downtime therapies, many energy-
based aesthetic devices were presented at TAS 2015. The PicoSure laser-
based platform from Cynosure (Westford, Mass.), which is already well
known for tattoo removal, recently received FDA approval for its new
532 nm wavelength upgrade, which is designed to more effectively treat
red, orange and yellow tattoo inks. PicoSure is now the only picosecond
laser platform with a combination 755 nm / 532 nm wavelength option
to treat the full color spectrum of inks in fewer sessions. It treats pigment-
ed lesions, but has also been FDA cleared as a skin revitalization tool.
Among other products in the laser arena, the recently released Halo™
from Sciton, Inc. (Palo Alto, Calif.), is a groundbreaking hybrid fractional
laser that performs advanced phototherapy and skin tightening for aging
and sun damaged skin. Combining 2940 nm and 1470 nm wavelengths,
this system can deliver ablative and non-ablative energy in the same pulse
with a consistent pattern that yields uniform results. The Erbium:YAG wave-
length, which is usually employed for full ablation, is provided in fraction-
ated form to reduce downtime while the new 1470 nm wavelength is used
to non-ablatively treat both epidermal and dermal pigmented lesions.
Jumping from lasers to microwaves, miraDry from Miramar Labs (Santa
Clara, Calif.), which is FDA cleared for permanent reduction of axillary
hyperhidrosis, recently received FDA approval for the treatment of un-
wanted underarm hair and permanent reduction of underarm hair of all
colors. miraDry utilizes miraWave Technology, a precisely delivered mi-
crowave energy combined with hydroceramic contact cooling to destroy
sweat and odor glands, as well as reduce underarm hair.
CoolSculpting® from ZELTIQ Aesthetics (Pleasanton, Calif.) is another
system featuring a unique technology – cryolipolysis – which non-invasive-
ly cools adipose tissue for fat reduction. The company showed a range
of new applicators that work with this popular treatment, most of which
utilize suction to hold target tissue in place while applying controlled
cooling. The CoolCore® or CoolMax® applicator is used to treat the
abdomen, while the CoolCurve® Plus applicator fits on and around the
curves of the body, such as on the love handles or flanks. CoolFit® is a
flat applicator that works on vertical fat in hard-to-treat areas like the in-
ner thigh and CoolSmooth® PRO is a surface applicator that works with
non-pinchable fat, such as on the outer thigh.
When it comes to energy-based body shaping systems, the ZWave Pro:
Body Shaping from Zimmer MedizinSystems (Irvine, Calif.), uses acoustic
radial waves to efficiently treat dermatologic and cosmetic conditions,
especially tissue weakness and cellulite. This device offers skin tightening
and body contouring results when used in combination with ultrasound,
low-level laser, radiofrequency (RF) and cryolipolysis procedures. Tissue
reactions and metabolic effects include a short-term increase of blood

blood flow caused by formation of blood vessels; lymphatic drainage ef-
fects; mechanical and biochemical lipolysis; softening of fibrotic structures
(septum, scar tissue); cell regeneration and collagen neogenesis. The
advantages of acoustic radial waves include an easy to operate non-
invasive procedure that is painless with no downtime.
Another interesting use of energy for body countouring can be found
in the the BTL Vanquish™ ME platform from BTL Aesthetics (Boston,
Mass.), which was recently FDA cleared for circumferential reduction of
the abdomen area. Using Selective Radiofrequency™ to non-invasively
address fat reduction, the system’s patented technology creates a high-
frequency energy field that targets the thermal effects into the fat layer
while protecting surrounding skin tissue. This system employs a panel
array that emits Selective RF energy with the largest spot size in the
industry, providing a clinically proven and safe treatment option that
doesn’t touch the patient’s skin
Continuing in the realm of radiofrequency technology, ThermiRF from
Thermi, LLC (Dallas, Texas) employs temperature controlled RF for innova-
tive approaches to a wide-range of face and body indications, resulting
in optimal skin tightening, fat reduction and body contouring outcomes.
The platform features a proprietary combination of percutaneous RF
energy delivered via a minimally invasive probe with two temperature
feedback mechanisms, including thermistor regulation of internal tissue
temperature and visual infrared monitoring of the surface skin heat lev-
els. ThermiRF’s range of handpieces include ThermiTight, which applies
subdermal heating to lift, tighten and rejuvenate the lower face and neck,
tighten skin on the abdomen, arms and other body areas, as well as
reduce cellulite. ThermiSmooth addresses the appearance of wrinkles
and lines on the skin, ThermiRase treats the nerves responsible for facial
muscle contractions and ThermiVa facilitates feminine rejuvenation by
reducing vaginal laxity.
In order to help physicians stay current with their technologies and
treatment offerings, Ascentium Capital (Kingwood, Texas) offers custom-
ized solutions to physicians and a streamlined process that makes financ-
ing fast and flexible. This includes a simple, one-page application for
loans up to $250,000 with credit decisions in as little as two hours.
Affordable financing and leasing structures are available for up to $1.5

In aesthetic medicine, online consults can help physicians give prospective patients a preliminary idea of a proposed treatment plan prior to an in person meeting.   • 3. 3 The recently released Zwivel platform from Zwivel, LLC (Paramus, N.J.) is transforming the traditionally inefficient cosmetic consultation process by provid- ing an online interactive prescreening tool, saving both aesthetic consumers and practitioners valuable time. This online program integrates seamlessly within a practice’s website, allowing the consultation process to begin remotely. Using Zwivel’s online portal, prospective cosmetic patients can easily reach out to a physician from their computer tablet or smartphone, or search for and select from multiple practitioners based on factors such as proximity, specialty and board certification status. Patients can submit information detailing the de- sired procedures and outcomes; upload photos and record videos of them- selves to illustrate their concerns about a single body part or multiple areas; and answer some basic questions about their medical history, budget and time frame for treatment. Physicians can then review the information and respond with treatment recommendations and fee estimates. If the patient is satisfied with the information received, he or she can ask for a full, in-person consultation. “Zwivel is like Match.com for the aesthetic industry,” expressed plastic sur- geon Gary D. Breslow, M.D., F.A.C.S., who is the co-founder and CEO of Zwivel. “It does not replace the full, in-office consultation. Just like Match.com, you still have to go out on the date, but you can find out if the date is worth going on.” According to Dr. Breslow, “Over 60% of all cosmetic consultations end up with no procedure being performed. As a result, the process can be very inef- ficient for both the physicians and patients, causing busy people to waste valu- able time on consultations that go nowhere.” Zwivel tackles this inefficiency head on, said Neil J. Zemmel, M.D., F.A.C.S., a plastic surgeon in Midlothian, Va., “Zwivel’s abilities run pretty deep. It lets us bring the practice to the patient, and it gives them a really good feel for what we do. We’ve gotten patients that have come into the office because of Zwivel, and we’ve operated on many of them so far. It is a phenomenal online automa- tion system that has changed the way we do business.” For Robert Zubowski, M.D., a plastic surgeon in Paramus, N.J., any physician running a high volume practice will see many people come into the office with unworkable goals or unrealistic expectations in terms of surgical results. “It is to the point where I’m trying to maximize the time I have for consultations and avoid any that may be unnecessary,” he shared. For instance, a prospective patient that comes in and is significantly over- weight may want a procedure that is contraindicated, Dr. Zubowski expressed. “If that patient was a Zwivel consultation, then I could have told her she needs to lose weight before any procedures, and to come in after she loses the weight so we can discuss her options. That process alone saves the patient a consultation fee and time.   • 6. 6 and allows us to leave consultation slots open for someone who might be a better candidate for a treatment.” Dr. Breslow agreed, adding, “Aesthetic practitioners are some of the busiest people in medicine. Many have long waiting lists of patients to get in to see them, and no time to spare on consultations that don’t result in a procedure.” Conversely, for cosmetic patients, “Internet search engines like Google can tell you where a practice is located, contact information, the types of procedures performed and gives you an overview of the practice in general,” Dr. Breslow con- tinued. “However, they can’t tell you specifically what treatment would be best for you, how much it will cost and who is the right physician for you. Zwivel creates a sophisticated filter to fine tune that discovery process for each and every patient.” The current version of Zwivel is free for both cosmetic patients and physicians, so any aesthetic practitioner can sign up to start the program immediately. This free tool called ZwivelDirect™, is uploaded to a practice’s website as a “Start Your Consultation Now” widget, utilizing a colorful, eye-catching “pop up” design that encourages use. The platform provides filters to ensure that physicians will only receive requests for procedures that they perform. “We live in a world where everything is Internet based, so my website is a very important component to growing my practice,” expressed Robert Brueck, M.D., a plastic surgeon in Fort Myers, Fla. “Zwivel provides me with another option to offer my patients. Also, many patients today like convenience, thus a platform enabling an online consult is appealing to them.” In addition to starting the Zwivel process via ZwivelDirect™, consumers can also start their online consultation with multiple physicians simultaneously using the ZwivelSearch™ platform located on Zwivel.com. They can go through the same online consultation process as when they visit a practice’s website directly, but with ZwivelSearch they have the option of selecting as many as five prac- titioners to start a consultation with based on zip code, specialty and board certification status. Any physician who signs up to use Zwivel can be found on ZwivelSearch. “We chose to limit search selections to five physicians at a time, because we didn’t want people sending requests out to 50 or 100 physicians,” Dr. Breslow explained. “Each physician gets a separate request and none of the other practi- tioners can see that correspondence. It’s all on a one-to-one basis between each patient and each clinician. The benefit of this service is that patients can receive multiple opinions at the same time.” The next edition of Zwivel will be a subscription-based premium service called ZwivelPremium™, which is due in late 2015 and will carry even more interac- tive features, Dr. Breslow added. “With this upgraded service, the physician can save all the replies that they’ve sent and can reuse their videos for future replies. They can also customize their replies and invite staff to participate in the correspondence.” Once patient and physician find one another through either the ZwivelDirect™ or ZwivelSearch™ platform, it proves to be cost-effective for both parties, Patients can start their consultation online via any smartphone, computer or tablet device.   • 7. 7 Dr. Zemmel reiterated. “Zwivel has allowed me to actually render a preliminary medical opinion. It’s not a substitute for an in-person exam, of course, but I can at least get a patient somewhere in the ballpark. And then we can even quote them how much it’s going to cost, so the Zwivel process also helps to keep patient expectations — at least their financial expectations — in line with what we offer.” Most prospective patients using Zwivel are reaching out tentatively and trying to find that ballpark, too, Dr. Zubowski pointed out. “It doesn’t really seem like they’re looking for a full consultation on Zwivel. They are basically saying this is what I’m thinking. Is it realistic? Is my budget realistic? Do you agree that this is something that would help me? If the answers to those questions are yes then they will more likely than not book an appointment.” For Dr. Zemmel, Zwivel has also increased website traffic and boosted conver- sion rates. “Say you have a patient whose financial expectations are not reason- able and they don’t have the budget or the resources,” he expressed. “Rather than bringing them in and spending an hour doing a full consultation we’re able to go online, render a preliminary opinion and let them know what the fee is going to be. The patient might come back and say, ‘I need to save up a little bit more money and then come in for a consultation in six months.’ It is a great system for helping us run our business with very little waste.” According to physician users, Zwivel requires prospective patients to provide as much comprehensive information as possible when sending a request to prac- titioners, which also gives them an opportunity to become more educated about medical aesthetics prior to a face-to-face consultation. “For instance, a patient might post on Zwivel, ‘I’m thinking about liposuction of my abdomen and hips,” stated Dr. Zemmel. “I will look at their picture and see that their weight is good and their height is good, but their photograph shows that they have significant skin laxity. Therefore, they are not a good candidate for a liposuction procedure. Instead, I would tell them that they need a tummy tuck.   • 8. 8 send a video or selfies as part of the Zwivel consultation. After going through that process, they may be able to more readily understand that they are indeed not a good candidate. Before I have even met them face to face, I have refocused them into the mindset of needing a more invasive procedure, such as a tummy tuck.” After the practitioner gives the patient a quote on Zwivel and a consultation is scheduled, another time saver occurs. “Then my staff becomes involved,” stated Dr. Zubowski. “Somebody takes over from me, either my patient coordinator or our front desk, and they use Zwivel to continue communicating with the patient.” After a Zwivel patient has been scheduled for an in person consultation, a high touch kind of communication has to be employed in order to successfully com- plete the new patient process, he noted. “You have to take care of these patients quickly and efficiently, and that in- volves training staff on Zwivel and encouraging them to use it,” Dr. Zubowski said. “It helps that my staff has been with me for over 20 years. If I dropped dead during the consultation then they could probably finish it. Your staff has to know exactly what to look for, but if they don’t know the answer to a question they need to come to the physician. In my practice, in the course of the day, staff will go online, check the Zwivel account and then post answers, so I don’t necessarily have to do it all myself.” Any time you introduce a new product, service or technology into a practice, the practitioner has to expect some initial pushback from the staff. “There are always concerns when we change or add a process, especially in the area of patient outreach which is so critical to the business,” Dr. Zubowski continued. When Zwivel was introduced to his staff, Dr. Zubowski’s office manager came to him with a concern: “Oftentimes, when prospective patients come in for a consultation, they don’t know yet whether or not they are a good candidate for surgery or a procedure, or if they want to proceed with surgery at all,“ she noted. “However, once they come in, meet the staff, see the office and how it is run, then they see the value of the providers and the professional environment, which is something that one can’t see or experience as easily on the Internet. Therefore, wouldn’t that person be less likely to schedule a consult since they are remote?“ Despite the understandable logic in his office manager’s concern, Dr. Zubowski advised that many Zwivel patients still call to make an appointment after submit- ting a Zwivel request. “It seems that people want to get information quickly, and Zwivel allows for that back and forth communication and patient education. If a prospective patient calls for an appointment, and it’s a busy office then they may have to wait several weeks to get in for a consultation. Zwivel allows the physician or someone on staff to give a new patient immediate feedback regarding what kind of procedure they want and whether their goals are realistic,” he said. Practitioners also view Zwivel as an excellent adjunct to phone, email, website and other methods used to capture new business. “Zwivel seems to have a lot of functionality and capacity for patient recruitment, but it’s not something that everyone would use,” Dr. Zemmel stated. “A lot of people continue to just make a phone call for an appointment, but for the people that do use Zwivel, they say it is very accessible and very easy to use.” THE Aesthetic Guide July/August 2015 www.miinews.com ”If a prospective patient calls for an appointment, and it’s a busy office then they may have to wait several weeks to get in for a consultation.   • 9. 9 Zwivel’s ease of use and accessibility benefits the physician in other ways, Dr. Brueck pointed out. “I am not always in the office when new Zwivel patients contact me, but I can take my smartphone wherever I am and respond to a request. I try to an- swer them as quickly as possible. The online consult is easy and does not have a steep learning curve, and that is saying a lot for someone like me who is a computer flop.” Since privacy issues are critical in online consults and Zwivel is entirely private, secure and HIPAA/HITECH compliant. According to Dr. Breslow, “In the future, we are planning on utilizing Zwivel’s HIPAA/HITECH compliant platform to en- able physicians to not only interact with new patients, but conduct the same kind of interactive, multimedia rich communication with existing patients. One of the new tools in the works will accommodate Zwivel communication with existing patients. The physician would send the patient a link that can be used to sign into the system and create a HIPAA compliant portal between them.” Even as Zwivel grows in complexity and feature richness, the system remains simple to navigate, stated Dr. Zemmel. “We have not had anyone who doesn’t get it,” he said. “In addition, I have never before seen such a positive response to a program from both physicians and patients. It introduces efficiency into a process that is traditionally very inefficient.” According to Dr. Breslow, patient feedback has been overwhelmingly positive. “Patients tell me all the time how great and cool this is. With 90% of all patients being women – most having jobs, children or both – they don’t have time to go from practice to practice looking for the right cosmetic physician. Zwivel stream- lines the process for both parties.” As Dr. Breslow maintained, “Zwivel makes the consultation process seamless and more efficient. In a strange way, I am just as happy when patients don’t come into the office after they go through the Zwivel process as when they do. This is because I know that I just saved a lot of time and effort for both the patient and myself.   • 28. 2828 Aesthetic practitioners are still looking for just the right energy-based tech- nology or combination of treatments that will achieve better outcomes with little to no downtime. Most practices have likely acquired more than one device to address various aesthetic indications – the non-invasive elimination of fat being among the most requested. Popular fat reduction energies include electromagnetic waves, ultrasound and lasers as well as two other unique technologies: cryolipolysis, which reduces fat by freezing it, and Selective Radiofrequency™ (RF), which is proven to eradicate fat and was recently FDA cleared for circumferential reduc- tion. The BTL Vanquish ME platform from BTL Aesthetics (Boston Mass.) represents an advanced implementation of Selective RF.   • 29. 29 (IAPAM), added CoolSculpting in his practice two years ago, followed by the BTL Exilis and then BTL Vanquish. “We needed the ability to treat skin laxity, which CoolSculpting doesn’t provide,” he said. The BTL Exilis comes in at this point, noted Dr. Kessler. “CoolSculpting has been very nice for my healthy patients, but my older patients were getting laxity. I researched skin tightening technologies and chose the BTL Exilis. I’ve been using it over three years.” When he upgraded to the BTL Vanquish ME, Dr. Kessler also saw its value as a kind of aesthetic “finishing tool,” he added. “We’ve been able to smooth out any irregularities resulting from cryolipolysis with the BTL Exilis and BTL Vanquish ME. For instance, if you do both flanks on a person, one flank is going to respond better than the other. Sometimes it’s a noticeable difference. It has nothing to do with the technology of CoolSculpting; it’s just that every- body’s body is different.” As well, patients increasingly desire treatments on different areas of the body and BTL Vanquish ME is a good adjunct to cryolipolysis in these situations. “For instance, you can treat the inner thigh with CoolSculpting and the outer thigh with BTL Vanquish ME. You can also treat the arms with Vanquish ME. You would have to invest in additional applicators to treat these areas with cryolipolysis. For Peter J. Jenkin, M.D., F.A.A.D., a medical and cosmetic dermatologist in Seattle, Wash., “The cryolipolysis science and technology made sense.   • 30. 3030 to come back in four times, but not many patients are willing to stay for one six hour treatment either.” Since it can do a much larger area in less time, for Dr. Kessler, the BTL Vanquish ME is much more time effective. “Also, it allows for a larger patient pool and we haven’t had any complications or side effects with BTL Vanquish.” “There is never a pain issue with BTL Vanquish ME, and it is not messy. You can turn a room over in five minutes. There are no oils, gels or marking up involved with treatment,” Mr. Russell maintained. Ultimately, for many the bottom line in choosing BTL Vanquish ME comes down to cost and return on investment. Specifically, Dr. Jenkin felt that, “With CoolSculpting, the cost of consumables was high, so as the BTL systems came along, the cryolipolysis device rapidly became marginalized in my practice.” In terms of business models, there is a clear advantage to employing the BTL Vanquish ME, Dr. Kessler agreed. “Rather than paying a fixed fee for each consumable that we use, which requires us to charge the patient a certain amount for each area we treat, I have control over the price point,” he said. “Both systems are around $100K, so your per-month cost is going to be the same,” Mr. Russell pointed out. “The big difference really is with BTL Vanquish there are no consumables. The only other cost to consider is the expense of your operator.” BTL Vanquish has become a workhorse in the office, Dr. Kessler stat- ed. “It is user and time friendly. From the patient’s experience, it’s also very comfortable.” Recently, Mr. Russell considered purchasing a second CoolSculpting unit. “Having two units working together is popular. It’s call ‘dual sculpt- ing’,” he said.   • 31. 31 However, Dr. Kessler pointed out one of CoolSculpting’s most important benefits, from the practice business perspective. “You or someone on your staff can position the patient, initiate the treatment, and then step away and leave the area,” he said. The system has a pager button that patients can use to summon staff. “With the BTL Vanquish ME device, the tempera- ture is building and adipose tissue is getting hot, so somebody has to be stationed nearby in order to lower the power or change the position of the handpiece if the temperature gets too hot. Somebody has to be within earshot of the treatment room.” Patient safety and correct patient choice are a priority, Dr. Jenkin stated. “With any energy-based device, it comes down to proper education of the patient, giving them realistic expectations and also making sure they are the right candidates for the procedure.” Mr. Russell concurred, noting that it’s better to under promise and over deliver. “Managing patient expectations is critical,” he said. “The ideal can- didate wants to lose a few inches and also has to have realistic expectations. This patient cares about the measurable changes, and we see people that run from very firm to overweight.” Ultimately, patient satisfaction is key, but so is practitioner satisfaction. “I’m very pleased with both the BTL Exilis and BTL Vanquish ME,” Dr. Jenkin re- ported. “They both have their own niche, patient satisfaction is very high and the virtual complete absence of side effects and consumables makes it very easy to deal with in terms of price point.” According to Dr. Kessler, the complementary nature of CoolSculpting and BTL Vanquish ME points the way towards more practitioners embrac- ing energy-based technologies that interoperate well with other systems and modalities. “We will never have that singular magic bullet kind of device that is going to do everything we need to do,” he said. “This also goes for plastic surgery procedures. A distinguished faculty of luminaries and global thought leaders pre- sented instructive live treatment demonstrations, hands-on workshops and a diverse scientific program that covered all things relevant and cur- rent in aesthetic medicine, including practice management, cutting edge technologies and the newest procedures in the field. TAS’s program began on Thursday, July 9 with three pre-show edu- cational sessions. New this year was the industry’s first ever Feminine Rejuvenation Symposium, which brought together world renowned experts in non-surgical vulvovaginal rejuvenation techniques. This course included live demonstrations of the latest technologies used in these procedures. As in previous years, the pre-show program also featured two ad- vanced hands-on injectable training courses that provided attendees with the principles and practices necessary to achieve optimal results for facial shaping and rejuvenation, as well as wrinkle reduction using neurotoxins and dermal fillers. These sessions included lectures and demonstrations from the top physician injectors, such as William Philip Werschler, M.D., F.A.A.D., F.A.A.C.S.; Michael H. Gold, M.D.; Christopher Khorsandi, M.D.; and Joseph Niamtu, III, D.M.D. Each year, one of the highlights of the TAS pre-show session is THE Certified Aesthetic Consultant (CAC) educational program, which covers all aspects of running an aesthetic practice. Overseen by Jay A. Shorr and Mara Shorr, this advanced training course offers a highly structured, interactive platform with modules that educate and test attendees on mar- keting, management skills, advanced practice metrics and more. Those that successfully complete the curriculum and pass the final exam receive national recognition as a Certified Aesthetic Consultant, which is the only degree of its kind in our industry – helping all staff members gain the core competencies critical to practice success. On Friday, July 10, the main scientific program got underway with THE Aesthetic Visionaries Welcome Address, in which the industry’s major opinion leaders shared inspiring, forward-looking presentations related to aesthetic medicine. Leif Rogers, M.D., F.A.C.S., emphasized the show’s theme by discussing how to successfully manage the male aesthetic patient. Red M. Alinsod, M.D., F.A.C.O.G., A.C.G.E., went the opposite direction, delving into the new world of energy-based feminine rejuvenation. Other important topics covered in this keynote session included transdermal delivery of drugs and topicals, rescuing botched patients via revision treatments and the developing market of lipodissolving procedures. Running through Sunday, the main program covered the foremost topics in aesthetic medicine, including trends in body shaping and skin tightening technologies; advanced injectable techniques and ap- plications; the latest advances in anti-aging skin treatments, ranging from topicals to the newest energy-based procedures; and even the art of optimizing the business of skincare. Other sessions focused on emerging procedures and technologies, including a session entirely dedicated to feminine rejuvenation. In addition to offering the most progressive educational program, TAS continued its tradition of being the most entertaining meeting in the in- dustry, underscored by THE Aesthetic Awards™, where physicians and individual practices were recognized for their outstanding contributions to the profession. Incorporating two new co-hosts – plastic surgeon Leif Rogers, M.D. and his practice partner, Rand Rusher, R.N. – the ceremony also featured an expanded series of awards categories comprising some of the most inter- esting emerging treatment segments including Best Patient Enhancement - Lipofilling, Best Botched Patient Revision, Best Feminine Rejuvenation and Best Male Patient Enhancement. Additionally, Andrew P. Ordon, M.D., F.A.C.S., TAS’s scientific program chairman was on hand to present his namesake Humanitarian Award to Steven B. Hopping, M.D., a Washington D.C.-based plastic surgeon that has been a tireless advocate for the disadvantaged, volunteering his time and surgical skills to help those in need around the world. As well, Howard Murad, M.D., honored David Matlock, M.D., F.A.C.O.G., with the prestigious Howard Murad Lifetime Achievement Award for his life’s work in the field of cosmetic surgery. Throughout the entirety of TAS, Aesthetic TV® (ATV) was keenly pres- ent, filming webisodes in the heart of the exhibit hall on the expansive center stage. The ATV crew also filmed the awards show, conducted news interviews and covered demonstrations of new products and tech- nologies on the show floor. Along these lines, the accompanying exhibit hall boasted more ven- dors than ever, presenting a multitude of new products and services. Among the most innovative technologies discussed was the novel internet product, Zwivel from Zwivel, LLC (Paramus, N.J.). This unique consultation platform enables physicians and patients to be matched in a convenient, efficient and interactive manner. Potential patients quickly search for aes- thetic physicians in their area that specialize in a given procedure, and begin a cosmetic consultation with one or more physicians online from their computer, smartphone or tablet without ever leaving their home. Patients can share their concerns, indicate their procedure(s) of interest, budget and timeframe, and even record a profile video. The selected cosmetic physician(s) can then reply with their opinions, estimated fees and a custom recorded video response. TAS was not without advances in topical products such as Silagen® Scar Therapy from NewMedical Technology, Inc. (Northbrook, Ill.). This comprehensive line of physician-exclusive silicone scar treatments em- ploys 100% pure medical grade silicone gel. Since keloid and thick scars are formed by excess amounts of collagen production, when applied to the skin, Silagen forms a flexible and invisible barrier over the scar. This helps to prevent excessive transepidermal evaporative water loss and aids in normalizing the production of collagen, keeping the creation of keratinocytes under control. Made from calcium hydroxylapatite, it is injected through a fine needle sometimes in multiple injections points, and is followed by gentle massage. Bellafill® from Suneva Medical, Inc. (San Diego, Calif.) also stood out for having recently become the only dermal filler FDA cleared for the treat- ment of acne scars. It is safe and effective for the correction of moderate to severe, atrophic, distensible facial acne scars on the cheek in patients over the age of 21. Bellafill adds volume to the skin to lift and smooth out pitted acne scars to the level of the surrounding skin. This long lasting treatment is a simple, in-office procedure with minimal to no downtime. Staying on the topic of low or no downtime therapies, many energy- based aesthetic devices were presented at TAS 2015. The PicoSure laser- based platform from Cynosure (Westford, Mass.), which is already well known for tattoo removal, recently received FDA approval for its new 532 nm wavelength upgrade, which is designed to more effectively treat red, orange and yellow tattoo inks. PicoSure is now the only picosecond laser platform with a combination 755 nm / 532 nm wavelength option to treat the full color spectrum of inks in fewer sessions. It treats pigment- ed lesions, but has also been FDA cleared as a skin revitalization tool. Among other products in the laser arena, the recently released Halo™ from Sciton, Inc. (Palo Alto, Calif.), is a groundbreaking hybrid fractional laser that performs advanced phototherapy and skin tightening for aging and sun damaged skin. Combining 2940 nm and 1470 nm wavelengths, this system can deliver ablative and non-ablative energy in the same pulse with a consistent pattern that yields uniform results. The Erbium:YAG wave- length, which is usually employed for full ablation, is provided in fraction- ated form to reduce downtime while the new 1470 nm wavelength is used to non-ablatively treat both epidermal and dermal pigmented lesions. Jumping from lasers to microwaves, miraDry from Miramar Labs (Santa Clara, Calif.), which is FDA cleared for permanent reduction of axillary hyperhidrosis, recently received FDA approval for the treatment of un- wanted underarm hair and permanent reduction of underarm hair of all colors. miraDry utilizes miraWave Technology, a precisely delivered mi- crowave energy combined with hydroceramic contact cooling to destroy sweat and odor glands, as well as reduce underarm hair. CoolSculpting® from ZELTIQ Aesthetics (Pleasanton, Calif.) is another system featuring a unique technology – cryolipolysis – which non-invasive- ly cools adipose tissue for fat reduction. The company showed a range of new applicators that work with this popular treatment, most of which utilize suction to hold target tissue in place while applying controlled cooling. The CoolCore® or CoolMax® applicator is used to treat the abdomen, while the CoolCurve® Plus applicator fits on and around the curves of the body, such as on the love handles or flanks. CoolFit® is a flat applicator that works on vertical fat in hard-to-treat areas like the in- ner thigh and CoolSmooth® PRO is a surface applicator that works with non-pinchable fat, such as on the outer thigh. When it comes to energy-based body shaping systems, the ZWave Pro: Body Shaping from Zimmer MedizinSystems (Irvine, Calif.), uses acoustic radial waves to efficiently treat dermatologic and cosmetic conditions, especially tissue weakness and cellulite. This device offers skin tightening and body contouring results when used in combination with ultrasound, low-level laser, radiofrequency (RF) and cryolipolysis procedures.
