• Corrective Actions & Learning
4. Near-Miss
What defines a Near-Miss Incident?
– Occupational event that is unplanned and
unexpected within reason
– Has potential to cause an accident or incident
– Workplace damage, or injury /illness causing
harm to an individual
EXAMPLE:
Almost slipping on a wet floor
4
5. Incident
What constitutes a workplace Incident?
– Unplanned and unexpected
– Results in NO lost time
– Results in NO injury
– Results in property loss that involves
occupational hazards (uncontrolled hazards)
EXAMPLE:
Material and floor damage as a result of a
wet floor
5
6. Accident
Defining a workplace Accident:
– Occupational accident that is unplanned and
unexpected
– Results in an injury/illness and/or property loss
– Involved occupational hazards that are
uncontrolled
EXAMPLE:
Worker slipping and falling on wet floor, breaking
an arm as result of trying to break their fall.

7. Preparation & Practiced for
Occurrences
Prior to an
Occurrence:
Prepared Investigation
Procedure
Response Team
Prepared Material
Environmental Awareness
Awareness & Response to
Hazards & Controls
8. Conducting Investigations
You want to determine contribution
and/or causation of the following:
People:
•You need to determine insight from what happened
•Determine their thought-process in the
investigation
Equipment:
•How is it being used/operated?
• What is the maintenance schedule?
9. Conducting Investigations
You want to determine contribution and/or
causation of the following:
Material:
•How long has it been there?
•How is it used and who uses it?
Environment :
•Is the workplace layout suitable for the type of work
•What about the sounds, temperature, traffic?
9
10. Conducting Investigations
You want to determine contribution and/or
causation of the following:
Process:
•What is the size of the company?
•What are the jobs and their tasks?
•What are the standards and procedures?
•Training schedule?
10
12. When Investigating an Accident….
Understand the Equipment, Material, Environment,
and Process
Prior to interviewing people…Understanding the
mechanic aspects of the occurrence first will guide
you to understand the human factor
12
13. Conducting Investigations
At the Scene:
•No chance of reoccurrence
•Gain insight of layout and function
•Account for all workers
•Account for actual and possible contributing hazards
and controls
•Take pictures, and obtain drawing and
documentation
•Appraise and account for any physical damage
15. Conducting Investigations
Nature of injury:
•Understand and describe sustain injury(s)
•Understand if there are pre-existing conditions
•Measure the impact of the loss
16. Critical Injury Response
What is a Critical Injury?
•Places life in jeopardy
•Produces unconsciousness
•Results in substantial loss of blood
•Involves the fracture of a limb
•Involves amputation of a limb
•Consists of burns to major parts of the body
•Loss of eye sight
•Hospital stay of more than two days
17. Critical Injury Response
• Seek Medical attention immediately
• Notify Leadership Teams or Senior Management
immediately
• Notify appropriate authorities
• Be prepared to deal with Media
• Investigate with authorities
• Report
19. Document and Analysis
Documentation Findings:
•Root Cause
•Real or underlying cause(s) of an event
•Immediate Causes
•Apparent/obvious cause(s) of injury
•Contributing factors Involved in an accident
20. Documentation and Analysis
Contains:
•A description of the accident
•The consequences
•The causes, especially root and underlying
•Recommended corrective actions
21. Corrective Actions & Learning
• Sustainable Solutions
• Learn from mistakes and prevent re-occurrences
• Communicate with your team

Sandy – How many people have conducted a workplace investigation in the past?
Near-Miss- is referring to an occupational event that is unplanned and unexpected within reason could have the potential of causing an accident or incident. ***Just to note there are Health Hazards that workers can be exposed to and are categorized as Environmental, Physical, Biological, chemical, Physcosocial, and Ergonomic. Safety Hazards are referring to mechanical & Machine, fire, conveyor and moving parts, equipments and tool, electrical, material handling, fall hazards, etc….*** 2. Incidents is referring to an occupational incident is an unplanned and unexpected event resulting in no lost time injury and/or property loss involving occupational hazards that are uncontrolled. 3. Accident is referring to an occupational accident that is unplanned and unexpected event resulting in an injury and/or property loss involving an occupational that are uncontrolled.
Incidents is referring to an occupational incident is an unplanned and unexpected event resulting in no lost time injury and/or property loss involving occupational hazards that are uncontrolled.
INSERT POLL: Have you ever experienced one of these occurrences in the workplace? A) Yes B)NO C) I prefer not say Accident is referring to an occupational accident that is unplanned and unexpected event resulting in an injury and/or property loss involving an occupational that are uncontrolled.
-Preparation is key in Conducting Investigations Make Certain you have the following Do you have a procedure in place also be aware that there are legislative requirements for documenting and reporting an accident. You should prepare a team that can respond to an accident, incident, or near miss assist workers, ensure the workplace is secure, and document and report the occurrences. You should be prepared for an emergency situation by having the first aid response, understanding of the dangers and risk in the workplace, you should have Investigation forms and witness forms prepare and easy to obtain if there is an occurrence that required and investigation. Workplace Environment are you aware of the emergency features and controls in you operation? You should be familiar and be able to evaluate in a controlled and uncontrolled in your operations: People Equipment Material Environment Process Are you aware of the risk associated with hazards and how to respond to them.
GO OVER POLL RESULTS HERE BEFORE SLIDE STARTS Who, What, Where, Why, and How for each. People will make decisions we need to gain insight from their experience and thought process. Equipment- How is it being used, how it operated, maintenance schedule and service dates, Material,- How long has the material been the workplace, this there a difference in size and shape, how is it used, who is in contact with it, Environment, -Is the workplace layout suitable for the work being conducted, is it congested, is it loud, is the temperature to hot or cold. Process,- Size of the organization, Jobs and tasks, standards and procedures, best practices, and training.

1. Near Miss, Injury or Accident 2. Critical Injury 3. Your Team 4. No chance of reoccurence 5. Name + phone #
My ideal preference when investigating accidents, incidents, or near misses is to understand Equipment, Material, Environment, and Process. Prior to interviewing people. Understanding the mechanic aspects of the occurrence first will guide you to understand human factor associated with – Example to draw this process
The Investigator will need to survey the scene of the accident to determine what happen, some of the things to consider when surveying the scene are: Ensure the scene is secure that there are no further chance of injury occurring. Investigate with someone who is familiar with the workplace and explain layout and function. Have someone account for all workers at the location. Identify if anyone might be missing. Begin to identify all key hazards. Take pictures, and draw if possible, obtain documentation. Be attuned and your senses when investigating. Appraise and account for any physical damage to the work environment.
POLL 2: DOES YOUR ORGANIZATION HAVE AN EAP IN PLACE? YES NO
You need to understand the impact of the loss by understanding what is involved in the recovery process. This could be inclusive of understanding impact of personal loss, How is the loss/injury going to effect there personal life. Understand impact of injury and physical limitations associated injury. Will the individual be able to function and conduct everyday activities prior to injury occurring.
Why is this important?
1) What are some steps I can take as a manager if an injury occurs in my workplace to help with employee morale.
