The app fulfills requirements sat in e.g. ISO 9001/14001, OHSAS 18001 and JQS/StartBANK/TransQ/Sellihca (Achilles). The possibilities for improvement if introducing it in your company are huge.
2. About Mellora AS
o Established in 2012
o Deliver smartphone applications and consulting services related to safety and quality - HSEQ
o Our main product, «HSEQ Free», is an app used around the globe for the reporting of
deviations, near misses, accidents, etc., performing and documenting job safe analysis and
various inspections and audits. Customized versions of the app have been made for companies
in countries such as USA, Australia, United Kingdom, The Faroe Islands, Germany and The
Netherlands
o Vision: “HSEQ Free” from Mellora shall be the world’s most widely used mobile application for
submission of safety and quality messages and documentation of relevant HSEQ tasks.
o Owned and managed by Trond Hansen. Mr Hansen has nearly 20 years experience as both a
Quality System Manager, Lead Auditor and Senior HSEQ Advisor. He is an internationally
accredited Quality Manager and Quality Auditor and has got certifications from The European
Organization for Quality (EOQ) within both areas.
3. «HSEQ Free»
Available in 9
languages, but can
be translated to
whatever a company
needs
Developed for iOS,
Android and
Windows Phone
6 pre-defined types
of quick-reports +
Job Safety Analysis,
(JSA/SJA) and
Inspections / Audits
4. Why an App?
o One will most likely receive more HSEQ-reports since “everyone” have, or will get in the near
future, a smartphone, hence they will always carry the “report-book” with them
o Most likely the enthusiasm around HSEQ will increase since the technology used for this
purpose is more up to date
o The possibilities with an app is much greater than using pen and paper, and is simplifies the
reporting process enormously since it is possible to get all reports directly into your own
database or management system
o An app can be extended with even more forms or functions
o One will get a positive environmental and economic effect since one will not use paper at all
when reporting
o The marketing effect will be positive since you can advertise and people will see that you want
to be in the front seat when choosing the best methods for reporting safety and quality
5. Using the app
The idea behind the app is that it should be easy and fast to report
issues related to safety and quality
The user choose from pre-defined texts or writes explaining texts himself
The information entered will be short and sweet - there is rarely a need
for long dissertations
Up to three images may be attached to each report to display the
message even better (NB: may be extended to even more if needed)
Can be used even if you are not online
A PDF and an XML are attached to each report sent from the app
6. Job Safety Analysis / Risk Assessment
The module for job safety analysis / risk assessment is built into the app
as a so-called a local database.
A local database implemented in the app means that you can save the
work as you perform it, you can edit it and get a complete overview of all
work done (“JSA in progress” and “completed JSA”)
Note the "tab-menu" at the bottom; this gives easy access to the quick-
reports (deviations, near misses, accident etc.), the JSA and the
Inspections/Audits, the info-page and the settings-page
7. Job Safety Analysis / Risk Assessment
The top section in JSA/SJA is general information. «Name…» is
automatically collected from the settings page.
The bottom section that is shown in the image is where the user tap to
add new subtasks that is needed to risk assess in order to be able to
perform the main task
When all subtasks have been evaluated (see next page), the user have to
consider if the total risk is acceptable and that the work can be done
Both the person filling out the JSA/SJA and a participant may sign the
report directly in the app to verify the content. The signatures are
included in the generated pdf.
8. Job Safety Analysis / Risk Assessment
After tapping the «Add subtask» menu in the page showed on the
previous slide, the image to the left will appear. In here the user will
descrice the subtask and conduct the risk evaluation. A clickable risk
matrix will be used to make it easy to find the correct risk.
«No» is set as default in the «Action performed». Only when tapping
«Yes» the field «New risk value» will appear
Saving is done after each subtask and new subtasks can be added
directly after saving
Warnings will appear if the user tries to save without having filled out all
necessary fields
All subtaks may be edited through the «main page»
9. Job Safety Analysis / Risk Assessment
The risk matrix is made simple with only nine combinations (three times
three levels)
Each color has explaining text to make it even easier to understand and
consider the risk
A click on the color will add the risk-value to the page shown on the
previous slide
The risk may be edited later if needed
10. Inspections and Audits
Built as a local database just like JSA/SJA
Included in the app are common checklists for various safety inspections,
quality adits and environmental audits.
Existing checklists may be edited in customized versions and even more
checklists may be added
11. Inspections and Audits
The top section in built like JSA/SJA with information about the
inspection/audit that one is about the perform and document
The checkpoints are listed from top till bottom and are easy to scroll
within
The user may select «OK» and «Not applicable» in this image, while one
have to enter the checkpoint to be able to select «Not OK» and enter
additional information
Saving may be done as one perform the inspection/audit
All answers on the checkpoints may be edited later
12. Inspections and Audits
If the user tap on «Not OK» (the red button with the x-mark), a menu
will appear below it where the user define the main cause of the
deviation.
Comments may be added if needed
A picture may be added on each checkpoint
When the inspection/audit is ready and sent, a pdf is generated,
including all text and all images.
In all pictures added into the app’s forms, it is possible to make remarks
(e.g. draw a line, a circle or an arrow to point out something)
