Respiratory failure, defined as a major abnormality of gas exchange, is the sole indication for mechanical ventilation. Hypoxic = characterized by failure of gas exchange Hypercapneic = characterized by failure of the ventilatory pump.
A sixth mechanism, reduced oxygen saturation of mixed venous blood due to excessive systemic oxygen consumption and /or reduced cardiac output can worsen the severity of arterial hypoxemia.
