A recent study by Haynesworth et al represented the first published data showing a direct effect of PRP releasate on purified human MSCs, which play a pivotal role in the process of musculoskeletal tissue repair. This study confirmed that PRP has a dose dependent effect on Chemotaxis – the ability to attract cells, by either biologic signals or physiological cues, into the wound site or graft to initiate repair or regeneration. Proliferation or mitogenisis– the increase in cell number due to cell division. The study also concluded that although PRP could not cause differentiation, the ability to influence conversion of a cell to another specialized cell, it did enhance the function of cells that had already undergone differentiation. Let’s examine each of these effects in more detail. Platelets may enhance the bioactivity of graft materials because they serve two very important functions - Structure and Signaling. When the platelet concentrate meets the graft material it forms a clot, due to its role in creating a three dimensional fibrin mesh. A concentration of growth factors improves signaling and recruitment of cells to the graft site. More platelets will mean more growth factors to signal for osteoprogenitor cells.
