The document presents three case studies demonstrating the effectiveness of the SpineCor dynamic corrective brace in treating adolescent idiopathic scoliosis. Case study 1 shows improvements in a patient's spinal alignment and reduction of the vertebral wedge after 18 months of brace treatment. Case study 2 presents x-ray images tracking a patient's curve correction from initial fitting to 5-year follow up, demonstrating reduced Cobb angles and improved coronal and postural alignment. Case study 3 similarly documents reductions in the patient's thoracic and thoracolumbar Cobb angles as well as maintained correction at 5-year follow up, along with improvements in coronal balance and posture.
This slide clearly shows how SpineCor treatment can actually reduce the vertebral wedge shown by the intercostals spaces by opening the curve dynamically and stimulating localised vertebral growth. We have fundamentally changed something in this child. This kind of correction is only possible starting treatment before Risser 1, before first menses (before hormonal maturation and high velocity period of growth) and with flexible curves. Early treatment more likely give better results.
This is the same girl, showing clinically a dramatic improvement in her posture.
Case study 2-A female patient with a Left Lumbar Curve who is 13 years and 3 months old and has a Risser 0.-Her Initial x-rays without the brace show that she has a thoracic kyphosis and lumbar lordosis within normal limits, and a scoliosis curve of 30 degrees Cobb angle.
-The patient was fitted with the brace and was supplied with a 1cm shoe lift for her left shoe. Her in-brace x-ray showed a reduction in Cobb angle to 22 degrees.
She wore the brace for 1 year and 10 months, by which time she was 15 years and 2 months old and a Risser 3. Her curve was still at 22 degrees at this time. She continued to wear the 1 cm shoe lift.
2 years follow up showed that the curve had not changed and was stable at 22 degrees. By this time the patient had also stopped using the shoe lift.
At 5 years follow up the curve was 20 degrees.
A comparison of the first and last x-rays demonstrates a moderate curve reduction.
A comparison of the first and last x-rays also shows an improvement in the Global, regional and apex coronal balance. At the beginning of treatment the Global Coronal balance, shown by T1, is significantly to the left of the Central Sacral Line, or CSL. The regional Coronal Balance is also left of the CSL, shown by T12. The APEX is significantly left of the CSL.At 5 years follow up the Global balance is slightly left of the CSL, the regional balance is in line with the CSL, and the Apex is closer to the CSL than at the beginning of treatment.
This patient also had a significant improvement in her posture. It is clear that at 5 years she is much more symmetrical in her waist creases than in the photo taken at the beginning of treatment.
Case study 3-Another female patient with a Left ThoracoLumbar Type 2 Curve who is 10 years and 9 months old and has a Risser 0.-Her Initial x-rays without the brace show that she has a thoracic Hypokyphosis and a slight lumbar Hyperlordosis, and two scoliosis curves with Cobb angles of 38 and 24 degrees.
-The patient was fitted with the brace and her in-brace x-ray showed a 20 degree reduction in the thoracic curve to 4 degrees and a 26 degree reduction in the thoracolumbar curve to 12 degrees.
She wore the brace for 2 years, by which time she was 14 years and 2 months old and a Risser 3. Her curves were 15 and 11 degrees at this time, and her thoracic kyphosis and lumbar lordosis were within normal limits.
At 5 years follow up her curves had reduced further to 8 and 5 degrees. So with this patient there was curve correction after brace weaning.
A comparison of the first and last x-rays demonstrates a significant curve reduction.
A comparison of the first and last x-rays also show an improvement in the Regional and apex coronal balance which in this type of curve is the same thing because the apex is at located at T12.
This patient also had a significant improvement in her posture. It is clear that at 5 years she is much more symmetrical in her waist creases than in the photo taken at the beginning of treatment.
