It leverages familiar technology - cell phones, smartphones and other mobile tablet PCs - with incentives for consumers and providers to manage preventive, chronic and post-acute care.The mPHR shows great promise as an optimal platform for engaging consumers in self-care. Combined with actionable decision support, the mPHR can analyze aggregate data to activate mobile, patient-specific output such as medication reminders, healthy habit tips and medical bill reminders.
