Psychotherapy for Addictive Disorders “A long-standing debate has roiled over whether addicts have a choice over their behaviors. The disease creates distortions in thinking, feelings, and perceptions, which drive people to behave in ways that are not understandable to others around them. Simply put, addiction is not a choice. Addictive behaviors are a manifestation of the disease, not a cause." –Dr. Raju Hajela, former president of the Canadian Society of Addiction Medicine A common misconception is that addiction is a choice, and addicts are often labeled as individuals who lack morals, willpower, or responsibility. However, addiction is a clinical disorder that must be treated with the support of a health care professional. Although many people who are exposed to potentially addictive substances and behaviors continue life unaltered by their experiences, some people are fueled by these experiences and spiral out of control. In your role as the psychiatric mental health nurse practitioner, you must be prepared to not only work with these individuals who struggle with addiction, but also help them and their families overcome the social stigmas associated with addictive behavior. This week, as you explore psychotherapy for addiction, you assess clients presenting with addictive disorders. You also examine therapies for treating these clients and consider potential outcomes. Finally, you develop diagnoses for clients receiving psychotherapy and consider legal and ethical implications of counseling these clients. Most of us have experienced a time or situation when we were dramatically helped by talking things over with a relative or friend. Most therapists, like all good listeners, rely on receptiveness, warmth, and empathy and take a nonjudgmental approach to the problems their clients present. But there is more to therapy than just giving someone an opportunity to talk. Therapists also introduce into the relationship psychological interventions that are designed to promote new understandings, behaviors, or both on the client’s part. The fact that these interventions are deliberately planned and systematically guided by certain theoretical preconceptions is what distinguishes professional therapy from more informal helping relationships. An Overview of Treatment The belief that people with psychological problems can change—can learn more adaptive ways of perceiving, evaluating, and behaving—is the conviction underlying all psychotherapy. Achieving these changes is by no means easy. Sometimes a person’s view of the world and her or his self-concept are distorted because of pathological early relationships that have been reinforced by years of negative life experiences. In other instances, environmental factors such as an unsatisfying job, an unhappy relationship, or financial stresses must be the focus of attention in addition to psychotherapy. Because change can be hard, people sometimes find it easier to bear their present problems than to challenge themselves to chart a different life course. Therapy also takes time. Even a highly skilled and experienced therapist cannot undo a person’s entire past history and, within a short time, prepare him or her to cope adequately with difficult life situations. Therapy offers no magical transformations. Nevertheless, it holds promise even for the most severe mental disorders. Moreover, contrary to common opinion, psychotherapy can be less expensive in the long run than alternative modes of intervention (Dobson et al., 2008; Gabbard et al., 1997). Numerous therapeutic approaches exist, ranging from psychoanalysis to Zen meditation. However, the era of managed care has prompted new and increasingly stringent demands that the efficacy of treatments be empirically demonstrated. This chapter will explore some of the most widely accepted psychological and biological treatment approaches in use today. One of the theorists that speaks to my nursing practice is Madeleine Leininger’s Theory of Culture Care Diversity and Universality otherwise known as transcultural nursing. The theory of transcultural nursing promotes understanding of both the universally held and common understandings of care among humans and the culture-specific caring beliefs and behaviors that define any particular caring context of interaction (Butts & Rich, 2018, p. 572). Culture is the specific pattern of behavior that distinguishes any society from others and gives meaning to human expressions of care (McFarland & Wehbe-Alamah, 2015). This theory is intended to view the patient as a whole being. As an APN, this theory will allow me to be considerate of my patient’s needs. In turn, they will feel respected and listened to and hopefully this will promote a long-lasting relationship. Leininger discovered that inadequate knowledge of cultural factors represented a missing link in her ability to provide care (Nelson, 2006). As a future APN, I recognize the need and sometimes missing component of culture care nursing in our women’s health practice today. For example, in South Dakota we have a lot of Native American reservations. Since we see a number of Native American women, I think it would be beneficial for me to better understand their culture and beliefs related to healthcare in order to offer these women expert care. Also, with Leininger’s transcultural nursing theory she believes seeing the patient as a whole and to appreciate that the patient represents a complex constellation of physical, emotional, psychological, spiritual, social, environmental, and economic factors that interact and contribute to the patient’s unique wholeness and influence their ability to participate in health promotion, health maintenance, and disease modifying activities (McFarland & Eipperle, 2008).
