- 2. About Me Clinical Nurse Specialist for Older Person since January 2009. My post is based in St Mary’s Hospital and serves the older adult population in Dublin North City An active member of AIGNA & Diabetes Ireland Member of the ISHCA project team Education completed to support my post includes:- Diploma in First-line Management - Diploma in Care of older Adults - Certificate in Diabetes Nursing Management - Certificate in Continence promotion & Incontinence Management

My Role in St.Mary’s Carry out a comprehensive nursing assessment of older adults attending St Mary’s Day Hospital Carry out a specialist nursing assessment on patients with Diabetes Carry out a specialist nursing assessment on patients with incontinence problems Co-ordinator of services between community service providers, GPs and St Mary’s Provide education and support to staff, patients and their families

Specialist Nursing Assessment Diabetic Patient Comprehensive assessment on patients who have a diagnosis of Diabetes Mellitus which includes:• Foot screening • Blood glucose monitoring • Management of Diabetes Number of new patients with Diabetes assessed Jan – Oct 2013 - 88 Number of patients reviewed Jan – Oct 2013 - 70
- 5. Diabetes Management Link Staff - Quarterly meeting with diabetes link nurses from PPCNU and St.Mary’s. Provision of education and empowerment to all nurses at ward level Introduction of new measures and techniques in monitoring diabetes

Specialist Nursing Assessment Comprehensive continence assessment for older adults with incontinence problems Number of new patients with incontinence problems assessed Jan – Oct 2013 - 29 Number of patients reviewed Jan - Oct 2013 – 18 Outcome of interventions Problems resolved - 5 Significantly improved quality of life - 13 Better incontinence Management - 7

Continence Management Provide education and support to staff and to patient post assessment: Prompted Voiding Fluid Intake Changes Caffeine Reduction Bedside Commode Personal Hygiene Incontinence Product Bladder Training Pelvic Floor Exercises (Physiotherapy) Urge suppression Medical Team input
- 8. Other key responsibilities…. Audit and Research - monitoring care and feed back to staff regarding any necessary changes in related specialist areas Providing and assisting in delivery of in service education such as palliative care, dementia care, pain management and syringe drivers & pumps. Recognised need for further development Pain Management in patients with Osteo Arthritis

- AIGNA is a good source to enhances the gerontological nurses’ knowledge, skills and expertise through networking, information sharing and other knowledge translation activities
- 1)includes MMSE, GDS, ECG, Bloods etc in order to evaluate an older person’s functional status, physical health, cognition, mental health and social environmental circumstances 2) in order to enhance and maintain health and wellbeing of elderly people and maintain them in their own home and to enable them to live with dignity and independence. 3)In relation to diabetes management and continence promotion
- 3 )includes foot screening-to role out early complications, education and support on diet and glucometer, management of hypoglycaemia and hyperglycaemia, diabetes complications and insulin management, foot care, blood test for hbaic and ketone test.
- to increase nurses knowledge and skills in identifying early complications of diabetes and to all patients and residents with diabetes and their families such as Ketone testing, HbA1c 3 monthly, annual eye test, individual Glucometer, retractable insulin needles and policy on safe use of Insulin and insulin pens
- Currently auditing the practice in pain management. I would like to look at how they managing the pain, way of monitoring, evaluating, reviewing, and outcome. Pain free is one of the important factor to increase the quality of life of older adults .
