SEIZURES
• The term seizure refers to a transient
alteration of behavior due to the disordered,
synchronous, and rhythmic firing of
populations of brain neurons.
EPILEPSY
• The term epilepsy refers to a disorder of brain
function characterized by the periodic and
unpredictable occurrence of seizures.

TREATMENT OF EPILEPSY
• The choice of medication depends either on
the type of seizures that the patient exhibits
or on the patient’s syndromic classification.
• For patients with hard-to-control seizures,
multiple drugs are usually used
simultaneously
Molecular targets of antiseizure drugs
GABA ANALOGUE
GABAPENTIN & PREGABALIN
• Gabapentin and pregabalin, known as
“gabapentinoids,” are amino acid-like
molecules that were originally synthesized as
analogs of GABA.
• MOA-> gabapentinoids bind avidly to α2δ, a
protein that serves as an auxiliary subunit of
voltage-gated calcium channels and enhance
the GABA release .
• Clinical Uses -> Gabapentin is considered to
be a first line drug for neuralgic pain due to
diabetic neuropathy and postherpetic
neuralgia.
• Gabapentin and pregabalin are effective in the
treatment of focal seizures.
• Side effect->Somnolence, dizziness, ataxia
Pharmacokinetics
• Gabapentin and pregabalin are eliminated
unchanged in the urine.
• Both drugs are absorbed by the l-amino acid
transport system, which is found only in the
upper small intestine.
• The half-life of both drugs is ranging from 5 to
8 hours for gabapentin and 4.5 to 7.0 hours
for pregabalin.
TIAGABINE
• Tiagabine, a selective inhibitor of the GAT-1
GABA transporter, is a second-line treatment
for focal seizures.
Clinical Uses
• Tiagabine is indicated for the adjunctive
treatment of focal seizures, with or without
secondary generalization.
• In adults, the recommended initial dose is 4
mg/d.
Adverse effects;
• Nervousness.
• Dizziness.
• Depression
Pharmacokinetics
• Tiagabine is 90–100% bioavailable, has linear
kinetics, and is highly protein bound.
• The half-life is 5–8 hours and decreases in the
presence of enzyme-inducing drugs.
Vigabatrin
• Vigabatrin is an irreversible inhibitor of GABA
transaminase, thus increases GABAergic
activity. It is the DOC for infantile spasms
associated with tuberous sclerosis (ACTH is
DOC for all other cases).
Pharmacokinetics
• Complete absorption.
• Peak concentrations in 1 h .
• Not bound to plasma proteins.
• Eliminated unchanged in urine
• t1/2 5–8 h .

• Block N-methyl-d-aspartate (NMDA) receptor.
• Oral felbamate is well absorbed (> 90%).
• It is used in the treatment of focal seizures
and in the Lennox-Gastaut syndrome.
• Of the absorbed dose, 30–50% is excreted
unchanged in the urine. The remainder is
metabolized by CYP3A4 and CYP2E1 in the
liver.
Lennox Gastaut Syndrome
• It is a difficult-to-treat form of childhood-onset
epilepsy that most often appears between 2 to 6 years
of age. It is characterized by frequent occurrence of
different seizure types.
• EEG shows characteristic slow spike-wave complexes.
-> First-line drugs for treatment are
• rufinamide, valproate and benzodiazepines
(clonazepam and clobazam).
-> Second-line
• drugs are felbamate and topiramate
PERAMPANEL
• Perampanel is a potent non competitive
antagonist of the AMPA receptor.
• Clinical Use-> Focal and focal-to-bilateral
• Tonic-clonic seizures
• Generalized tonic-clonic seizures
Pharmacokinetics
• Perampanel has a long half-life, typically
ranging from 70 to 110 hours, which permits
once-daily dosing.
• Absorption is rapid and the drug is fully
bioavailable.
Adverse reaction
Dizziness, somnolence, headache
Calcium Channels Blockers
ETHOSUXIMIDE
• Ethosuximide is a first-line drug for the
treatment of generalized absence seizures.
• MOA->Ethosuximide acts by inhibition of low-
voltage activated T-type calcium channels .
Clinical Uses
• It is effective in the treatment of atypical
absence and epileptic negative myoclonus.
• If ethosuximide in monotherapy does not lead
to seizure control, the drug can be used in
combination with valproate or other agents
such as benzodiazepines,
Pharmacokinetics
• During long term administration,
approximately 20% of the dose is excreted
unchanged by the kidney. The remaining drug
is metabolized in the liver, principally by
CYP3A4 hydroxylation, to inactive metabolites.
• Ethosuximide has a very low total body
clearance (0.25 L/kg/d).
Drug Interactions & Toxicity
• The most common dose-related adverse effect
of ethosuximide is gastric distress, including
pain, nausea, and vomiting.
• Other dose-related adverse effects are
transient lethargy or fatigue and, much less
commonly, headache, dizziness and euphoria.
• Interactions-> Valproate, phenobarbital,
phenytoin, carbamazepine.
SODIUM CHANNELS BLOCKERS
PHENYTOIN
• Phenytoin is the oldest nonsedating drug
used in the treatment of epilepsy.
• It is prescribed for the prevention of focal
seizures and generalized tonic-clonic seizures
and for the acute treatment of status
epilepticus.
• MOA-> sodium channel-blocker.
Pharmacokinetics
• Absorption of phenytoin is highly dependent
on the formulation. Particle size and
pharmaceutical additives affect both the rate
and the extent of absorption.
• Phenytoin is metabolized by CYP2C9 and
CYP2C19 to inactive metabolites that are
excreted in the urine.

Interactions with
• Phenobarbital.
• Carbamazepine.
• Felbamate.
• Oxcarbazepine.
• Topiramate.
• Steroids.
• oral Contraceptives.
• Warfarin.
SIDE EFFECTS
–vertigo, ataxia, headache and nystagmus
–Hyperplasia of the gums
–Hirsutism (in females)
–Osteomalacia
–Vit K deficiency
–Megaloblastic anaemia
–Hypersensitivity reactions
–Teratogenic
KEY POINTS
• Different brand are not bioequivalent.
• It follows zero order kinetics.
• It is enzyme inducer.
Carbamazepine
• It is a prototype of the antiseizure drugs
primarily used in the treatment of focal onset
seizures.
• In addition to being effective in the treatment
of tonic-clonic (grand mal) seizures.
.
Mechanism of Action
• Carbamazepine is a prototypical sodium
channel-blocker.
Clinical Uses
• Carbamazepine is effective for the treatment
of focal and bilateral tonic-clonic seizures.
• Carbamazepine is also effective for the
treatment of trigeminal neuralgia, and mania
in bipolar disorder.
Pharmacokinetics
• Carbamazepine has nearly 100% oral
bioavailability.
• Drug after meals causes a reduction in peak
levels.
• Carbamazepine is metabolized in the liver, and
only about 5% of the drug is excreted
unchanged.
Adverse Effects
• Carbamazepine may cause dose-dependent
mild gastrointestinal discomfort, dizziness,
blurred vision, diplopia, or ataxia; sedation
occurs only at high doses, and rarely, weight
gain can occur, Stevens-Johnson syndrome
• The diplopia often occurs first and may last
less than an hour during a particular time of
day.

VALPROATE SODIUM
• Valproate is a first-line broad-spectrum
antiseizure drug that is thought to offer
protection against many seizure types.
• In addition, it is used as a mood stabilizer in
bipolar disorder and as prophylactic treatment
for migraine.
Mechanism of Action
• Acts by several mechanisms;
– Blockade of Na channels
– Increased activity of GABA
– Decreased release of glutamate in brain
– Inhibition of T type Ca channels
Clinical Uses
Valproate is drug of choice for
• GTCS
• Myoclonic Seizures
• Atonic Seizures
• Absence Seizures
• Clonic Seizures
• Tonic Seizures
Pharmacokinetics
• Valproate is well absorbed after an oral dose,
with bioavailability greater than 80%.
• Food may delay absorption.
• The halflife varies from 9 to 18 hours.
• Drug Interactions-> Valproate inhibits the
metabolism of several drugs, including
phenobarbita ,ethosuximide etc.
• Toxicity->The most common dose-related
adverse effects of valproate are nausea, vomiting,
and other gastrointestinal complaints such as
abdominal pain and heartburn.
• Treatment with valproate during the first
trimester of pregnancy is associated with a 1–2%
risk of neural tube defects including spina bifida.
Adverse Effects
– Weight gain
– Alopecia
– Tremors
– Hepatic necrosis ( children below 2yrs)
– Neural tube defects in baby (C/I in pregnancy)
Topiramate
Topiramate is FDA-approved as initial
monotherapy therapy for-
• Partial-onset or primary generalized tonic
clonic seizures,
• Lennox-Gastaut syndrome in patients 2 years of
age and older,
Mechanisms of Action
–Block Na and Ca channels
–Enhances action of GABA
–Blocks AMPA receptors
Pharmacokinetics.
• Topiramate is rapidly absorbed after oral
administration, exhibits little (10-20%) binding
to plasma proteins, and is mainly excreted
unchanged in the urine.
• The remainder undergoes metabolism by
hydroxylation, hydrolysis, and glucuronidation.
Adverse effects;
• fatigue, weight loss, and nervousness
• Renal stones
KEY POINTS
• It decrease the craving for the alcohol.
• Only antiseizure drug causing weight loss.
• Used in migrain prophylaxis.
• Also used in bipolar disorder.
LAMOTRIGINE
• Lamotrigine is considered a sodium channel-
blocking antiseizure drug; it is effective for the
treatment of focal seizures.
• MOA-> It block the voltage gated sodium
channels.
• Clinical Uses-> Lamotrigine is approved for
primary generalized tonic clonic seizures and
generalized seizures of the Lennox-Gastaut
syndrome.
• Pharmacokinetics-> Lamotrigine is almost
completely absorbed and has a volume of
distribution of 1–1.4 L/kg. Protein binding is
only about 55%.
Adverse effects
– Nausea, vomiting
– dizziness
– Ataxia
– hypersensitivity reactions
– Sloughing of the skin
BENZODIAZEPINES
• A large number of benzodiazepines have
broad anti-seizure properties, but only
clonazepam (KLONOPIN, others) and
clorazepate (TRANXENE, others) have been
approved in the U.S. for the long-term
treatment of certain types of seizures.
Mechanism of Action.
• Benzodiazepines act at subsets of GABAA
receptors and increase the frequency, but not
duration, of openings at GABA-activated Cl–
channels.
Drugs used in it
• Diazepam given intravenously for febrile seizures.
• Lorazepam is more commonly used in the treatment of
status epilepticus because it has a more prolonged
duration of action after bolus intravenous injection.
• Clonazepam is a long acting benzodiazepine that on a
milligram basis is one of the most potent antiseizure
agents known. It has documented efficacy in the
treatment of absence, atonic, and myoclonic seizures.
Pharmacokinetic Properties.
• Benzodiazepines are well absorbed after oral
administration, and concentrations in plasma
are usually maximal within 1-4 hours.
Adverse effects
->Drowsiness, lethargy, fatigue, disorientation and
impairment of psychomotor skills

PHENOBARBITAL
• Phenobarbital was introduced into the clinical
market in 1912 as a sleeping aid; it was
serendipitously found to be useful in the
treatment of epilepsy.
• Phenobarbital is preferred in the chronic
treatment of epilepsy because it is less
sedative at antiseizure doses.
Mechanism of Action
• Barbiturates increase the mean open duration
of the cl channel without altering either
channel conductance or opening frequency.
Clinical Uses
Phenobarbital is useful in the treatment of;
• ->Focal seizures.
• ->Generalized tonic-clonic seizures.
• ->Myoclonic seizures
Adverse Effect
• Sedation
• Nystagmus and ataxia
• irritability and hyperactivity in children
Drugs interaction
• Valproate, carbamazepine, felbamate,
phenytoin, cyclosporine, felodipine,
lamotrigine, nifedipine, nimodipine, steroids,
theophylline, verapamil, others
Primidone
• PK-> Nearly complete (>90%) absorption
• Minimal (10%) plasma protein binding
• Peak concentrations in 2–6 h
• Extensively metabolized in liver.
• Clinical Uses
• Generalized tonic-clonic seizures
• Partial seizures
Adverse effect
• Sedation, cognitive issues, ataxia.
Drugs interaction
• Valproate, carbamazepine, felbamate,
phenytoin, cyclosporine, felodipine,
lamotrigine, nifedipine, nimodipine, steroids,
theophylline, verapamil, others
RETIGABINE
• Retigabine a potassium channel opener, is a
third-line treatment for focal seizures.
• Because retigabine causes pigment
discoloration of the retina and skin, its use is
limited to those who have failed to respond to
other agents.
MOA-> Retigabine is an allosteric opener of
voltage-gated potassium channels, which are
localized, in part, in axons and nerve
terminals.
• Pharmacokinetics-> Absorption of retigabine
is not affected by food.
• The major metabolic pathways in humans are
N-glucuronidation and N-acetylation.
Adverse effect
• Somnolence.
• Vomiting.
• pyrexia.
• Diarrhea.
LEVETIRACETAM & BRIVARACETAM
• Levetiracetam is a broad-spectrum antiseizure
agent and one of the most commonly
prescribed drugs for epilepsy because of:
->Broad therapeutic window,
->Favorable pharmacokinetic properties, and
->Lack of drug-drug interactions.
Mechanism of Action
• Levetiracetam binds selectively to SV2A, a
ubiquitous synaptic vesicle integral membrane
protein,
• which may function as a positive effector of
synaptic vesicle exocytosis
• Binding to SV2A in the vesicle reduces the release
of the excitatory neurotransmitter glutamate
during trains of high-frequency activity.
Adverse effect
• Nervousness.
• Dizziness.
• Depression
Clinical use
• Focal seizures, generalized.
• Tonic-clonic seizures.
• Myoclonic seizures.

  • 2. SEIZURES • The term seizure refers to a transient alteration of behavior due to the disordered, synchronous, and rhythmic firing of populations of brain neurons.
  • 3. EPILEPSY • The term epilepsy refers to a disorder of brain function characterized by the periodic and unpredictable occurrence of seizures.

  • 15. TREATMENT OF EPILEPSY • The choice of medication depends either on the type of seizures that the patient exhibits or on the patient’s syndromic classification. • For patients with hard-to-control seizures, multiple drugs are usually used simultaneously
  • 16. Molecular targets of antiseizure drugs
  • 18. GABAPENTIN & PREGABALIN • Gabapentin and pregabalin, known as “gabapentinoids,” are amino acid-like molecules that were originally synthesized as analogs of GABA. • MOA-> gabapentinoids bind avidly to α2δ, a protein that serves as an auxiliary subunit of voltage-gated calcium channels and enhance the GABA release .
  • 19. • Clinical Uses -> Gabapentin is considered to be a first line drug for neuralgic pain due to diabetic neuropathy and postherpetic neuralgia. • Gabapentin and pregabalin are effective in the treatment of focal seizures. • Side effect->Somnolence, dizziness, ataxia

Pharmacokinetics • Gabapentin and pregabalin are eliminated unchanged in the urine. • Both drugs are absorbed by the l-amino acid transport system, which is found only in the upper small intestine. • The half-life of both drugs is ranging from 5 to 8 hours for gabapentin and 4.5 to 7.0 hours for pregabalin.
  • 21. TIAGABINE • Tiagabine, a selective inhibitor of the GAT-1 GABA transporter, is a second-line treatment for focal seizures.
  • 22. Clinical Uses • Tiagabine is indicated for the adjunctive treatment of focal seizures, with or without secondary generalization. • In adults, the recommended initial dose is 4 mg/d. Adverse effects; • Nervousness. • Dizziness. • Depression
  • 23. Pharmacokinetics • Tiagabine is 90–100% bioavailable, has linear kinetics, and is highly protein bound. • The half-life is 5–8 hours and decreases in the presence of enzyme-inducing drugs.
  • 24. Vigabatrin • Vigabatrin is an irreversible inhibitor of GABA transaminase, thus increases GABAergic activity. It is the DOC for infantile spasms associated with tuberous sclerosis (ACTH is DOC for all other cases).
  • 25. Pharmacokinetics • Complete absorption. • Peak concentrations in 1 h . • Not bound to plasma proteins. • Eliminated unchanged in urine • t1/2 5–8 h .

FELBAMATE • Block N-methyl-d-aspartate (NMDA) receptor. • Oral felbamate is well absorbed (> 90%). • It is used in the treatment of focal seizures and in the Lennox-Gastaut syndrome. • Of the absorbed dose, 30–50% is excreted unchanged in the urine. The remainder is metabolized by CYP3A4 and CYP2E1 in the liver.
  • 28. Lennox Gastaut Syndrome • It is a difficult-to-treat form of childhood-onset epilepsy that most often appears between 2 to 6 years of age. It is characterized by frequent occurrence of different seizure types. • EEG shows characteristic slow spike-wave complexes. -> First-line drugs for treatment are • rufinamide, valproate and benzodiazepines (clonazepam and clobazam). -> Second-line • drugs are felbamate and topiramate
  • 29. PERAMPANEL • Perampanel is a potent non competitive antagonist of the AMPA receptor. • Clinical Use-> Focal and focal-to-bilateral • Tonic-clonic seizures • Generalized tonic-clonic seizures
  • 30. Pharmacokinetics • Perampanel has a long half-life, typically ranging from 70 to 110 hours, which permits once-daily dosing. • Absorption is rapid and the drug is fully bioavailable. Adverse reaction Dizziness, somnolence, headache
  • 32. ETHOSUXIMIDE • Ethosuximide is a first-line drug for the treatment of generalized absence seizures. • MOA->Ethosuximide acts by inhibition of low- voltage activated T-type calcium channels .
  • 33. Clinical Uses • It is effective in the treatment of atypical absence and epileptic negative myoclonus. • If ethosuximide in monotherapy does not lead to seizure control, the drug can be used in combination with valproate or other agents such as benzodiazepines,
  • 34. Pharmacokinetics • During long term administration, approximately 20% of the dose is excreted unchanged by the kidney. The remaining drug is metabolized in the liver, principally by CYP3A4 hydroxylation, to inactive metabolites. • Ethosuximide has a very low total body clearance (0.25 L/kg/d).
  • 35. Drug Interactions & Toxicity • The most common dose-related adverse effect of ethosuximide is gastric distress, including pain, nausea, and vomiting. • Other dose-related adverse effects are transient lethargy or fatigue and, much less commonly, headache, dizziness and euphoria. • Interactions-> Valproate, phenobarbital, phenytoin, carbamazepine.
  • 37. PHENYTOIN • Phenytoin is the oldest nonsedating drug used in the treatment of epilepsy. • It is prescribed for the prevention of focal seizures and generalized tonic-clonic seizures and for the acute treatment of status epilepticus. • MOA-> sodium channel-blocker.
  • 38. Pharmacokinetics • Absorption of phenytoin is highly dependent on the formulation. Particle size and pharmaceutical additives affect both the rate and the extent of absorption. • Phenytoin is metabolized by CYP2C9 and CYP2C19 to inactive metabolites that are excreted in the urine.
  • 39. Drug Interactions Interactions with • Phenobarbital. • Carbamazepine. • Felbamate. • Oxcarbazepine. • Topiramate. • Steroids. • oral Contraceptives. • Warfarin.

  • 42. Carbamazepine • It is a prototype of the antiseizure drugs primarily used in the treatment of focal onset seizures. • In addition to being effective in the treatment of tonic-clonic (grand mal) seizures. .
  • 43. Mechanism of Action • Carbamazepine is a prototypical sodium channel-blocker.
  • 44. Clinical Uses • Carbamazepine is effective for the treatment of focal and bilateral tonic-clonic seizures. • Carbamazepine is also effective for the treatment of trigeminal neuralgia, and mania in bipolar disorder.
  • 45. Pharmacokinetics • Carbamazepine has nearly 100% oral bioavailability. • Drug after meals causes a reduction in peak levels. • Carbamazepine is metabolized in the liver, and only about 5% of the drug is excreted unchanged.

Adverse Effects • Carbamazepine may cause dose-dependent mild gastrointestinal discomfort, dizziness, blurred vision, diplopia, or ataxia; sedation occurs only at high doses, and rarely, weight gain can occur, Stevens-Johnson syndrome • The diplopia often occurs first and may last less than an hour during a particular time of day.

  • 48. VALPROATE SODIUM • Valproate is a first-line broad-spectrum antiseizure drug that is thought to offer protection against many seizure types. • In addition, it is used as a mood stabilizer in bipolar disorder and as prophylactic treatment for migraine.
  • 49. Mechanism of Action • Acts by several mechanisms; – Blockade of Na channels – Increased activity of GABA – Decreased release of glutamate in brain – Inhibition of T type Ca channels
  • 50. Clinical Uses Valproate is drug of choice for • GTCS • Myoclonic Seizures • Atonic Seizures • Absence Seizures • Clonic Seizures • Tonic Seizures
  • 51. Pharmacokinetics • Valproate is well absorbed after an oral dose, with bioavailability greater than 80%. • Food may delay absorption. • The halflife varies from 9 to 18 hours.
  • 52. • Drug Interactions-> Valproate inhibits the metabolism of several drugs, including phenobarbita ,ethosuximide etc. • Toxicity->The most common dose-related adverse effects of valproate are nausea, vomiting, and other gastrointestinal complaints such as abdominal pain and heartburn. • Treatment with valproate during the first trimester of pregnancy is associated with a 1–2% risk of neural tube defects including spina bifida.

Pharmacokinetics. • Topiramate is rapidly absorbed after oral administration, exhibits little (10-20%) binding to plasma proteins, and is mainly excreted unchanged in the urine. • The remainder undergoes metabolism by hydroxylation, hydrolysis, and glucuronidation. Adverse effects; • fatigue, weight loss, and nervousness • Renal stones
  • 57. KEY POINTS • It decrease the craving for the alcohol. • Only antiseizure drug causing weight loss. • Used in migrain prophylaxis. • Also used in bipolar disorder.
  • 58. LAMOTRIGINE • Lamotrigine is considered a sodium channel- blocking antiseizure drug; it is effective for the treatment of focal seizures. • MOA-> It block the voltage gated sodium channels.
  • 59. • Clinical Uses-> Lamotrigine is approved for primary generalized tonic clonic seizures and generalized seizures of the Lennox-Gastaut syndrome. • Pharmacokinetics-> Lamotrigine is almost completely absorbed and has a volume of distribution of 1–1.4 L/kg. Protein binding is only about 55%.

  • 61. BENZODIAZEPINES • A large number of benzodiazepines have broad anti-seizure properties, but only clonazepam (KLONOPIN, others) and clorazepate (TRANXENE, others) have been approved in the U.S. for the long-term treatment of certain types of seizures.
  • 62. Mechanism of Action. • Benzodiazepines act at subsets of GABAA receptors and increase the frequency, but not duration, of openings at GABA-activated Cl– channels.
  • 63. Drugs used in it • Diazepam given intravenously for febrile seizures. • Lorazepam is more commonly used in the treatment of status epilepticus because it has a more prolonged duration of action after bolus intravenous injection. • Clonazepam is a long acting benzodiazepine that on a milligram basis is one of the most potent antiseizure agents known. It has documented efficacy in the treatment of absence, atonic, and myoclonic seizures.
  • 64. Pharmacokinetic Properties. • Benzodiazepines are well absorbed after oral administration, and concentrations in plasma are usually maximal within 1-4 hours. Adverse effects ->Drowsiness, lethargy, fatigue, disorientation and impairment of psychomotor skills
  • 66. PHENOBARBITAL • Phenobarbital was introduced into the clinical market in 1912 as a sleeping aid; it was serendipitously found to be useful in the treatment of epilepsy. • Phenobarbital is preferred in the chronic treatment of epilepsy because it is less sedative at antiseizure doses.
  • 67. Mechanism of Action • Barbiturates increase the mean open duration of the cl channel without altering either channel conductance or opening frequency.
  • 68. Clinical Uses Phenobarbital is useful in the treatment of; • ->Focal seizures. • ->Generalized tonic-clonic seizures. • ->Myoclonic seizures Adverse Effect • Sedation • Nystagmus and ataxia • irritability and hyperactivity in children

  • 70. Primidone • PK-> Nearly complete (>90%) absorption • Minimal (10%) plasma protein binding • Peak concentrations in 2–6 h • Extensively metabolized in liver. • Clinical Uses • Generalized tonic-clonic seizures • Partial seizures
  • 71. Adverse effect • Sedation, cognitive issues, ataxia. Drugs interaction • Valproate, carbamazepine, felbamate, phenytoin, cyclosporine, felodipine, lamotrigine, nifedipine, nimodipine, steroids, theophylline, verapamil, others
  • 72. RETIGABINE • Retigabine a potassium channel opener, is a third-line treatment for focal seizures. • Because retigabine causes pigment discoloration of the retina and skin, its use is limited to those who have failed to respond to other agents.
  • 73. MOA-> Retigabine is an allosteric opener of voltage-gated potassium channels, which are localized, in part, in axons and nerve terminals. • Pharmacokinetics-> Absorption of retigabine is not affected by food. • The major metabolic pathways in humans are N-glucuronidation and N-acetylation.

  • 75. LEVETIRACETAM & BRIVARACETAM • Levetiracetam is a broad-spectrum antiseizure agent and one of the most commonly prescribed drugs for epilepsy because of: ->Broad therapeutic window, ->Favorable pharmacokinetic properties, and ->Lack of drug-drug interactions.

Mechanism of Action • Levetiracetam binds selectively to SV2A, a ubiquitous synaptic vesicle integral membrane protein, • which may function as a positive effector of synaptic vesicle exocytosis • Binding to SV2A in the vesicle reduces the release of the excitatory neurotransmitter glutamate during trains of high-frequency activity.
