Este documento discute el papel de la autoreactividad de los anticuerpos IgE en la dermatitis atópica. Explica que la autoreactividad puede ser un factor patogénico que contribuye a la inflamación crónica y la severidad de la enfermedad. También sugiere que la comprensión de este mecanismo podría conducir a nuevas estrategias terapéuticas para limitar la progresión de la dermatitis atópica y reducir sus altos costos y el impacto en la calidad de vida de los pacientes.

The Lower prevalence of atopic dermatitis in rural as compared with urban areas suggests a link to the “hygiene hypothesis,” which postulates that the absence of early childhood exposure to infectious agents increases susceptibility to allergic diseases.
Hace aproximadamente dos décadas atrás, Wüthrich refería que la dermatitis atópica (DA) podía clasificarse en dos formas básicas: una variable extrínseca -que afecta aproximadamente entre 70% y 80% de los pacientes que se acompaña de elevación sustancial de los niveles de IgE en suero- y una forma intrínseca o "pura", que se observa en una minoría de enfermos (20% a 30% de los casos), en combinación con bajos niveles de IgE y en ausencia de sensibilización alérgica detectable.
Llamativamente, los estudios epidemiológicos y genéticos mostraron que la DA de comienzo en la niñez no siempre se asocia con elevación de la IgE en suero y con sensibilización alérgica. Por ello cabe la posibilidad de que la enfermedad comience como una forma intrínseca, no mediada por IgE, que evoluciona luego hacia la forma extrínseca, mediada por IgE.
More than 50% of children who are affected in the first 2 years of life do not have any sign of IgE sensitization, but they become sensitized during the course of atopic dermatitis. Up to 70% of these children have a spontaneous remission before adolescence.
Risk for developing asthma at 8 years of age increases with AD severity
The atopic march is the natural history of atopic manifestations, characterized by a typical sequence of progression of clinical signs of atopic disease, with some signs becoming more prominent while others subside. In general, the clinical signs of AD predate the development
of asthma and allergic rhinitis, suggesting that AD is an “entry point” for subsequent allergic disease.
The cutaneous manifestations of atopy often represent the beginning of the atopic march. On the basis of several longitudinal studies, approximately half of AD patients will develop asthma, particularly with severe AD, and two thirds will develop allergic rhinitis. Epicutaneous sensitization has been thought to be responsible, with subsequent migration of sensitized T cells into the nose and airways, causing upper and lower airway disease.
As a result of genetically determined epidermal-barrier dysfunction and the effect of environmental factors, non-atopic dermatitis is the first manifestation of atopic dermatitis. Subsequently, because of their genetic predisposition for IgE-mediated sensitization (presentes en 60-80% de DA), patients become sensitized. Recently, the 3D structures of many allergens have been solved, allowing detailed comparisons between
homologous structures. With respect to cross-reactivity to self antigens, the most interesting structures are those

The primary structure of this important enzyme is highly conserved among phylogenetically distant species, indicating the involvement of CyP in basic cellular functions

In sensitized atopic individuals contact with exogenous allergens (orange asterisks) may lead to tissue damage and release of autoantigens (blue asterisks).
