automatically covered by PhilHealth and instructs the state health
insurance agency to develop packages tailor fit for their health and
development needs.
4. How to apply for issuance of Persons with Disability ID?
1.There are two ways for applying for issuance of Persons with Disability ID thru: (1) walk-in application and
(2) online application.
How do I apply for issuance of Persons with Disability thru "walk-in application"?
1.Go to the nearest Municipal/City Social Welfare Development Office in your residence and submit
your complete requirements for Persons with Disability ID application.
How do I apply for issuance of Persons with Disability thru "online application"?
1.1. Click the Online Application at the Menu and fill-out the Form.
2.2. After filling-out the Online Application Form, click the Add button.
3.3. Review your application details.
4.4. Click the Edit button if you want to edit your application. If the information entered are correct already,
click the Confirm button to submit the Form. The system shall prompt a receipt message.
5. What are the requirements needed to submit for the Persons with Disability ID
application?
Please bring the following requirements to the nearest Municipal/City Social
Welfare Development Office from your residence:
1. Two (2) “1×1” recent ID pictures with the names and signatures or thumb marks
at the back of the picture.
2. One (1) Valid Government ID or barangay clearance/certification.
3. Disability Certificate / Medical Certificate / School Assessment.
What ID does the Person with disability have to submit? Is it mandatory for them to
submit voter’s certificate?
No.The Person with Disability should submit any valid ID or Barangay
Clearance/Certificate.
6. 1. Is it okay if I do not visit any Municipal/City Social Welfare Development Officer anymore after my online
application?No. Successful online application does not guarantee ID issuance. The Persons with Disability ID
issuance shall officially be accomplished in the nearest Municipal/City Social Welfare Development Office
with your complete documentary requirements.
2. How do I verify the status of my Persons with Disability ID card?Under Menu, click the ID Verification. Input
your ID number into the space provided to check the status of your Persons with Disability ID.
3. What if the Persons with Disability is bedridden and he/she cannot physically appear to submit documentary
requirements?His/Her Guardian should bring an authorization letter and other requirements on behalf of
the Persons with Disability.
