What is Connected Care?
Community led commissioning - involving communities in design and delivery of
services
A new definition of commissioning - understanding individual and/or community
needs to build a platform for procurement
The model can focus on whole communities or be adapted to specific groups
(Gypsy, Roma Travellers, people with Long Term Conditions and carers are a
few examples)
Connected Care is about doing things differently

Projects undertaken in 14 areas in England - including Basildon, Bolton, Clacton
and Jaywick, Hartlepool, Warrington, East Lancashire, East Sussex, London,
Norfolk and Suffolk
Over 210 community researchers recruited and trained
Engaging a total population size of 150,000
Better use of resources - £4 saved for every £1 invested
Education, employment, training opportunities and new social capital
New community-led social enterprises delivering services
Locally developed models of integration

Hartlepool
The pilot aimed to develop more responsive and accessible local services in the
Owton ward
An audit of need was conducted by a team of navigators in the area
Benefits and services secured for the community included:
An independent living service for vulnerable older people in the community
A supported housing complex
A handyman service
A time bank

6. Connected Care
Barnsley
Two year partnership project with Barnsley Metropolitan Borough Council and
partner providers
It aims to build community engagement, set up a new community enterprise to
take on delivery of some public services, create employment and make savings
Year 1 – community engagement, service design and developing a baseline for
the current cost of service delivery in the area
Year 2 – set up of community-led integrated neighbourhood services
The partnership is based on a risk and gain share basis with the Council
