  2. WE‘VE ALL HEARD STORIES OF CATASTROPHIC INJURIES ON THE JOB – EVEN FATALITIES. ACCORDING TO OSHA STATISTICS, APPROXIMATELY 60,000 INJURIES AND MORE THAN 100 FATALITIES OCCUR EVERY YEAR, AND IT IS ESTIMATED THAT 10 PERCENT OF INDUSTRIAL ACCIDENTS ARE LOCKOUT/TAGOUT (LO/TO) RELATED. THESE STATISTICS REPRESENT ACCIDENTS RESULTING SPECIFICALLY FROM FAILURE TO DE-ENERGIZE AND FOLLOW A LOCKOUT PROGRAM WHEN PERFORMING SERVICE AND MAINTENANCE WORK. STUDIES HAVE SHOWN THAT AN EFFECTIVE LO/TO PROGRAM CAN REDUCE ACCIDENTS BY 30-50 PERCENT, AND SOME INSURANCE COMPANIES EVEN OFFER LOWER PREMIUMS TO COMPANIES WITH DEMONSTRATED, EFFECTIVE LOCKOUT PROGRAMS. EFFECTIVE LO/TO PROCEDURES CAN ALSO DRAMATICALLY INCREASE YOUR COMPANY‘S PRODUCTIVITY BY MAKING LOCKOUT ACTIVITIES MORE EFFICIENT, REDUCING DOWNTIME.UNFORTUNATELY, MANY COMPANIES‘ PROCEDURES FOR INDIVIDUAL MACHINES ARE INCONSISTENT OR UNCLEAR, USING VARIED SOFTWARE PROGRAMS AND FORMATS. EVEN WORSE, MANY COMPANIES SIMPLY DO NOT HAVE A FORMAL ENERGY CONTROL PROGRAM, LEAVING MANY WORKERS WITHOUT THE GUIDANCE OR TRAINING THEY NEED TO PERFORM ESSENTIAL LO/TO PROCEDURES EFFICIENTLY AND EFFECTIVELY. THE RESULTS OF THIS APPROACH CAN BE DISASTROUS:
  3. IN JUNE 2001, A WORKER IN MARIETTA, GEORGIA, WAS TAMPING DOWN CARDBOARD INSIDE A COMPACTOR WHEN HIS FEET BECAME CAUGHT IN THE CARDBOARD. PINNED INSIDE THE MACHINE, BOTH OF HIS LEGS WERE SEVERED ABOVE THE KNEE WHEN THE MACHINE UNEXPECTEDLY RESTARTED. THIS MAN‘S TRAGEDY IS ONE OF MANY. BECAUSE OF THE FREQUENCY AND SERIOUSNESS OF THESE ACCIDENTS, OSHA INSPECTORS ARE SPECIFICALLY INSTRUCTED TO LOOK FOR DOCUMENTED PROCEDURES DURING LO/TO INSPECTIONS.
  4. LO/TO VIOLATIONS ARE ONE OF THE MOST FREQUENT OSHA CITATIONS, LEADING TO COSTLY FINES FOR MANY COMPANIES – AND THAT‘S EVEN WITHOUT THE LOST PRODUCTIVITY, HEALTHCARE COSTS, AND POTENTIAL LITIGATION OF A WORK-RELATED INJURY OR FATALITY. IN THE CASE PREVIOUSLY MENTIONED, THE COMPANY WAS FINED $140,000, INCLUDING $70,000 FOR FAILURE TO USE DOCUMENTED LOCKOUT PROCEDURES TO RENDER THE COMPACTOR INOPERABLE WHILE THE EMPLOYEE WAS IN THE MACHINE‘S CHAMBER. COSTS FOR ONE CATASTROPHIC ACCIDENT CAN SPIRAL INTO MILLIONS OF DOLLARS. FAILURE TO LOCK OUT AND BLOCK OUT MACHINERY BEFORE WORKING ON IT IS A MAJOR CAUSE OF SERIOUS INJURY AND DEATH WITHIN INDUSTRIAL ENVIRONMENTS. WORKERS ARE ELECTROCUTED—OR LOSE FINGERS, HANDS, ARMS – OR SUFFER SEVERE CRUSHING INJURIES BECAUSE MACHINERY IS INADVERTENTLY TURNED ON WHILE IT IS BEING MAINTAINED, REPAIRED OR ADJUSTED. THESE INJURIES CAN BE PREVENTED BY ESTABLISHING AND USING AN EFFECTIVE LOCKOUT PROGRAM.
  5. FAILURE TO LOCK OUT AND BLOCK OUT MACHINERY HAS CAUSED DEVASTATING INJURY AND DEATH TO WORKERS, AS THE FOLLOWING CASE HISTORIES INDICATE: THREE MEN WERE DOING MAINTENANCE INSIDE AN ASPHALT PUG MILL TYPE MIXER. ONE EMPLOYEE WAS STILL INSIDE THE MIXER WHEN THE POWER WAS TURNED BACK ON, THEREBY STARTING THE MIXER. HE WAS KILLED INSTANTLY. CAUSE: FAILURE TO DISCONNECT POWER SOURCE AND LOCK OUT. A COTTON GIN OPERATOR CLIMBED INTO A JAMMED COTTON CLEANER/SEPARATOR. THE TOGGLE SWITCH CONTROLLING THE OPERATION OF THE GIN WAS TURNED OFF BUT NOT LOCKED OUT. FOR SOME REASON, SOMEONE ACCIDENTALLY TURNED THE MACHINE BACK ON, NOT KNOWING THE GIN OPERATOR WAS INSIDE THE GIN. THE OPERATOR'S LEFT LEG WAS PULLED THROUGH THE FEED ROLLERS. CAUSE: FAILURE TO DISCONNECT POWER SOURCE AND LOCK OUT. A WAREHOUSEMAN WAS REPAIRING AN AIR-OPERATED VALVE WHICH HE HAD TURNED OFF BUT NOT DISCONNECTED AND LOCKED OUT. DURING THE REPAIR OPERATION HE SLIPPED AND INADVERTENTLY TURNED ON THE SWITCH WHICH LET AIR INTO THE VALVE. HIS HAND WAS CAUGHT AND CRUSHED IN THE VALVE. CAUSE: FAILURE TO DISCONNECT POWER SOURCE AND LOCK OUT. A MAINTENANCE EMPLOYEE WAS CHANGING V-BELTS ON AN EXHAUST FAN. HE DE-ENERGIZED THE FAN BEFORE STARTING WORK. HOWEVER, HE DID NOT BLOCK THE BLADES OF THE FAN. THE SUCTION IN THE DUCT WORK TURNED THE FAN BLADES, AND HIS HAND WAS CAUGHT IN THE V-BELT DRIVE. CAUSE: FAILURE TO BLOCK OUT POTENTIAL ENERGY SOURCES.
  6. BUT EVEN A LOCKED-OUT MACHINE MAY NOT BE SAFE IF THERE ARE PARTS OF THE MACHINE THAT ARE NOT BLOCKED TO PREVENT INADVERTENT MOVEMENT. POTENTIAL ENERGY THAT MAY NEED TO BE BLOCKED CAN COME FROM SUSPENDED PARTS, SUBJECT TO GRAVITY, OR MAY BE ENERGY STORED IN SPRINGS. IN A U.S. BUREAU OF LABOR STATISTICS STUDY ON INJURIES RELATED TO SERVICING EQUIPMENT, 80 PERCENT OF THE WORKERS SURVEYED FAILED TO EVEN TURN OFF THE EQUIPMENT BEFORE PERFORMING THE SERVICE WORK. THERE IS A DIFFERENCE BETWEEN TURNING OFF A MACHINE AND ACTUALLY DISENGAGING OR DE-ENERGIZING A PIECE OF EQUIPMENT. WHEN YOU TURN OFF A CONTROL SWITCH, YOU ARE OPENING A CIRCUIT. THERE IS STILL ELECTRICAL ENERGY AT THE SWITCH, AND A SHORT IN THE SWITCH OR SOMEONE INADVERTENTLY TURNING ON THE MACHINE MAY START THE MACHINE RUNNING AGAIN.
  7. STATISTICS SHOW THAT OF THE 20 PERCENT OF THE WORKERS WHO DID TURN OFF THE MACHINERY, ABOUT HALF OF THEM WERE INJURED WHEN SOMEONE, GENERALLY A COWORKER WHO WAS UNAWARE THAT THE MACHINE WAS BEING SERVICED, ACCIDENTALLY REACTIVATED THE MACHINERY.
  8. AND A FIFTH OF THOSE WORKERS WHO TURNED OFF THE CONTROL SWITCH WERE INJURED BY THE ENERGY STILL IN THE MACHINE WHICH SHOULD HAVE BEEN BLOCKED. THE MOVING PARTS OF THE MACHINE EITHER CONTINUED TO COAST, OR THE PARTS MOVED WHEN THE JAM-UP WAS CLEARED. AN ACCIDENT IN CALIFORNIA ILLUSTRATES THIS PROBLEM. A TABLE SAW WAS TURNED OFF, BUT THE SAW BLADE WAS STILL COASTING AND HAD NOT COME TO A COMPLETE STOP. AN EMPLOYEE BEGAN CLEANING THE MACHINE, AND HIS FINGER WAS AMPUTATED BY THE BLADE. OTHER ACCIDENTS HAVE OCCURRED WHEN THE CONTROL SWITCH ON A MACHINE WAS TURNED OFF, BUT A SHORT IN THE SWITCH RESTARTED THE MACHINE. ACCIDENTS HAVE ALSO OCCURRED EVEN WHEN WORKERS DID TAKE THE NECESSARY STEPS OF DISCONNECTING THE MAIN POWER SOURCE. BUT THEY DID NOT PERFORM A CRUCIAL STEP FOR A COMPLETE LOCKOUT PROCEDURE.
  9. ACCIDENTS HAVE ALSO OCCURRED EVEN WHEN WORKERS DID TAKE THE NECESSARY STEPS OF DISCONNECTING THE MAIN POWER SOURCE. BUT THEY DID NOT PERFORM A CRUCIAL STEP FOR A COMPLETE LOCKOUT PROCEDURE. THEY DID NOT TEST THE EQUIPMENT TO MAKE SURE THE MACHINERY WAS, IN FACT, DE-ENERGIZED. ONE CASE, THE LOCKOUT HAD BEEN DONE ON THE WRONG POWER LINE. IN ANOTHER CASE, A SECOND POWER LINE HAD BEEN SPLICED INTO THE WIRING BEYOND THE POINT OF THE LOCKOUT.
  10. JUST SHUTTING OFF, UNPLUGGING, OR DISCONNECTING EQUIPMENT IS NOT ENOUGH… COMPLY WITH OSHA REGULATIONS, LOCK IT OUT! BEFORE REPAIRS, SERVICING, MAINTENANCE, OR SET-UP OF MACHINES AND EQUIPMENT CAN BEGIN, PREVENT THE UNEXPECTED — THE ACCIDENTAL START UP! PROCEDURES, DEVICES, AND PERSONNEL MUST BE SET IN PLACE TO PREVENT A SERIOUS INJURY OR DEATH THAT COULD OCCUR WHEN SOMEONE THINKS SOMETHING IS SAFELY OFF. TO ENSURE MACHINES AND EQUIPMENT REMAIN OFF MEANS TO ESTABLISH A ―PROGRAM CONSISTING OF ENERGY CONTROL PROCEDURES, EMPLOYEE TRAINING, AND PERIODIC INSPECTIONS...”, AS DESCRIBED IN THE OSHA STANDARD. THE STANDARD ALSO IDENTIFIES PRODUCTS USED FOR LOCKOUT/TAGOUT, SUCH AS LOCKOUT DEVICES AND TAGOUT DEVICES.
  11. HIGHLIGHTED HERE ARE SEVEN STEPS TO CREATING AN EFFECTIVE LOCKOUT/TAGOUT PROGRAM: 1. BE AWARE OF THE LOCKOUT REGULATIONS — BECOME FAMILIAR WITH THE FEDERAL REGULATIONS KNOWN AS THE CONTROL OF HAZARDOUS ENERGY (LOCKOUT/TAGOUT). 2. DEVELOP LIST OF MACHINERY & EQUIPMENT — SURVEY YOUR FACILITY FOR ALL MACHINERY AND EQUIPMENT THAT HAVE THE POTENTIAL FOR REQUIRING LOCKOUT. 3. IDENTIFY & MARK ENERGY SOURCES & LOCKOUT POINTS — LOCATE AND DOCUMENT START UP AREAS OF ENERGY SOURCES, SUCH AS: • MECHANICAL • ELECTRICAL • HYDRAULIC • THERMAL • PRESSURE • PROCESS CONTROL • STORED ENERGY — GRAVITY — SPRINGS POST SIGNS, APPLY LABELS AND TAGS… …TO WARN AND IDENTIFY ENERGY SOURCE AREAS AND POINTS REQUIRING SHUT DOWN AND LOCKOUT/TAGOUT PROCEDURES. 4. DETERMINE THE LOCKOUT CONTROLS — IDENTIFY AND DOCUMENT THE NECESSARY DEVICES THAT ISOLATE AND LOCKOUT ENERGY SOURCES, AND SECURE THEM FOR ON-SITE USE. STANDARDIZE THE DEVICES FOR VARIOUS FUNCTIONS FOR PROPER IDENTIFICATION. 5. SET-UP LOCKOUT TRAINING & PROCEDURE PROGRAM — DEVELOP, STANDARDIZE, AND DOCUMENT THE LOCKOUT PROCESS — FOR EACH ENERGY SOURCE. IDENTIFY EMPLOYEES‘ LEVEL OF INVOLVEMENT IN THE PROGRAM – THOSE THAT ARE MADE AWARE, AFFECTED, AND TO PERFORM THE LOCKOUT PROCEDURES. ACCOMPLISH THIS THROUGH COMPREHENSIVE TRAINING PROGRAMS AND CERTIFICATIONS. 6. IMPLEMENT THE LOCKOUT PROGRAM — INITIATE AND ENACT THE LOCKOUT/TAGOUT PROGRAM THAT FOLLOWS THE TRAINING PROCEDURES. POST REMINDERS AND HAVE THE NECESSARY LOCKOUT/TAGOUT DEVICES READILY AVAILABLE.7. REVIEW LOCKOUT PROGRAM — IDENTIFY AND DOCUMENT CHANGES TO EXISTING LOCKOUT PROCEDURES AND NEW ENERGY SOURCES THAT REQUIRE LOCKOUT. MAKE CORRECTIONS, CHANGES AND ADDITIONS TO THE PROGRAM. CONDUCT PERIODIC REVIEW OF LOCKOUT PROCEDURES (REQUIRED AT LEAST ANNUALLY) TO ENSURE THEY ARE UP TO DATE AND REQUIREMENTS ARE BEING MET.
  12. HIGHLIGHTED HERE ARE SEVEN STEPS TO CREATING AN EFFECTIVE LOCKOUT/TAGOUT PROGRAM: 1. BE AWARE OF THE LOCKOUT REGULATIONS — BECOME FAMILIAR WITH THE FEDERAL REGULATIONS KNOWN AS THE CONTROL OF HAZARDOUS ENERGY (LOCKOUT/TAGOUT). 2. DEVELOP LIST OF MACHINERY & EQUIPMENT — SURVEY YOUR FACILITY FOR ALL MACHINERY AND EQUIPMENT THAT HAVE THE POTENTIAL FOR REQUIRING LOCKOUT. 3. IDENTIFY & MARK ENERGY SOURCES & LOCKOUT POINTS — LOCATE AND DOCUMENT START UP AREAS OF ENERGY SOURCES, SUCH AS: • MECHANICAL • ELECTRICAL • HYDRAULIC • THERMAL • PRESSURE • PROCESS CONTROL • STORED ENERGY — GRAVITY — SPRINGS POST SIGNS, APPLY LABELS AND TAGS… …TO WARN AND IDENTIFY ENERGY SOURCE AREAS AND POINTS REQUIRING SHUT DOWN AND LOCKOUT/TAGOUT PROCEDURES. 4. DETERMINE THE LOCKOUT CONTROLS — IDENTIFY AND DOCUMENT THE NECESSARY DEVICES THAT ISOLATE AND LOCKOUT ENERGY SOURCES, AND SECURE THEM FOR ON-SITE USE. STANDARDIZE THE DEVICES FOR VARIOUS FUNCTIONS FOR PROPER IDENTIFICATION. 5. SET-UP LOCKOUT TRAINING & PROCEDURE PROGRAM — DEVELOP, STANDARDIZE, AND DOCUMENT THE LOCKOUT PROCESS — FOR EACH ENERGY SOURCE. IDENTIFY EMPLOYEES‘ LEVEL OF INVOLVEMENT IN THE PROGRAM – THOSE THAT ARE MADE AWARE, AFFECTED, AND TO PERFORM THE LOCKOUT PROCEDURES. ACCOMPLISH THIS THROUGH COMPREHENSIVE TRAINING PROGRAMS AND CERTIFICATIONS. 6. IMPLEMENT THE LOCKOUT PROGRAM — INITIATE AND ENACT THE LOCKOUT/TAGOUT PROGRAM THAT FOLLOWS THE TRAINING PROCEDURES. POST REMINDERS AND HAVE THE NECESSARY LOCKOUT/TAGOUT DEVICES READILY AVAILABLE.7. FAILURE TO LOCK OUT AND BLOCK OUT MACHINERY HAS CAUSED DEVASTATING INJURY AND DEATH TO WORKERS, AS THE FOLLOWING CASE HISTORIES INDICATE: THREE MEN WERE DOING MAINTENANCE INSIDE AN ASPHALT PUG MILL TYPE MIXER. ONE EMPLOYEE WAS STILL INSIDE THE MIXER WHEN THE POWER WAS TURNED BACK ON, THEREBY STARTING THE MIXER. HE WAS KILLED INSTANTLY. CAUSE: FAILURE TO DISCONNECT POWER SOURCE AND LOCK OUT. A COTTON GIN OPERATOR CLIMBED INTO A JAMMED COTTON CLEANER/SEPARATOR. THE TOGGLE SWITCH CONTROLLING THE OPERATION OF THE GIN WAS TURNED OFF BUT NOT LOCKED OUT. CAUSE: FAILURE TO DISCONNECT POWER SOURCE AND LOCK OUT. A WAREHOUSEMAN WAS REPAIRING AN AIR-OPERATED VALVE WHICH HE HAD TURNED OFF BUT NOT DISCONNECTED AND LOCKED OUT. DURING THE REPAIR OPERATION HE SLIPPED AND INADVERTENTLY TURNED ON THE SWITCH WHICH LET AIR INTO THE VALVE. HIS HAND WAS CAUGHT AND CRUSHED IN THE VALVE. CAUSE: FAILURE TO DISCONNECT POWER SOURCE AND LOCK OUT. A MAINTENANCE EMPLOYEE WAS CHANGING V-BELTS ON AN EXHAUST FAN. HE DE-ENERGIZED THE FAN BEFORE STARTING WORK. HOWEVER, HE DID NOT BLOCK THE BLADES OF THE FAN. THE SUCTION IN THE DUCT WORK TURNED THE FAN BLADES, AND HIS HAND WAS CAUGHT IN THE V-BELT DRIVE. CAUSE: FAILURE TO BLOCK OUT POTENTIAL ENERGY SOURCES.
