La coledocolitiasis se refiere a la presencia de piedras en el conducto biliar común y causa dolor abdominal, ictericia y signos de colestasis. El diagnóstico incluye ultrasonido, pruebas de laboratorio y posiblemente colangiopancreatografía retrógrada endoscópica. El tratamiento consiste en la remoción endoscópica de las piedras y una colecistectomía para prevenir la recurrencia.
3. Generalidades
■ Sus características clínicas
incluyen: dolor en el
cuadrante superior derecho
y signos de colestasis
extrahepática.
■ El diagnóstico inicial
incluye un ultrasonido y
estudios de laboratorio de
rutina
5. Epidemiología
■ Sexo: ♀ > ♂
■ Prevalencia: ~5-20% de los
pacientes que sufren una
colecistectomía han tenido
coledocolitiasis al momento de la
cirugía
■ Mayor incidencia: >40 años

6. Etiología
■ Coledocolitiasis secundaria (mas
común)
– Colelitiasis → pasaje de
piedras al conducto biliar
común → obstrucción del
conducto biliar común →
espasmos de los conductos
biliares
■ Factores de riesgo: historia de
colelitiasis

7. Etiología
■ Coledocolitiasis primaria
– Condiciones que predisponen
a una estasis biliar →
formación intraductal de
piedras
■ Factores de riesgo:
– Fibrosis quística
– Nutrición parenteral total
prolongada

8. Características clínicas
■ Dolor en cuadrante superior
derecho
– Más severo y prolongado que
en colelitiasis
– Postpandriale
– Se puede irradiar al
epigastrio, hombro derecho
y la espalda
9. Características clínicas
■ Náusea, vómito, anorexia
■ Signos de colestasis extrahepática
(ictericia, acolia, coluria, prurito)
■ Características de complicaciones
10. Diagnóstico
■ Evaluación inicial
– Pruebas de función hepática
– Ultrasonido de cuadrante
superior izquierdo

11. Diagnóstico
■ Imágenes confirmatorias:
– Alta probabilidad de
coledocolitiasis: CPRE
– Probabilidad intermedia de
coledocolitiasis: RM o USG
endoscópico

coledocolitiasis: no
requerido
