By Beth W. Orenstein
Musculoskeletal (MSK) sonography, which is
used in the evaluation and treatment of joint and
soft-tissue diseases, has long been popular in
Europe and other countries overseas. While MSK
sonography isn’t used as widely in the United
States, it has been growing rapidly here over the
last decade as well.
As the U.S. looks at ways to reduce health-care
costs yet maintain quality, and, as concerns
about radiation safety grow, more American
physicians are expected to use MSK sonography
- not only to diagnose but also to treat
musculoskeletal injuries.
In response to this growing demand worldwide,
the American Registry for Diagnostic Medical
Sonography (ARDMS) is launching an MSK
sonography credential. ARDMS will begin
accepting examination applications for its
Registered in Musculoskeletal™ (RMSK™)
ARDMS to Launch Exam for MSK
Sonography Credential This Fall
sonography credential this summer. Medical
professionals will be able to take the exam
starting in mid-September.
ARDMS CEO and Executive Director Dale
R. Cyr says the ARDMS RMSK credential is a
pioneering effort in the field of sonography. It
was established, he says,“to bridge global and
continued
SDMS News Wave  January 2012 2
domestic musculoskeletal sonography practices
and build a community of health-care professionals
working toward the same goal of patient safety
through standardized practice.”
At first, ARDMS is expecting to see a greater demand
for the new exam overseas where MSK is the number
one or number two most commonly performed
sonographic study.“Outside of North America, it
fluctuates between OB/GYN and MSK, ”says Kevin D.
Evans, PhD, RT(R)(M)(BD), RDMS, RVS, FSDMS, Chair
of the ARDMS Board of Directors. However, he says,
“as MSK sonography becomes more common in the
United States, ARDMS expects to see the U.S. demand
for this new credential to increase dramatically.”
RMSK answers a growing need
ARDMS has been working on developing the RMSK
credential for several years because of the growing
interest it has seen.“One of the reasons we started
looking into developing the RMSK credential was
the growing use of MSK sonography in the U.S. and
because we were getting so many inquiries from our
Registrants and the medical community at large,”
Evans says.“It’s an exciting time for ARDMS and
practice of sonography.”
Before deciding to go ahead with developing the
MSK exam, ARDMS surveyed the marketplace and
found interest to be at a high level.“The ARDMS
mission is to promote quality care and patient safety
through the certification and continuing competency
of ultrasound professionals, ”says Evans.“MSK
sonography is quickly becoming a worldwide imaging
standard. It was clear that a professional credential in
that specialty is both timely and needed. ”
The MSK credentialing exam will be open to
medical professionals who practice musculoskeletal
ultrasound. That includes not only sonographers but
also physical medicine and rehabilitation specialists,
sports medicine practitioners, osteopathic surgeons,
radiologists, chiropractors, rheumatologists,
podiatrists and orthopedic surgeons. The same exam
will be given to physicians and non-physicians.
The MSK exam will be a single exam with the
sonography principles and instrumentation content
all rolled into it. Because the exam is open to anyone
who practices MSK ultrasound, it’s modeled more
after ARDMS’s RPVI (Registered Physician in Vascular
Interpretation) exam than other sonographer exams,
Evans says. There will be one prerequisite applicants
will need to meet, Evans says, but it has yet to be
announced.
It’s likely those who register for the exam will hold an
existing medical degree or certification and will have
performed a certain number of MSK studies, he says.
“The prerequisite was designed so that a vast majority
of medical professionals will be eligible to take the MSK
exam and that anyone using MSK sonography should
be able to take it, ”Evans says.
Exam content being developed
The content outline for the RMSK credential is
available at www.ARDMS.org/RMSK. According to the
outline, the tasks to be covered include: anatomy and
physiology (28 %) pathology (28 %), patient care (2%),
integration of data (8%), protocols (15%), physics and
instrumentation (7%), treatment (10%) and“other”
(2%). The anatomic areas to be covered will include:
shoulder, elbow, wrist and hand, hip, knee, ankle and
foot, soft tissue and general.
It’s still undecided whether the RMSK credential
exam will be all multiple choice questions or include
advanced item types – where the questions involve
simulated ultrasound consoles. Such questions might
list a problem statement and display the controls of an
ultrasound machine. The test-taker would be asked to
adjust the controls that translate to a correct solution
to the problem. Or it could be that the test-takers are
asked to use their mouse to indicate a particular detail
of the image on the screen.
Item writers are working on the questions and,“they’re
being vetted and reviewed, ”Evans says.“Multiple
versions of the examination are being developed.”
Because the vast majority of MSK sonography studies
are done outside of the U.S., ARDMS has sought input
from experts from around the world in developing the
credential, Cyr notes.
ARDMS is developing a practice version of the MSK
examination and hopes to have it ready prior to the
formal exam launch in mid-September.“Medical
professionals will be able to visit the ARDMS website
(www.ARDMS.org) and take a practice version of the
MSK examination for a nominal fee, ”Evans says.
Opportunity to take exam limited
Because it is the first time it’s being given, the exam
will have a window of about a month when it can be
taken, Evans says.“Our other exams are administered all
the time, ”Evans says,“but our MSK is going to be in a
SDMS News Wave  January 2012 3
specific time frame. We will announce the time frames
in the spring and summer for the next several years of
administration.”
After the first group has taken the exam, ARDMS will
determine what score test-takers will need to achieve
in order to pass, says Ellen Julian, PhD, Director of
Testing and Research at ARDMS.
Dr. Julian explains why the test is scored that way:
“Typically, the way we build a test, we have questions
we’ve used in the past and know exactly how difficult
they are. Those are the ones on which we score people
and, because they are known quantities, we can
calculate in advance the number correct needed to
pass.”
However, because this is a new test,“and we’ve never
used any of the questions before, we don’t know how
hard the questions will be and which ones are working
well. We have to get all the data back and analyze the
questions to determine what the cut score will be.”
Also, because it’s a new test, examinees will not hear
the results as quickly as other ARDMS credentialing
exams.“We’re saying 60 days from the last test date,
”Dr. Julian says,“but we’re hoping to have the scores
out in less time.”
The first administration for the MSK exam will be
mid-September to mid-October 2012. The second
administration is slated for mid-March through
mid-April 2013.“If demand is high enough, the MSK
examination may move to continuous administration
just like our other exams, ”Evans says.
Sonographers who are interested also can sign-up on
the ARDMS Website (www.ARDMS.org/RMSK) so that
they will receive alerts about the RMSK exam when
things are happening, Evans says.
SDMS Annual Conference to include MSK
Sonographers who are interested in taking the new
ARDMS RMSK exam can prepare by attending this
year’s SDMS Annual Conference, Sept. 20-23, at the
Washington State Convention and Trade Center in
Seattle.
At this year’s conference, musculoskeletal ultrasound
will be part of the abdominal track. Janette Wybo, BS,
RDMS, RDCS, RVT, Program Coordinator at Providence
Hospital School of Diagnostic Medical Sonography
and a member of the SDMS Conference Management
Committee, says MSK sonography is one of the most
requested topics by attendees.
The Thursday tutorial will have two 90-minute MSK
lectures with hands-on scanning by invited faculty,
Veronica Rodriguez, RDMS, CEO, of Great Lakes
Musculoskeletal Ultrasound Inc. Rodriquez has 15
years of experience dedicated to MSK sonography
and has conducted lectures and scanning workshops
at national and international musculoskeletal
conferences, Wybo says.
At the first Thursday session, Rodriguez will lecture
on shoulder scanning protocol and sonographic
appearances with the majority of time devoted to
live scanning. The second session will focus on MSK
scanning of the ankle, knee, wrist and elbow.
“To make the session most effective for everyone,
there will be dual screens with a videographer
projecting scanning transducer placement along with
displayed machine image, ”Wybo says.“This worked
out well for attendees last year for scanning technique
and instruction, ”she notes.
During the main conference track, the SDMS
Conference Management Committee has asked
Sandra J. Allison, MD, Associate Professor of Radiology
from Georgetown University in Washington, D.C., to
return as part of the conference faculty. Dr. Allison
has a passion for musculoskeletal imaging and will
be giving two lectures on MSK ultrasound and their
applications for the upper and lower extremities. She
will focus on how ultrasound has advanced in imaging
and diagnosis along with what circumstances it
should be the modality of choice for clinicians.
“Attendees have enjoyed Dr. Allison’s lectures as part
of the SDMS conference in the past and we are excited
she will be joining us in Seattle, ”Wybo says.

Attention Advanced
Practice Sonographers and
Ultrasound Practitioners
Your Participation Requested in a Survey
The Advanced Practice
Committee of the SDMS,
in collaboration with
the American Registry
of Diagnostic Medical
Sonography (ARDMS),
is hosting an“Advanced
Practice in Sonography”
survey during the month
of February. SDMS is
soliciting participation
from sonographers who
are currently functioning
in the capacity of a mid-level provider, physician
extender. The link to complete this survey is:
http://www.surveymonkey.com/s/LT9YMJB
The SDMS has been actively working on the concept
of an Ultrasound Practitioner (UP) for over a decade.
The foundation for the UP concept is the creation
of a mid-level provider of sonography services that
functions as a physician extender. While there is
currently no“official”designation, title, or certification
examination associated with this position, we know
that some sonographers are currently functioning at
this level. It is important that we know and understand
the extent to which some sonographers have already
moved into this role, and the responsibilities and
clinical activities that they have assumed in making
this transition. To document this information to
aid in future decisions, the SDMS and ARDMS have
developed this survey.
We ask that sonographers who are currently
functioning in this physician extender role (not to be
interpreted as a“lead sonographer”or the SDMS APS
membership classification, to complete the survey).
For purposes of this survey, this sonography physician
extender shall be referred to as an Ultrasound
Practitioner.

First Sonography Bachelor’s Degree Students
on the Continent of Africa!
SDMS and the SDMS Foundation recently donated
textbooks, educational videos, scrub tops, and
totebags for sonography students at the Kwame
Nkrumah University of Science and Technology in
Kumasi, Ghana, where Ann Polin, MS, RDMS, RDCS, RVT,
is a faculty lecturer for the Sonography Department.
Ann recently sent SDMS this picture of the sonography
students with their new scrub tops and totebags.
These students will be the first sonography bachelor’s
degree students on the continent of Africa – quite an
accomplishment for all involved!
Ann has returned to Ghana approximately three times
a year since 2000, conducting ultrasound workshops
for physicians, residents and medical students at
the Komfo Anokye Teaching Hospital, and teaching
classes to students in the university’s sonography
bachelor’s degree program.
If you’d like information on how you can get involved,
please contact Ann at WestAfricaAnn@comcast.net.
Correction: The December 2011 SDMS News Wave stated that Ann Polin is currently volunteering at the Aninwah
Medical Center. She is actually a faculty member at the Kwame Nkrumah University of Science and Technology.

Joy Guthrie, PhD, RDMS,
RDCS, RVT
Current position:
Ultrasound Supervisor
and Program Director
and Instructor of the
Diagnostic Medical
Sonography Program
- General and Cardiac
Tracks, at Community
Regional Medical Center
in Fresno, California.
Year named fellow: 2010
What inspired you to make a career of sonography?
A very good mentor, Dr. Wan Chan, introduced me
to Lynda Pitts, RDMS, in the early 1980s. She was an
absolute pioneer and phenomenal sonographer for
her time. She offered private ultrasound lessons and
inspired me to continue forward. The first time I saw
an echocardiogram, I was hooked for life.
Did you have a memorable role model/teacher?
The list is long of those whom I admire and have
served as very memorable role models. These people
share a common thread...”doing the right thing
when no one is looking.”I continue to revere them as
mentors, friends, and all around great people. Who are
they? At the top of the list, for me, is Joan Baker who
emulates all of these qualities. Others are Dr. Michael
Myracle, Dr. William Hastrup, the cardiologist team at
Children’s Hospital Central California, Dr. Krish Rajani
and Dr. Steve Elliott, Marsha Neumeyer, Dale Cyr, Jean
Lea Spitz, Dr. Diane Kawamura, and the entire Board of
Directors/Foundation Board of the SDMS and staff that
I have been privileged to serve with. As well, I continue
to learn from a host of colleagues in our profession
nationwide.
What was the most significant advance in the
technology that you directly experienced?
I believe the move to digital imaging and video clips
This is a continuing series of interviews of
our distinguished SDMS Fellow members.
SDMS Fellow
Spotlight
was the most significant advance. Before the digital
era (developing our films) there was an enormous
amount of pressure to“get it right”the first time as
there was no way to erase a mistake. That, in itself, was
not a bad thing. But with the advent of digital imaging
our exams have improved resolution and diagnostic
capability.
Were there any memorable developments, cases
or events that changed the way you practiced
sonography?
There were a few cases that stand out. One involved
a patient that was misled by a non-sonographer
and business woman at a 3D establishment. The
patient was told by this non-sonographer that her
baby“looks great”when in fact the fetus had a
life-threatening congenital heart defect. This case
prompted me to immediately join the Government
Relations Committee for the SDMS to try and keep
the transducer in the hands of fully qualified and well
educated sonographers.
Another case, which I am sure we can all share, was an
instance that my gut told me that I needed to go the
extra mile. I did, and the end result was the patient
barely escaped an aortic aneurysm rupture. That case
taught me to never give up on a case, especially when
the patient needs you. I have had other sonographers
that I work with share the same type of stories. At the
end of the day, those cases keep us going because we
know we do make a difference in the lives of those
who may not be as fortunate as ourselves. And finally,
from a personal side, was the first case I got to share
with my daughter, Allison. She was one of my students
at the time...it was the best ever...time stood still. She
has now graduated, passed her board exams and is
employed full time as a sonographer in a Perinatal
Center. Good stuff!
What changes do you predict in the immediate
future?
We are at a pivotal time. It is a time for all
sonographers, across all modalities, to unite. We need
to shore up both sides of our profession including
SDMS News Wave  January 2012 14
mandatory certification and/or credentialing of
sonographers as well as to pave a way for a mid-level
practitioner to ensure our place in global health-
care market. I feel that Point of Care sonography will
continue to expand in untraditional venues.
What advice would you give to students/ future
sonographers?
You make your own destiny. Get involved and let your
voice be heard. I am the first to admit that all of you
kids are WAY smarter than I was when I first started.
You have the opportunity to mold our future. You
have a lot to offer. Try and find a mentor to help you
along the way. Tap into their vast knowledge and use
your innate skills to refine that into a newer and fresh
version that only your generation can do. And finally,
remember that there is no room for mediocrity in
medicine. Try your best...it will always serve you well.
We are proud of you!
What’s your favorite part of your job?
I love clinical research and the time in the NICU
scanning our premature infants. I love listening to
our staff and graduates teach each other. To see them
“pass it forward”not only with each other but also
with the students makes me very proud of all
of them.
Maria Johnson is a first
year student in the General
Sonography program at El
Centro College in Dallas,
Texas. She is a mother of
three children of which
all are in college and
developing their own
professional careers. Maria
was employed with AT&T
for ten years where she
worked as an account representative and manager.
After her third child was born she was fortunate to be
able to stay at home with her children. During that
time she took the opportunity to become active in the
community by volunteering in her children’s schools
PTA and Boosters Clubs.
We asked Maria why she decided to re-enter the
workforce and why she chose diagnostic medical
sonography, she replied“When my kids started
looking into colleges I began to think about what
my next steps would be in life. I knew I wanted
to go back to work but I also felt that I wanted a
career where I could make a difference. I had always
regretted not getting into healthcare because I’ve
been around nurses all my life, my mother and aunt
were both nurses.”So she researched many different
areas in healthcare and found that she was drawn to
sonography due to the rapidly advancing technology
as well as its importance in the diagnostic process.
Maria said when she joined the SDMS; she was mostly
excited about the free subscription to the Journal
of Diagnostic Medical Sonography (JDMS). She said
the JDMS is an important teaching tool both for her
current classes and continuing education as a future
sonographer. Maria said,“Overall, my favorite benefits
of SDMS, is the vast information available to me
on the website and in the JDMS. Between the two
sources I have a plethora of valuable information from
exam scheduling information to new technological
advances and interesting pathology cases. This
information has been invaluable to me as a new
sonography student and makes me very excited to be
getting started in this amazing field.”
Maria, welcome to the SDMS!

Comments, questions or concerns about the articles appearing in SDMS News Wave, should be directed to newswave@sdms.org. By Beth W. Orenstein Musculoskeletal (MSK) sonography, which is used in the evaluation and treatment of joint and soft-tissue diseases, has long been popular in Europe and other countries overseas. While MSK sonography isn’t used as widely in the United States, it has been growing rapidly here over the last decade as well. As the U.S. looks at ways to reduce health-care costs yet maintain quality, and, as concerns about radiation safety grow, more American physicians are expected to use MSK sonography - not only to diagnose but also to treat musculoskeletal injuries. In response to this growing demand worldwide, the American Registry for Diagnostic Medical Sonography (ARDMS) is launching an MSK sonography credential. ARDMS will begin accepting examination applications for its Registered in Musculoskeletal™ (RMSK™) ARDMS to Launch Exam for MSK Sonography Credential This Fall sonography credential this summer. Medical professionals will be able to take the exam starting in mid-September. ARDMS CEO and Executive Director Dale R. Cyr says the ARDMS RMSK credential is a pioneering effort in the field of sonography. It was established, he says,“to bridge global and continued
  • 2. SDMS News Wave  January 2012 2 domestic musculoskeletal sonography practices and build a community of health-care professionals working toward the same goal of patient safety through standardized practice.” At first, ARDMS is expecting to see a greater demand for the new exam overseas where MSK is the number one or number two most commonly performed sonographic study.“Outside of North America, it fluctuates between OB/GYN and MSK, ”says Kevin D. Evans, PhD, RT(R)(M)(BD), RDMS, RVS, FSDMS, Chair of the ARDMS Board of Directors. However, he says, “as MSK sonography becomes more common in the United States, ARDMS expects to see the U.S. demand for this new credential to increase dramatically.” RMSK answers a growing need ARDMS has been working on developing the RMSK credential for several years because of the growing interest it has seen.“One of the reasons we started looking into developing the RMSK credential was the growing use of MSK sonography in the U.S. and because we were getting so many inquiries from our Registrants and the medical community at large,” Evans says.“It’s an exciting time for ARDMS and practice of sonography.” Before deciding to go ahead with developing the MSK exam, ARDMS surveyed the marketplace and found interest to be at a high level.“The ARDMS mission is to promote quality care and patient safety through the certification and continuing competency of ultrasound professionals, ”says Evans.“MSK sonography is quickly becoming a worldwide imaging standard. It was clear that a professional credential in that specialty is both timely and needed. ” The MSK credentialing exam will be open to medical professionals who practice musculoskeletal ultrasound. That includes not only sonographers but also physical medicine and rehabilitation specialists, sports medicine practitioners, osteopathic surgeons, radiologists, chiropractors, rheumatologists, podiatrists and orthopedic surgeons. The same exam will be given to physicians and non-physicians. The MSK exam will be a single exam with the sonography principles and instrumentation content all rolled into it. Because the exam is open to anyone who practices MSK ultrasound, it’s modeled more after ARDMS’s RPVI (Registered Physician in Vascular Interpretation) exam than other sonographer exams, Evans says. There will be one prerequisite applicants will need to meet, Evans says, but it has yet to be announced. It’s likely those who register for the exam will hold an existing medical degree or certification and will have performed a certain number of MSK studies, he says. “The prerequisite was designed so that a vast majority of medical professionals will be eligible to take the MSK exam and that anyone using MSK sonography should be able to take it, ”Evans says. Exam content being developed The content outline for the RMSK credential is available at www.ARDMS.org/RMSK. According to the outline, the tasks to be covered include: anatomy and physiology (28 %) pathology (28 %), patient care (2%), integration of data (8%), protocols (15%), physics and instrumentation (7%), treatment (10%) and“other” (2%). The anatomic areas to be covered will include: shoulder, elbow, wrist and hand, hip, knee, ankle and foot, soft tissue and general. It’s still undecided whether the RMSK credential exam will be all multiple choice questions or include advanced item types – where the questions involve simulated ultrasound consoles. Such questions might list a problem statement and display the controls of an ultrasound machine. The test-taker would be asked to adjust the controls that translate to a correct solution to the problem. Or it could be that the test-takers are asked to use their mouse to indicate a particular detail of the image on the screen. Item writers are working on the questions and,“they’re being vetted and reviewed, ”Evans says.“Multiple versions of the examination are being developed.” Because the vast majority of MSK sonography studies are done outside of the U.S., ARDMS has sought input from experts from around the world in developing the credential, Cyr notes. ARDMS is developing a practice version of the MSK examination and hopes to have it ready prior to the formal exam launch in mid-September.“Medical professionals will be able to visit the ARDMS website (www.ARDMS.org) and take a practice version of the MSK examination for a nominal fee, ”Evans says. Opportunity to take exam limited Because it is the first time it’s being given, the exam will have a window of about a month when it can be taken, Evans says.“Our other exams are administered all the time, ”Evans says,“but our MSK is going to be in a
  • 3. SDMS News Wave  January 2012 3 specific time frame. We will announce the time frames in the spring and summer for the next several years of administration.” After the first group has taken the exam, ARDMS will determine what score test-takers will need to achieve in order to pass, says Ellen Julian, PhD, Director of Testing and Research at ARDMS. Dr. Julian explains why the test is scored that way: “Typically, the way we build a test, we have questions we’ve used in the past and know exactly how difficult they are. Those are the ones on which we score people and, because they are known quantities, we can calculate in advance the number correct needed to pass.” However, because this is a new test,“and we’ve never used any of the questions before, we don’t know how hard the questions will be and which ones are working well. We have to get all the data back and analyze the questions to determine what the cut score will be.” Also, because it’s a new test, examinees will not hear the results as quickly as other ARDMS credentialing exams.“We’re saying 60 days from the last test date, ”Dr. Julian says,“but we’re hoping to have the scores out in less time.” The first administration for the MSK exam will be mid-September to mid-October 2012. The second administration is slated for mid-March through mid-April 2013.“If demand is high enough, the MSK examination may move to continuous administration just like our other exams, ”Evans says. Sonographers who are interested also can sign-up on the ARDMS Website (www.ARDMS.org/RMSK) so that they will receive alerts about the RMSK exam when things are happening, Evans says. SDMS Annual Conference to include MSK Sonographers who are interested in taking the new ARDMS RMSK exam can prepare by attending this year’s SDMS Annual Conference, Sept. 20-23, at the Washington State Convention and Trade Center in Seattle. At this year’s conference, musculoskeletal ultrasound will be part of the abdominal track. Janette Wybo, BS, RDMS, RDCS, RVT, Program Coordinator at Providence Hospital School of Diagnostic Medical Sonography and a member of the SDMS Conference Management Committee, says MSK sonography is one of the most requested topics by attendees. The Thursday tutorial will have two 90-minute MSK lectures with hands-on scanning by invited faculty, Veronica Rodriguez, RDMS, CEO, of Great Lakes Musculoskeletal Ultrasound Inc. Rodriquez has 15 years of experience dedicated to MSK sonography and has conducted lectures and scanning workshops at national and international musculoskeletal conferences, Wybo says. At the first Thursday session, Rodriguez will lecture on shoulder scanning protocol and sonographic appearances with the majority of time devoted to live scanning. The second session will focus on MSK scanning of the ankle, knee, wrist and elbow. “To make the session most effective for everyone, there will be dual screens with a videographer projecting scanning transducer placement along with displayed machine image, ”Wybo says.“This worked out well for attendees last year for scanning technique and instruction, ”she notes. During the main conference track, the SDMS Conference Management Committee has asked Sandra J. Allison, MD, Associate Professor of Radiology from Georgetown University in Washington, D.C., to return as part of the conference faculty. Dr. Allison has a passion for musculoskeletal imaging and will be giving two lectures on MSK ultrasound and their applications for the upper and lower extremities. She will focus on how ultrasound has advanced in imaging and diagnosis along with what circumstances it should be the modality of choice for clinicians. “Attendees have enjoyed Dr. Allison’s lectures as part of the SDMS conference in the past and we are excited she will be joining us in Seattle, ”Wybo says. Beth W. Orenstein is writer for SDMS.
  • 4. SDMS News Wave  January 2012 4 Congratulations The SDMS would like to congratulate Jill Trotter, BS, RT(R), RDMS, RVT on being awarded the prestigious Vanderbilt Radiology Chairman’s Leadership Award. Jill is the Director of the Diagnostic Medical Sonography Program at Vanderbilt University. She also currently serves on the SDMS Foundation Board and is a former SDMS Board member. JillTrotter,BS,RT(R),RDMS,RVT Attention Advanced Practice Sonographers and Ultrasound Practitioners Your Participation Requested in a Survey The Advanced Practice Committee of the SDMS, in collaboration with the American Registry of Diagnostic Medical Sonography (ARDMS), is hosting an“Advanced Practice in Sonography” survey during the month of February. SDMS is soliciting participation from sonographers who are currently functioning in the capacity of a mid-level provider, physician extender. The link to complete this survey is: http://www.surveymonkey.com/s/LT9YMJB The SDMS has been actively working on the concept of an Ultrasound Practitioner (UP) for over a decade. The foundation for the UP concept is the creation of a mid-level provider of sonography services that functions as a physician extender. While there is currently no“official”designation, title, or certification examination associated with this position, we know that some sonographers are currently functioning at this level. It is important that we know and understand the extent to which some sonographers have already moved into this role, and the responsibilities and clinical activities that they have assumed in making this transition. To document this information to aid in future decisions, the SDMS and ARDMS have developed this survey. We ask that sonographers who are currently functioning in this physician extender role (not to be interpreted as a“lead sonographer”or the SDMS APS membership classification, to complete the survey). For purposes of this survey, this sonography physician extender shall be referred to as an Ultrasound Practitioner. • 12. SDMS News Wave  January 2012 12 First Sonography Bachelor’s Degree Students on the Continent of Africa! SDMS and the SDMS Foundation recently donated textbooks, educational videos, scrub tops, and totebags for sonography students at the Kwame Nkrumah University of Science and Technology in Kumasi, Ghana, where Ann Polin, MS, RDMS, RDCS, RVT, is a faculty lecturer for the Sonography Department. Ann recently sent SDMS this picture of the sonography students with their new scrub tops and totebags. These students will be the first sonography bachelor’s degree students on the continent of Africa – quite an accomplishment for all involved! Ann has returned to Ghana approximately three times a year since 2000, conducting ultrasound workshops for physicians, residents and medical students at the Komfo Anokye Teaching Hospital, and teaching classes to students in the university’s sonography bachelor’s degree program. If you’d like information on how you can get involved, please contact Ann at WestAfricaAnn@comcast.net. Correction: The December 2011 SDMS News Wave stated that Ann Polin is currently volunteering at the Aninwah Medical Center. She is actually a faculty member at the Kwame Nkrumah University of Science and Technology.
  • 13. SDMS News Wave  January 2012 13 Joy Guthrie, PhD, RDMS, RDCS, RVT Current position: Ultrasound Supervisor and Program Director and Instructor of the Diagnostic Medical Sonography Program - General and Cardiac Tracks, at Community Regional Medical Center in Fresno, California. Year named fellow: 2010 What inspired you to make a career of sonography? A very good mentor, Dr. Wan Chan, introduced me to Lynda Pitts, RDMS, in the early 1980s. She was an absolute pioneer and phenomenal sonographer for her time. She offered private ultrasound lessons and inspired me to continue forward. The first time I saw an echocardiogram, I was hooked for life. Did you have a memorable role model/teacher? The list is long of those whom I admire and have served as very memorable role models. These people share a common thread...”doing the right thing when no one is looking.”I continue to revere them as mentors, friends, and all around great people. Who are they? At the top of the list, for me, is Joan Baker who emulates all of these qualities. Others are Dr. Michael Myracle, Dr. William Hastrup, the cardiologist team at Children’s Hospital Central California, Dr. Krish Rajani and Dr. Steve Elliott, Marsha Neumeyer, Dale Cyr, Jean Lea Spitz, Dr. Diane Kawamura, and the entire Board of Directors/Foundation Board of the SDMS and staff that I have been privileged to serve with. As well, I continue to learn from a host of colleagues in our profession nationwide. What was the most significant advance in the technology that you directly experienced? I believe the move to digital imaging and video clips This is a continuing series of interviews of our distinguished SDMS Fellow members. SDMS Fellow Spotlight was the most significant advance. Before the digital era (developing our films) there was an enormous amount of pressure to“get it right”the first time as there was no way to erase a mistake. That, in itself, was not a bad thing. But with the advent of digital imaging our exams have improved resolution and diagnostic capability. Were there any memorable developments, cases or events that changed the way you practiced sonography? There were a few cases that stand out. One involved a patient that was misled by a non-sonographer and business woman at a 3D establishment. The patient was told by this non-sonographer that her baby“looks great”when in fact the fetus had a life-threatening congenital heart defect. This case prompted me to immediately join the Government Relations Committee for the SDMS to try and keep the transducer in the hands of fully qualified and well educated sonographers. Another case, which I am sure we can all share, was an instance that my gut told me that I needed to go the extra mile. I did, and the end result was the patient barely escaped an aortic aneurysm rupture. That case taught me to never give up on a case, especially when the patient needs you. I have had other sonographers that I work with share the same type of stories. At the end of the day, those cases keep us going because we know we do make a difference in the lives of those who may not be as fortunate as ourselves. And finally, from a personal side, was the first case I got to share with my daughter, Allison. She was one of my students at the time...it was the best ever...time stood still. She has now graduated, passed her board exams and is employed full time as a sonographer in a Perinatal Center. Good stuff! What changes do you predict in the immediate future? We are at a pivotal time. It is a time for all sonographers, across all modalities, to unite. We need to shore up both sides of our profession including
  • 14. SDMS News Wave  January 2012 14 mandatory certification and/or credentialing of sonographers as well as to pave a way for a mid-level practitioner to ensure our place in global health- care market. I feel that Point of Care sonography will continue to expand in untraditional venues. What advice would you give to students/ future sonographers? You make your own destiny. Get involved and let your voice be heard. I am the first to admit that all of you kids are WAY smarter than I was when I first started. You have the opportunity to mold our future. You have a lot to offer. Try and find a mentor to help you along the way. Tap into their vast knowledge and use your innate skills to refine that into a newer and fresh version that only your generation can do. And finally, remember that there is no room for mediocrity in medicine. Try your best...it will always serve you well. We are proud of you! What’s your favorite part of your job? I love clinical research and the time in the NICU scanning our premature infants. I love listening to our staff and graduates teach each other. To see them “pass it forward”not only with each other but also with the students makes me very proud of all of them. Maria Johnson is a first year student in the General Sonography program at El Centro College in Dallas, Texas. She is a mother of three children of which all are in college and developing their own professional careers. Maria was employed with AT&T for ten years where she worked as an account representative and manager. After her third child was born she was fortunate to be able to stay at home with her children. During that time she took the opportunity to become active in the community by volunteering in her children’s schools PTA and Boosters Clubs. We asked Maria why she decided to re-enter the workforce and why she chose diagnostic medical sonography, she replied“When my kids started looking into colleges I began to think about what my next steps would be in life. I knew I wanted to go back to work but I also felt that I wanted a career where I could make a difference. I had always regretted not getting into healthcare because I’ve been around nurses all my life, my mother and aunt were both nurses.”So she researched many different areas in healthcare and found that she was drawn to sonography due to the rapidly advancing technology as well as its importance in the diagnostic process. Maria said when she joined the SDMS; she was mostly excited about the free subscription to the Journal of Diagnostic Medical Sonography (JDMS). She said the JDMS is an important teaching tool both for her current classes and continuing education as a future sonographer. Maria said,“Overall, my favorite benefits of SDMS, is the vast information available to me on the website and in the JDMS. Between the two sources I have a plethora of valuable information from exam scheduling information to new technological advances and interesting pathology cases. This information has been invaluable to me as a new sonography student and makes me very excited to be getting started in this amazing field.” Maria, welcome to the SDMS! New Member SCAN
