2. ENFERMEDAD DIARREICA
La diarrea se define como una
alteración intestinal en la que se
produce un aumento del número de
deposiciones (más de 3 al día) y de
su volumen (más de 250 g al día),
así como una disminución de la
consistencia de las heces (más del
75% acuoso). En ocasiones,
pueden presentarse otros
elementos en las heces como pus,
sangre o moco

sido demostrada clínicamente,
que tiene como objetivo
clasificar las heces de
acuerdo con 7 tipos, desde

diarrea. Gracias a la escala de
Bristol los pacientes pueden
definir sus deposiciones de
acuerdo con un tipo de los 7
que ofrece la escala.

5. epidemiologia
● Es la segunda causa de mortalidad infantil de menores de 5 años, ya
que provoca la muerte de 1,5 millones de niños cada año a nivel
mundial.
● En Colombia, la Enfermedad Diarreica Aguda (EDA) es un evento de
interés en salud pública de control nacional, que requiere acción
desde vigilancia y seguimiento rutinario con el objetivo de
caracterizar el comportamiento a nivel departamental y municipal,
para detectar situaciones de alarma de manera oportuna que sirven
para limitar y controlar el riesgo frente a una población.

6. epidemiologia
● Durante el 2022 la enfermedad diarreica aguda aumento con
respecto al periodo observado 2020-2021. La menor
incidencia se registró en el periodo IV y de 2020 agudizados
por la contención del virus SARS CoV 2 desde periodo III.
● Este aumento de casos se encuentra ligado a las diferentes
aperturas poblacionales, se identificó mayor aumento en
población menor de 5 años. Semana 19 boletín Epidemiológico 8
al 14 de mayo de 2022

8. DIARREA AGUDA
Su duración es inferior a 14 días. Si su inicio es
agudo, pero se mantiene de 2 a 4 semanas, se
denomina diarrea persistente. Suele ir acompañada
de síntomas como nauseas, vómitos, dolor
abdominal e incluso fiebre y puede entenderse como
una respuesta protectora frente a agresiones
intestinales.

La diarrea es un problema especial entre adultos y niños:
• Con mala higiene
• Viajeros a regiones tropicales y subtropicales
• Homosexuales
• Personas inmunosuprimidas
• Viven en ambientes poco higiénicos y se exponen
a aguas o alimentos contaminados.

persona a persona
● Ingesta de alimentos
● Agua contaminada

13. INFECCIOSAS
● También es denominada gripe estomacal.
● Aparece entre las 4-48 horas
● Síntomas como vómitos, dolor abdominal e incluso fiebre.
● Generalmente, es un proceso autolimitado, leve. (Ancianos y niños)
● El rotavirus: Produce hasta el 40% de las diarreas agudas en países
desarrollados. Su transmisión, más frecuente en los meses de
invierno, es fecal-oral. Debe tenerse especial cuidado en bebés de
entre 6 y 24 meses de edad, ya que tanto su morbilidad como
mortalidad en este rango de edad es elevada.
● El virus Norwalk: perteneciente a la familia de los Calicivirus, es
habitual agente causal de diarreas. Produce cefaleas y dolores
musculares, y se transmite por vía oral y por contacto entre
personas. Se producen importantes brotes, en lugares donde
conviven grupos de personas como residencias, hospitales,
colegios, cruceros, etc.

• Los gérmenes inicialmente
adheridos a la superficie del
enterocito penetran en
el epitelio intestinal donde se
multiplican.
• Salmonella
• Shigella
• Escherichia coli enteroinvasor,
• Yersinia
• Campylobacter
• Las heces presentan leucocitos
moco (respuesta inflamatoria del
colon) y podrán tener sangre si
hay afección de la submucosa.
Mecanismo enterotóxico
• Segregan enterotoxinas que se
adhieren a la superficie del
enterocito inhibiendo la entrada
de sodio y cloro en la parte
superior de la vellosidad intestinal
y favorecen la salida de sodio y
agua.
• Vibrio cholerae
• E.Coli enterotoxígeno,
• Citrobacter
• En este tipo de diarrea las
deposiciones serán líquidas
explosivas, sin sangre.
Mecanismo citotóxico
• Clostridium difficile
• E.coli enterohemorrágico
• Shigella
• En el colon, donde lesionan
el colonocito
• produciendo fiebre dolor
abdominal y deposiciones
con moco y sangre.

diarreas son producidas por
parásitos, debido a la ingesta
de agua contaminada por los
protozoos:
• Cryptosporidium
• Entamoeba histolytica
• Giardia lambia
fúngicas
Los hongos causan diarrea
de manera excepcional y de
forma más frecuente en
personas inmunodeprimido
• Cándida
• Aspergillus

Puede venir provocada por distintas causas.
• Producida por fármacos
• Debida a alergias
• intolerancias a alimentos
• Causada por intoxicaciones
• Relacionada con el estrés psicológico
• Originada por otras patologías
• Enfermedad isquémica del intestino
• El cáncer de intestino
• Tumores secretores de hormonas
• Diarrea de sales biliares
• Enfermedad celiaca.

18. Diarrea crónica
Su duración es de más de 4
semanas. Las causas son muy
diversas y en muchas
ocasiones surge como
consecuencia de otra
patología.

DIARREA INFLAMATORIA.
Las características principales son la
presencia de fiebre, sangre o
leucocitos en heces y lesión
inflamatoria en la biopsia intestinal.
Ocasionan diarrea inflamatoria
patologías como:
● Las neoplasias intestinales
● La enfermedad inflamatoria
intestinal
DIARREA OSMÓTICA.
Dan lugar a diarrea osmótica
patologías como
● La ausencia congénita de
enzimas o de proteínas
● Trastornos pancreáticos.

20. fisiopatología
DIARREA SECRETORA O
ACUOSA.
Está asociada a patologías como
● La colitis colágena.
● La resección y derivación del
íleon terminal
● Colitis linfocítica.
● Mastocitis sistémica.
● Diarrea colérica o síndrome
carcinoide
DIARREA POR ALTERACIÓN DE LA
MOTILIDAD INTESTINAL:
Se debe al aumento del tránsito
intestinal, asociado en ocasiones a
sobrecrecimiento bacteriano. Puede
aparecer alternada con
estreñimiento. No cede con el ayuno
