PHYSICAL EXAMINATION 
GENERAL: Patient is in no acute distress. 
HEENT: Pupils are equal, round, and reactive to light. 
Mucous membranes are moist. There is no pharyngeal erythema. 
NECK: Supple without lymphadenopathy. 
LUNGS: Clear to auscultation bilaterally. No foreign sounds . 
CARDIOVASCULAR: Regular rate and rhythm. No rubs, murmurs or 
gallops. 
ABDOMEN: Soft, nontender, and nondistended. Bowel sounds are 
present. 
EXTREMITIES: There is no clubbing or edema. 
NEUROLOGIC: Cranial nerves II-XII are grossly intact. Muscle 
strength is 5/5 bilaterally. 
SKIN: No rash noted 
AFFECT is normal
