2. WOUND
The wound is a break in the integrity of the skin or tissue often
Which may be associated with distribution of the structure and function.
Wound is simply a distribution of any tissue soft tissue or bone or internal organs
3. CLASSIFICATION OF WOUNDS.
1. The classification of based on the Rank and Wakefield
a. Tidy wound.
Thy are wound like surgical incisions and wound coused by sharp objects .
a. Untidy wound -
They are due to.
Crushing.
Tearing.
Avulsion.
Devitalised injury.
Vascular injury.
Burn. etc.
4. 2. CLASSIFICATION BASED ON THE TYPE OF THE WOUND.
OPEN TYPE WOUNDS=.
● Incised wounds.
● Lacerated wounds.
● Crush injuries.
● Penetrating wounds.
● Gunshot injuries.
CLOSED TYPE WOUNDS=
● Contusion.
● Abrasion.
● Heamatoma .
3. CLASSIFICATION BASED ON THE THIKNESS OF THE WOUND.
a. Superficial wound.
b. Partial thikness.
c. Full thikness.
d. Deep wound.
e. Complicated wound.
f. Penetrating wound.
5.
6.
7.
8. 4. CLASSIFICATION BASED ON THE COMPLEXITY.
a. Simple.
b. Complex.
5 . CLASSIFICATION BASED ON THE TIME ELAPSED.
● ACUTE WOUNDS.
● CHRONIC WOUNDS .
6 CLASSIFICATION BASED ON THE SURGICAL WOUNDS .
● Clean wound.
● Clean contaminated wound.
● Contaminated wound.
● Dirty infected wound.
9.
10.
11. WOUND HEALING _
● It is the repairing of the wound.
● It is also known as new cell growth of after the wound for the repairing.
HEALING PATHWAY_
1. Haemostasis .
2. Inflammation.
3. Proliferation.
4. Matrix synthesis.(collagen and proteiglycan ground substance).
5. Maturation.
6. Remodeling.
7. Modeling.
12. TYPE OF WOUND HEALING _
PRIMARY HEALING (PRIMARY INTENTION)_
These types of wound are heals rapidly with complete heal.
It accures due to the clean incised wound or surgical wound.
● The scar is linear smooth..
SECONDARY INTENTION ( SECONDARY HEALING)
● It is accures due to the major trauma , burn and wound with sepsis.
● The scar is wide and it's often the hypertrophied and contracted .
● It heals slowly and fibrosis.
● The re _epithlisation occurs from remaining dermal elements or wound margins.
THIRD INTENTION _
It is the control of local infection wound is closed with sutures or covered using skin graft .
● The primary contaminated or mixed tissue wound heal by tertiary intention
13. Phase of wound healing _
Inflammatory phase (Lag or substrate or Exudative phase)_
The features of acute inflammation _. ( Rubor, calor tumour, dolor and loss of function.
● Here haemostasis, coagulation and chemotaxis occur.
Proliferative phase (collagen/fibroblastic phase)
● It begins from 3rd day last for 3_6 weaks.
● Growth of new blood vessels.
● In this phase the formation of granulation tissue and repair of the wound.
REMODELING PHASE (MATURATION PHASE)
● It begins at 6 weaks and last for 6 months to 1 or 2 years
● It is the maturation of collagen by cross linking and realignment of collagen fibres along the line of tension.
●
14. Factors affecting wound healing_
● Stress.
● Ischaemia
● Diseases, Diabetes, keloid, fibrosis, hereditary, healing.
● Obesity.
● Medication _ Glucocorticoid, steroidal, non_steroidal anti_inflammatory drugs.
● Alcoholism and smoking.
● Poor blood circulation.
Management _
● The airway should be maintained.
● Bleeding control.
● IV fluids are stored.
● Oxygen maintain.
● Deeper communication .
