I will share new results of countywide research defining the state of our health and wellbeing in 2018, and sharing some important changes emerging over the past year.
Let me start by sharing why PIHC is so committed to this work.
Our fast track session will highlight…
Any of theses questions, look familiar? Forget work…health and well-being questions like these are part of our everyday lives … at work or at home. You may be in a position to help a person challenged with homelessness, substance use, mental health, a chronic a disease, or someone at a particular stage of life, a senior, a pregnant mom or a family …. We know, is that people are not only their disease states or issue, but just people. People with lots of intertwined needs and desires. Thankfully, there is more recognition and support that the best care will probably require resources outside our specialty.
My high risk chronic disease clients were often best served by finding volunteering opportunities to find purpose and meaning, or help in finding job resources and training for better stability, ownership and control in their lives, or meditation resources for anxiety.
We are being asked to more to broadly and constantly and serve people more holistically because it can make a difference in better health outcomes – to think outside of our silo’s … a really good thing…and, a bit daunting too… even for a an experienced resource person and especially if out of our wheel house or capacity.
… Where do we find the answers? We might ask Google, ask a friend, learn from a colleague, keep file books, lists and sticky notes, your own vast wealth of inside scoop …
All great tools … and what if there was something a bit easier – to find and connect with the many community assets in Snohomish County that specifically support health and healing …
What if this was organized by what matters to our community – these 6 dimensions, name them, came from earlier work asking our community what health means….(HWBM) – important foundationally, because when messages resonate with people, it make a difference in driving self-improvement, health management and better personal and community health outcomes
What would it mean for communities to work together to gather, maintain and share information – giving community control of listing and content helps expand the depth, usability and equity of available resources.

In addition to hosting LWL for community use and design, a quick share of a few key program where LWL is an important foundation for action, and lifting up our local strengths and assets.
Examples
Live Well 360 – is a well-being plan for communities or individuals. Communities or individuals can start with the HWBM assessment, identify areas for action, set goals and take action to improve.
For example, with some the communities now engaging in using the HWBM, a need might in nutrition might lead to resources for community gardens, or for food insecurity may lead to on-site sign up for SNAP, financial classes or outreach to food resources/service providers
One of the United way Collaboratives … is using the HWBM to do individual client assessments, to support their work in improving school attendance, as needs are ID in the six well-being dimensions, coaches/navigators can link folks to resources already categorized in those dimensions.
Total Health
Health Ed….40 events, YMCA, Senior Centers, NNO, Nubian Jam, Elementary school, worksites…on a variety of health education topics from chronic disease to mental health. As folks are seeking to help or to improve, LWL provides opportunities to raise visibility/connect to our local community resources…
