I. PURPOSE
To establish a uniform procedure for the initiation, completion, and distribution of the Animal
and Facility Inspection Notice.
II. POLICY
It shall be the duty of all OC Animal Care personnel charged with the responsibility of
conducting Animal Permit and Business Licensing inspections to uphold all applicable
ordinances, regulations, and state and local laws pertaining to animal permits and animal
related businesses.
The Animal and Facility Inspection Notice shall be completed upon inspection when issuing a
new/renewal Animal Permit or Business License, or when investigating a complaint.
III. SCOPE
Applicable to all Business Licensing personnel involved in conducting Animal Permit and
Business Licensing inspections.

A. Form Completion (refer to the numbered sample)
1. Facility Information:
a. Enter the facility’s name and address.

2. Owner:
a. Enter the owner’s name and address.
3. Workers Compensation:
a. Select the applicable box: Contract City or Unincorporated Area.
b. Select the applicable box for employees: Yes or No.
c. Select the applicable box for Workers Compensation Verified: Yes or No.
4. Type of Inspection:
a. Mark the applicable box to indicate the type of inspection.
5. Type of License:
a. Select the applicable box for the type of business or animal permit.
b. Enter the California Department of Fish and Wildlife permit number if
applicable.
6. Inspection Results:
a. Select the applicable box for inspection results.
7. Section
a. Enter the section number of the violation.
8. Title:
a. Enter the title of the violation with a brief explanation of the discrepancy.
9. Re-inspection date:
a. Enter the date discrepancy is to be corrected by.
NOTE - Advise the licensee that they may be charged a re-inspection fee if the
discrepancy is not corrected by the re-inspection date.
10. Business License Inspector:
a. Enter the inspecting officer’s name and badge number.
b. Enter the date of the inspection.
11. Signature:
a. Obtain the signature of the licensee or designee. If neither is available, mail the
canary copy to the business address or permit holder’s home address, noting that
the licensee or designee was not available for signature.
