• The following slides are averaged 24 hour blood glucose curves, measured at days
24 to 28 of treatment (yellow curve), and then again at days 52 to 56 of treatment
(green curve). The baseline curve (dark blue) is actually an average of 8 days of
data (two separate 4 day periods separated by 6 days).
• All slides have the same scaling, where the vertical access indicates plasma glucose
levels in mg/dl, and the horizontal access is time of day starting at midnight and
finishing at midnight.
• There are a couple of key things to pay attention to as we go through the slides:
– Look to see if the treatment curves, yellow at 4 weeks and then green at 8
weeks, generally fall at or below the baseline blue curve, showing that we did, or
did not, help manage blood glucose levels.
– When someone already has well controlled blood glucose levels, falling
somewhere between 100 and 150 mg/dl, were we able to maintain this control
while at the same time dramatically expanding their world of food?
– Recognize that for some people, we did not help manage glucose levels (about
15% of participants)
– Finally, remember that these people had, on average, 45 minutes of instruction in
how to use the system, they were provided a spiral bound book with about 1,000
foods, and given a Palm Pilot PDA (monochrome display) which had a food
“look-up” function for those who could actually use a Palm Pilot. There were no
support groups, support lines, 1-800 numbers to call. Participants had a
refresher, if needed at 4 weeks which was typically 15 minutes induration. These
results, which are typical, followed a net intervention of about one hour.

First, notice that this person had levels exceeding 300 mg/dL
early in the morning, for an extended period of time, which we
have been able to drop by about 50 points, and dramatically
shorten in duration. Second, whereas the majority of the waking
hours were spent above 200 at baseline, the majority of the
waking hours are below 200 when using the Thrive 365 system.

In this case, the individual had reasonably well controlled glucose
levels, falling between 100 and 150 mg/dL. While we lowered
levels to around 100 during sleep, it would be fair to say that we
did not improve glucose control during the day, but essentially
maintained the control already established. However, we did this
while simultaneously expanding food choice, and making diet
management much simpler.

This individual we did not seem to be able to help at all. Blood
glucose levels were high and bouncing everywhere at
baseline, and seemed to stay that way for the full 8 weeks.
Difficult to say why this might have been the case, but highlights
the point that no treatment or diet regimen is perfect for
everyone.

This individual was doing a decent, but not great, job of
managing his/her glucose levels, with three problem
areas, possibly around meals (averaging over 200 mg/dL in the
mid-afternoon). However, at the end of the 8 week period (green
curve), glucose levels were staying primarily within the 100 to
150 mg/dL range (with a slight dip below those levels during the
night).

At first blush, this may look like we had no real impact on glucose
levels. But if you look more closely, you will see that even
though this person had very well controlled glucose levels, we
lowered those levels even further throughout the day, with just a
slight bump around 10 or 11 am. This is one of my favourite
slides, simply because it shows that we can help people who
thought that they had things well under control, and make their
lives simpler while expanding their food choices, all at the same
time.

For this person, I would argue that we did not improve, nor did
we worsen, their glucose management. However, repeating what
I have said before, managing their diet was much easier.

Finally, the improvement for this individual is dramatic at the 4
week time-point (they withdrew from the study before the 8
week measures for an unrelated medical issue). Notice how
stable the glucose levels are throughout the night. And during
the day, when previously the levels would rise above 200 mg/dL
and stay there for hours at a time, eating per the Thrive 365
system helped to keep the levels between 100 and 150
mg/dL, on average.
