WHAT IS THE LEGACY FOUNDERS
COTTAGE ?
LFC is a licensed care facility that provides
transitional housing for respite and hospice
individuals with HIV/AIDS.
2. Respite vs Hospice
Respite care – is care for individuals that need
assistance with daily activities such as bathing,
feeding, medication management, and appointment
compliance OR providing a caregiver break OR if a
homeless client has housing set up and needs a
stable place to go a month or two before moving in.
Hospice care - is humane and compassionate care for
people in the final phases (6 months or less) of an
incurable disease so that they may live as fully and
comfortable as possible and the focus is on pain
management and symptom control.
3. What We Provide
Legacy Founders Cottage provides the following services:
24 hour certified nurse aid care (not skilled nursing)
3 meals daily + evening snack
Cooking, cleaning, and laundry
Private room for clients with cable and a shared phone line
Psychosocial and Housing Case Management
Housing services: assistance with housing placement for
those who are ready to transition into independent living.
Program Director (PD) ensures that all psychosocial needs
are met.
Medication and adherence education
Independent living skills training (i.e. budgeting and saving,
anger management, sobriety management)
Mental health, IOP, and SOP counseling from Legacy
Counseling Center
4. Legacy Founders Cottage is NOT:
A bed and breakfast: respite clients must be willing to follow a
care plan with both HSC (Housing Services Coordinator) and PD
(Program Director). Must work toward goals of getting healthier,
take their medications, attending doctors/housing appointments
when able to, and follow house rules.
Institution (applies to both hospice and respite clients):
Substance Abuse Rehab: Drugs and alcohol are not allowed on premises and
random room sweeps will take place if there is reason to believe that they are
in the Cottage.
Skilled Nursing Facility: LFC only has CNAs (Certified Nurse Assistants) on
staff—no RNs, LVNs, Physicians, or Physical Therapists work at the facility, but
can be brought in through home health or hospice companies.
In-patient Psych Rehab: clients who are diagnosed with a major/serious mental
illness, must be in psych care and stable on medicines for at least 30 days.
Permanent Housing: LFC is a transitional housing facility where
clients stay 3-6 months. Stays can be extended only if clients are
still working toward reaching their goals and still need extra
assistance. This is a case-by-case situation.

*Individuals who are not HIV/AIDS
positive.
*Individuals who have ports,
tubes, and catheters unless
supported by a home health
agency.
*Individuals who may need more
medical assistance than a CNA
can provide.
*Individuals actively using drugs.
*Individuals who are registered
sex offenders or have felony
convictions for manufacturing
meth.
6. Steps to Getting Client
into LFC
Step 1: Contact Housing Services Coordinator (HSC)
to discuss clients situation
Step 2: client has to be referred by a case manager
at an AIDS Service Organization (ASO). Client will
contact case manger to sent over paperwork.
Step 3: HSC will set up assessment with client to
meet face-to-face and complete assessment to
determine if client is appropriate
Step 4: If client is appropriate, HSC will gather
documentation from the client and the case
manager to complete the client’s file and get
approval from Program Director
Step 5: Move in when a bed becomes available.

9. Misc. LFC Info
• We do not bill Medicare/Medicaid/private
insurance. Clients who are selected into Legacy
Founders Cottage are not required to have
income, but if they do HUD requires that they
contribute 30% of their income towards their
rent.
• Clients may be documented or undocumented
• Clients need access to medicine and medical care
• For respite clients— they need to have a housing
plan for where they are going to go after 3-6
months is up.
• Our waitlist moves relatively quickly and is based
off of medical need. Hospice clients always take
priority.
