ACCOMPLISHMENT REPORT AND INTERVENTION PLAN IN SCIENCE 5 QUARTER 2
Least Mastered Skills: Describe the different modes of reproduction in animals such as
butterflies, mosquitoes, frogs, cats and dogs.
I.RATIONALE
Based on the least mastered skills identified in Science Quarter 2, pupils encountered
difficulty in learning the subject. Teachers need to provide intervention activities to help pupils
learn the necessary skills that they must acquire in order to have effective learning outcomes.
Instructional materials are highly important for teaching, especially for inexperienced teachers.
Teachers rely on instructional materials in every aspect of teaching. These materials are also
needed by teachers to assess the knowledge of theistudents.
In learning competency (Describe the different modes of reproduction in animals such as
butterflies, mosquitoes, frogs, cats and dogs), Grade 5 learners must improve their mastery of the
lesson not only by learning but also by doing performance task and comprehension. To do this,
the teacher was able to have remedial teaching and re-assessment from the intervention made
which is appropriate to their level.

III. IMPLEMENTATION
The teacher exerted time and effort to be able to help the learners to master the lesson,
to improve comprehension skills/ performance task. The teacher used different pictures and
diagram to identify the mastery in this competency. The teacher also provides varied activities
related to the topic to be able to understand and mastered the lesson well. He/she encouraged the
pupils to participate and engaged their learnings about the topic.
