This document outlines 15 issues that can fracture the chain of survival in resuscitation efforts. These issues include problems with detecting agonal breathing and pulses, low rates of bystander CPR, incorrect dispatch instructions, chest compressions being too slow, fast, or shallow, interruptions during compressions, overinflating lungs or hyperventilating, delays and interruptions, and ineffective resuscitation techniques.
