The survey and benchmarking process are repeated every 2 years to evaluate changes over time. From

data collection tracking system, development of screening scripts, and implementation of the
electronic and paper surveys
All aspects of data collection including training, screening and reminder telephone calls, survey
mailings, data entry of the paper surveys, and final database cleaning and preparation for the client
Achieved response rates of 82% (2007 and 2009) and 83% (2011) for this combination web/paper

Managed web and paper surveys in support of health research studies for the National Institute of
Environmental Health Sciences (NIEHS), the Centers for Disease Control and Prevention (CDC), and the
King County Health Department:
Managed quantitative data collection tasks, which included a web/paper survey of grantees
Data collection coordinator responsible for the creation of data collection materials and protocols,
the design of the data collection tracking system, implementation
Achieved a final response rate of over 60% for the one of the CDC studies
